Il s'avère en effet que lorsque le choix d'une prise en charge ou d'une fin de vie au domicile est fait par le patient, accepté par les proches et accompagné par des professionnels compétents et coordonnés, l'impact sur la qualité de vie des patients est indéniable, et les personnes malades plébiscitent ce type de dispositif. Destiné non seulement aux professionnels médicaux et paramédicaux mais aussi bien aux lecteurs investis dans l'organisation en santé (administrateurs, politiques, chercheurs et patients), cet ouvrage, rédigé dans un langage accessible au plus grand nombre, décrit une des expériences qui, fortes de quinze années de fonctionnement, ont montré que les alternatives à la prise en charge purement hospitalière respectent dignité des patients et sécurité des soins.

Grâce à ce mode de prise en charge et à la formation des MG, la réalisation des chimiothérapies à domicile commence à se développer. Les expériences rapportées (Centre Léon Bérard à Lyon) et l'analyse critique des textes réglementaires ont permis de démontrer la faisabilité des chimiothérapies à domicile dont la qualité et la séciurité des soins sont garanties par les réseaux de soins. Le Plan Cancer intègre d'ailleurs dans ses objectifs, le développement des réseaux afin de permettre le maintien du patient à domicile y compris pour des actes techniques comme les chimiothérapies. Nous avons réalisé deux enquêtes auprès des MG du secteur sanitaire de ROSA : une évaluait les pratiques des MG du réseau sur l'année 2004 ; une autre interrogeait les MG sur leur désir d'implication dans les réseaux de soins et dans le développement des chimiothérapies à domicile. Les MG s'accordent pour définir leur rôle de coordonnateur des soins dans le cadre de réseaux afin notamment d'optimiser leurs compétences au service de leurs patients. Les nouvelles modalités d'organisation des soins autorisent aujourd'hui à "penser" des actes spécifiques au domicile comme la chimiothérapie, et redonnent au MG toute sa place dans le circuit de soins.

Why did we or ganize such a symposium on supportive care in cancer patients? Recent decades have witnessed remarkable success in cancer treat ment, and we have learned how to cure a finite number of neoplas tic diseases. Some malignant tumors that previously entailed high fatality rates, such as leukemias, lymphomas, and testicular can cers, can now be cured, even when at an advanced stage. Yet it seems to many that our struggle to improve results and to fight death from cancer has also imposed greater toxicity on patients. Conventional scientifically based oncology has only recently made adequate efforts to improve the subjective quality of life of cancer patients, for example through prophylaxis against emesis, nausea, and scalp hypothermia, pain control and the development of psy chosocial support structures. The search for less toxic and yet equally effective treatment measures has not been one of our pri mary goals in the past. Supportive care has always been part of nurses' professional aim, even though many have not known how best to offer it.
