Ce trouble spécifique du développement du langage oral touche environ 1 % des enfants. Nous avons souhaité faire le lien entre perception et production de la parole grâce à des tests de lecture et répétition de mots. Nous avons attaché une importance particulière à la perception des trois traits articulatoires : voisement, lieu et mode d’articulation. La 1ère étude a révélé l’existence d’un déficit important, de la perception de la parole dans le silence et dans le bruit chez les enfants dysphasiques expressifs. Notre objectif ensuite a été de vérifier si ce même déficit perceptif se manifeste également chez les enfants dysphasiques réceptifs. Un déficit de perception de la parole dans le bruit, est apparu pour l’ensemble des dysphasiques, avec une prédominance chez les expressifs. Dans un 3ème temps nous avons voulu vérifier si ce déficit se manifeste de façon identique quand on change la procédure de réponse. Nous avons alors à nouveau évalué l’identification des séquences de type voyelle/-consonne-voyelle/ dans le silence et dans le bruit, mais en modifiant le protocole de réponse. Nous avons supprimé toute lecture possible et avons utilisé un paradigme fréquemment employé en psychophysique, la procédure AXB. Nous retrouvons les mêmes déficits, soit un déficit significatif dans le bruit, concernant particulièrement le voisement. Enfin, n des marqueurs de déviance des dysphasies est le trouble de l’évocation lexicale qui pourrait s’expliquer par un déficit de la capacité de segmentation. Nous avons donc étudié le mécanisme de segmentation d’un flux de parole chez l’enfuant dysphasique, en cherchant à savoir si ce processus se déroule de manière identique à celui observable chez l’enfant témoin.

Book excerpt: Nous nous somme intéressés au langage oral des enfants atteints de dysphasie phonologico-syntaxique. Si de nombreuses études ont été menées sur le plan de la morpho syntaxe, la phonologie reste un domaine où la littérature est peu abondante. Nous avons voulu identifier les stratégies utilisées pour pallier à la difficulté d'accès à la complexité phonologique (Van der Lely 2007, Maillart & Parisse 2004). Les études en morpho syntaxe (Damourette 2004) montrent l'apparition de certaines stratégies spécifiques, qui nous ont interpellés sur cette potentialité en phonologie. Enfin, l'éventuelle similarité avec des enfants sans pathologie, de même âge phonologique, nous a interrogés.Nous avons réuni treize corpus de langage spontané d'enfants avec dysphasie de 6;6 à 12 ;11 ans, et treize corpus d'enfants de quatre ans. Ces corpus sont composés d'une soixantaine d'énoncés. Ils ont été transcrits grâce au logiciel Chi Ides (MacWhinney 2000), analysés et comparés sur le plan de la complexité segmentale (présence de/l/, /r/, /s/, /j/, /w/, ɥ/, de la complexité syllabique (positions et structures dans la syllabe), de la longueur de mots, et des erreurs commises. Les résultats montrent un nombre plus élevé d'erreurs chez les enfants avec dysphasie, et certaines qui n'apparaissent pas (ou peu) chez les enfants de quatre ans, comme les distorsions et les métathèses. On observe une grande sensibilité aux complexités segmentales et syllabiques, ainsi que des résultats hétérogènes. Le nombre d'erreurs est moins important que lors des tests de langage induit: les stratégies d'évitement seraient donc efficaces. Nos deux groupes présentent certaines similitudes qui nous font penser que la dysphasie pourrait être un retard de développement phonologique plus qu'un trouble spécifique, mais cela ne se vérifie pas à tous les niveaux. L'étude des erreurs et des positions amène des arguments en faveur de la sous-spécification des représentations phonologiques. Nos résultats sont similaires à ceux retrouvés en productions induites mais en quantités différentes, et à ceux de la morphosyntaxe. Ce domaine et la phonologie seraient donc liés. Cette étude pourrait permettre un diagnostic de la dysphasie précoce et ciblé, ainsi qu'une rééducation axée sur les déficits phonologiques réels.

The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training.
