Book excerpt: Introduction : L'exigence croissante de la société d'une meilleure qualité de soins entraine une reconsidération des compétences médicales en particulier celle de communiquer et d'entretenir une relation médecin-patient de bonne qualité. Dans cette logique, l'enseignement du DES de médecine générale a évolué et s'articule autour d'une approche par compétences. Les méthodes d'évaluation qui s'en rattachent sont au cœur des préoccupations actuelles. La communication professionnelle médicale représente une compétence fondamentale en médecine en général et en médecine générale en particulier. L'élaboration des outils d'évaluation de cette compétence passe par la caractérisation de niveaux progressifs d'acquisition. Un groupe national expert du CNGE a déjà proposé des hypothèses d'indicateurs servant à définir trois niveaux d'acquisition: Novice, Intermédiaire, Compétent pour la compétence "Communication, relation, approche centrée patient". Cette recherche vise à caractériser en particulier le niveau Compétent et à comparer les résultats obtenus aux hypothèses du groupe expert. Matériel et méthodes : La méthode de recherche qualitative est celle des focus-group. Trois focus-group ont été constitués. Le recueil de données a été réalisé à partir de la retranscription des trois focus-group à partir d'un enregistrement audio et vidéo des entretiens. La méthode d'analyse des données est celle d'analyse de contenu thématique des discours. Résultats :Les résultats décrivent un niveau compétent observé partiellement conforme au niveau Compétent prévu par le CNGE.Meme si l'on peut noter une évolution favorable depuis le niveau Novice , beaucoup d'éléments attendus semblent ne pas être maitrisés par les étudiants relevant du niveau Compétent. Conclusion : Malgré certains points non résolus, cette étude originale peut servir de travail.

Evaluer le niveau de compétences nécessite l’utilisation d'outils d’évaluation qui renseignent valablement sur le niveau atteint ou en cours d’acquisition.L’objectif principal est double : déterminer les attributs d’un interne de niveau « novice » pour la compétence : « relation, communication, approche centrée sur le patient » et les comparer avec les hypothèses du groupe national d’experts ayant travaillé sur ce niveau. L’objectif secondaire est de repérer des moments déterminants de développement de ce niveau.Trois focus groupes ont été réalisés auprès d’internes débutant leur DES de médecine générale à l’UPEC. Ils ont été retranscrits puis ont fait l’objet d’une analyse thématique avec triangulation des données, tout d’abord sans grille de lecture prédéterminée, puis avec la grille de lecture à partir des conclusions du groupe national d’experts. Le Novice est apparu plus avancé que prévu dans la maitrise de cette compétence notamment pour la communication envers les intervenants et l’entourage du patient alors que certaines connaissances théoriques attendues ne sont pas intégrées. Sa communication se limite aux explications et la recherche du consentement n’est pas systématique. Il identifie des situations difficiles du point de vue de la communication pour lesquelles il élabore quelques stratégies afin de pallier à ses difficultés, sans avoir le recul pour les résoudre. Le niveau concernant le non-verbal est hétérogène, probablement en rapport avec différents niveaux d’introspection.Cette étude originale a permis de valider les hypothèses du groupe national d’experts et d’ébaucher une description des attributs du niveau Novice pour cette compétence comme travail préliminaire à une étude sur les traces d’apprentissage. Ses implications sont importantes pour adapter la formation en deuxième et troisième cycles
