La Pharmacie Clinique connaît un développement et devient en janvier 2016 grâce à la loi n°2016-41 une des missions essentielles des pharmaciens hospitaliers. La Conciliation Médicamenteuse, discipline appartenant à la Pharmacie Clinique, évolue chaque jour et trouve sa place dans de nombreux services hospitaliers. Elle tend aujourd'hui à se généraliser grâce à l'expérimentation Med'Rec et la publication d'un guide par la Haute Autorité de Santé (HAS) en décembre 2016. L'étude présentée dans cette thèse a ciblé le processus de Conciliation des Traitements Médicamenteux des patients lors de leur admission dans plusieurs services du pôle Médecine du CHRU de Tours. Les différents paramètres nécessaires à sa réalisation ont été étudiés afin de déterminer les moyens étant nécessaires à la mise en place dans les services où la Conciliation Médicamenteuse n'est pas encore réalisée. L'étude a permis de préciser les modalités de réalisation de la Conciliation Médicamenteuse dans les services de Médecine au CHRU de Tours : la durée de réalisation (inférieure à 1 heure en moyenne) ; le nombre de sources consultées (2,3) ; la réalisation de la conciliation dans les 24 heures suivant l'hospitalisation (73%) ; le nombre de divergences par patient (6,9) ; le nombre d'erreurs médicamenteuses (0,3). Cette étude a également permis de mesurer l'activité de Conciliation Médicamenteuse des services du pôle Médecine du CHRU de Tours grâce aux différents indicateurs proposés par la Haute Autorité de Santé (HAS) et de la comparer à d'autres établissements de santé. De ce travail ressort un impact clinique positif de la Conciliation Médicamenteuse d'entrée et un probable bénéfice économique qu'il faut quantifier dans les années à venir.

L'un des points critiques en terme de sécurisation se trouve aux points de transition dans le parcours de soin. La moitié des erreurs médicamenteuses et 20% des effets indésirables médicamenteux sont imputables à un défaut de communication aux points de transition du parcours de soin. Le procédé retenu pour assurer la meilleure sécurité médicamenteuse possible à ce niveau a été appelé Conciliation médicamenteuse (CM). Cette démarche permet de formaliser la liste des médicaments pris et à prendre par un patient à un moment donné. La CM n'est pas adapté à certaines situations cliniques. Nous nous sommes donc intéressés à la CM proactive, qui prend place avant la rédaction de l'ordonnance médicamenteuse à l'admission. Cest dans le but d'évaluer la faisabilité à long terme de la CM proactive, que nous avons procédé à sa mise en place au sein de plusieurs services de chirurgie du CHRU de Tours. La CM proactive permet une conciliation plus rapide en comparaison d'une CM rétroactive. Elle permet également de s'affranchir d'une période de non-conciliation entre l'admission du patient et la réalisation de la CM rétroactive, et ainsi elle pourrait permettre de limiter le risque de iatrogénie médicamenteuse liée à des interactions médicamenteuses. Les patients et les prescripteurs sont majoritairement satisfaits du système mis en place.

It is the first translation of the work into a European language and provides unique access to this formative period in Chinese history. Because Yang Hsiung's interpretations drew upon a variety of pre-Han sources and then dominated Confucian learning until the twelfth century, this text is also a valuable resource on early Chinese history, philosophy, and culture beyond the Han period. The T'ai hsüan is also one of the world's great philosophic poems comparable in scale and grandeur to Lucretius' De rerum naturum.
