Le point de vue dépend du regard de différents acteurs sur cet individu : soignant, institution psychiatrique et société. En se basant sur l'hypothèse que l'utilisation du recours à la contention physique résulte d'une mauvaise articulation entre les valeurs de ces acteurs, il s'agit de se demander : Le recours à la contention physique est-il un « mal nécessaire » ? Les justifications de cette pratique se contredisent. Tout d'abord, deux des trois niveaux de jugement médical s'opposent sans entrer dans une logique dialectique. La contention physique peut être contraire à la responsabilité individuelle, mais conforme à la responsabilité institutionnelle et collective. Ces contradictions s'expliquent par les interrogations autour de la finalité de l'acte. Il existe une lecture déontologique et une approche conséquentialiste pour répondre à cette question : la contention physique, mesure sécuritaire ou acte de soin ? Ces antagonismes conduisent à bloquer le débat : politisation paralysant la décision législative, discorde entre les soignants, une société partagée entre logique sécuritaire et respect des droits. Le blocage vient aussi du caractère tabou de la contention physique visible par une faible connaissance des soignants sur ce sujet, par un corpus scientifique mince et des regards extérieurs rares. Ces blocages résultent de multiples traumatismes, en particulier chez les soignants et les patients. Pour l'étude empirique, la méthode qualitative s'impose car la parole reste peu libre. L'analyse des résultats fait apparaître une hétérogénéité locale favorisée par un cadre légal lacunaire. Des structures vont voir dans la contention physique un mal nécessaire à encadrer, d'autres vont chercher à l'éradiquer. L'étude montre que le débat devient plus ouvert mais d'une qualité médiocre, il a permis des avancées législatives et une prise de conscience, contrebalancées par des reculs. La culture d'établissement détermine fortement la nature du débat et les réponses apportées sur la nécessité de la contention physique.

Bien que son utilisation se soit considérablement humanisée, l'éthique de cette procédure suscite encore des interrogations. Les réponses apportées dans la littérature sont succinctes et ont tendance à prêter à la contention des vertus thérapeutiques. Or, aujourd'hui, la nature thérapeutique de la contention n'est source d'aucun consensus. Son utilisation reste pourtant nécessaire dans certaines situations d'urgence où les troubles présentés constituent un danger immédiat pour le patient ou pour autrui. La contention est alors justifiée pour des motifs de protection ou comme moyen de mise en œuvre de soins urgent, lorsque aucun autre moyen n'est possible. En France, les recommandations concernant la contention physique en psychiatrie font encore défaut et l'on est amené à extrapoler des référentiels voisins ou à exploiter des documents émanant d'autres systèmes de soins. L'un des critères de qualité des soins est le recueil du vécu subjectif des patients contenus. Une enquête a donc été menée dans un service d'urgences psychiatriques pour étudier le vécu de ces patients. 26 patients ont été interrogés, à l'aide d'un questionnaire structuré, afin de déterminer les aspects les plus pénibles de la contention. La peur, la colère, le sentiment d'agression sont les aspects négatifs les plus fréquemment rapportés derrière l'inconfort et les sensations douloureuses de la contention physique. 12 patients ont pu être recontactés à distance. 3 d'entre eux présentent des symptômes de stress post-traumatique mesurés sur l'échelle d'événement traumatique d'Horowitz (IES-R). Au travers de cas cliniques, les limites du modèle du psycho-traumatisme en rapport avec la contention sont discutées, compte tenu de la présence d'une autre pathologie psychiatrique et de l'intervention d'événements traumatiques extérieurs à la procédure.

Sa recrudescence dans les services psychiatriques constatée depuis les années 2000 la replace aujourd'hui au centre des préoccupations du personnel soignant et des usagers, mais aussi des législateurs et des politiciens. L'objectif de cette étude épidémiologique descriptive transversale du vécu soignant, jusqu'alors peu exploré, est de mettre en exergue le positionnement des soignants confrontés à cette pratique. Les résultats montrent une charge émotionnelle intense, avec dominance de la culpabilité, de la frustration et du sentiment d'échec. Cet acte reste néanmoins considéré comme difficile mais nécessaire. Une volonté de changement est perceptible, avec émergence de voies d'amélioration, comme la généralisation des formations à la gestion de la violence et l'ouverture de temps de parole en équipe autour des expériences de mises sous contention. Au vu des résultats et par l'observation d'autres modèles d'organisation des soins, une réflexion sur l'usage de ce moyen de contrainte semble incontournable, intégrant une dimension éthique. Les recommandations récentes et la loi en vigueur visent la réduction de la contention, désignée comme dernier recours, et une meilleure traçabilité de son utilisation. Accorder les pratiques des établissements de santé mentale à ces aspirations ne pourra se faire sans inclure les soignants ni leur en offrir les moyens humains et matériels.

Il est nécessaire de considérer les protocoles concernant l’utilisation des mesures de contraintes et de poursuivre les recherches dans ce sens-là.De ces divers résultats de notre revue de littérature émergent des habiletés relationnelles dont l’importance n’est plus à prouver : les compétences en communication, l’approche empathique et la collaboration soignant-soigné sont indispensables dans la prise en soins.

Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools?
