De plus, malgré les nombreux bénéfices et recommandations ce sont les femmes en situation de précarité qui ont le taux d'allaitement le plus bas. Méthode : Il s'agit d'une étude qualitative, basée sur des entretiens semi-dirigés auprès de dix femmes en situation de précarité de la région lyonnaise. La totalité des entretiens a été retranscrite et nous avons réalisé une analyse transversale des données.Résultats : Le choix de l'allaitement répond à de multiples facteurs. L'origine ethnique et culturelle est un facteur déterminant. De plus, même si elles considèrent le lait maternel comme supérieur aux préparations pour nourrissons comme en témoignent les nombreux avantages qu'elles lui attribuent, elles en pondèrent cependant les bénéfices devant les freins et les difficultés rencontrés. Ces derniers ne sont pas spécifiques, mais sont parfois suffisants pour compromettre la poursuite de l'allaitement. Ainsi, c'est le soutien reçu qui va faire la différence. Elles le trouvent spontanément auprès de leur entourage familial et amical. Les professionnels de santé ne sont sollicités qu'en deuxième intention et n'offrent pas pleine satisfaction. Leur premier recours reste les centres de Protection Maternelle et Infantile pour leur proximité, leur facilité d'accès et leur équipe pluridisciplinaire.Conclusion : C'est sur la qualité de l'information délivrée avant l'accouchement, les pratiques hospitalières et le réseau de soutien social qu'il faut œuvrer pour améliorer le taux d'allaitement dans cette population.

La durée moyenne de l'allaitement maternel en France est loin des recommandations de l'OMS. Cette étude a exploré la prise de décision et le vécu de femmes françaises qui prolongent l'allaitement au-delà de 6 mois, pour améliorer l'accompagnement des femmes par les soignants. Etude qualitative avec entretiens individuels semi-structurés de l7 femmes primipares recrutées dans des cabinets de médecine générale, des centres de PMI et des associations d'allaitement dans la région Rhône-Alpes (France). Le bénéfice santé attendu pour son enfant est la motivation principale d'une femme pour allaiter. Un vécu maternel positif avec des sentiments de plaisir partagé et de création d'un lien mère-enfant est la raison principale pour prolonger l'allaitement. Les femmes recherchent des informations sur l'allaitement mais se limitent à des sources d'information faciles d'accès. La confiance et l'autonomie maternelle dans l'allaitement sont contrastées avec une vulnérabilité sous-jacente. Les femmes sont influencées par la norme sociétale et l'allaitement d'un nourrisson qui a atteint l'âge de la marche et la parole devient moins évidente. Une personne-ressource assure le soutien de la femme allaitante sous forme de compagnonnage. La décision d'allaiter se différencie de la décision de poursuivre l'allaitement. Les décisions d'allaiter et de poursuivre d'allaitement sont propres à la femme allaitante mais influencées par l'entourage. Les soignants ont un rôle limité mais important sous forme de promotion et soutien de l'allaitement.
