Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

BACKGROUND AND AIMS: Mental health literacy has been defined by Jorm, et al as the u201cknowledge and beliefs about mental disorders which aid their recognition, management or preventionu201d. 1 It is known that mental illness stigma represents an important barrier to providing a high quality of mental health care, contributing negatively to the quality of life of the mentally ill, their relatives, and health service providers. (Sartorius & Schulze, 2005). Little discussed is the fact that mental illness stigma exists as well among healthcare professionals, leading to the underdiagnosis and neglect of physical symptoms, which ultimately results in poor outcomes for these patients. 2MATERIAL AND METHODS: The study was a cross-sectional descriptive survey conducted among a group of clinicians from a portuguese hospital. All the participants were specialized in areas other than psychiatry. The participants were presented with the following self-administered questionnaires: socio-demographic questionnaire, to access the demographic and professional features of the participants; Mental Health Literacy Questionnaire, to evaluate the mental health literacy of the clinicians; Opinions on Mental Illness Questionnaire, to access the beliefs and opinions about the mental illness of the participants; MICA-4 questionnaire, to access the attitudes of the clinicians towards people with mental illness. These questionnaires were previously validated to the portuguese population. RESULTS: A total of 100 questionnaires were handed over, with a response rate of 50%. From a total of 50 participants, 54% were male. The age ranged from 25 to 63 years (36,0 u00b111,565). More than a half had a masteru2019s degree (52%), and 50% were specialists, in a total of 18 different specialties. Only one participant referred not having had formation in mental health, and 4 participants have worked previously in psychiatry. Just 1 participant said to have mental illness, but most of the participants (68%, n=34) said to have a relative or a friend suffering from a mental illness. The mean score in the MICA-4 scale was 34,14u00b18,32. Within the OMI scale, the domains with higher values were the Interpersonal Aetiology (m=4,70u00b10,75) and the Authoritarianism (m=4,28u00b10,62), followed by the Social Restrictiveness (m=4,14u00b10,49), Benevolence (3,74u00b10,34), and Mental Hygiene Ideology (2,97u00b10,45). Regarding the MHLS, the mean score was 127,90u00b110,41. There was a negative correlation between the age of the participants and the OMI mean score. There was a positive relation between male gender and MICA-4 score (p=0,026) and contrarily to the expected, with the u201cbenevolenceu201d domain of the OMI scale (p=0,009). On the other hand, female gender was positively associated with higher scores in MHLS (p=0,044). Within the OMI scale, the u201cinterpersonal etiologyu201d domain stands out as the one with more negative opinions. The participants with personal contact with mental illness had higher scores in the following domains: Authoritarianism (p=0,002) and Interpersonal Aetiology (p=0,033). There was a negative relation between the years of professional experience and the Mental Health Ideology (p=0,014) and Interpersonal Aetiology (p=0,04) domains of OMI scale. CONCLUSION: This is, for the authorsu2019 knowledge, the first study evaluating the literacy in mental health among Portuguese doctors. The results obtained in the MHLQ were lower than we expected. Nevertheless, the scores obtained in the OMI scale, although not ideal, were similar to other studies developed in other countries. Stigma against the mental health consumers among health professionals remains an important problem. Consistent with the literature, knowledge about the disorder and contact with mental illness do not seem to be enough to dilute the problem of stigmatization. In the future, we aim not only to increase our sample, but also to include psychiatrists in it to compare the results, and to extend this study to all health professional categories. We also intend to implement standard educational interventions and to re-evaluate these parameters and perceive if any change in the knowledge, attitudes and stigma is observed. 1.tJorm, A.
