These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children. Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.

La loi HPST et le second Plan Cancer place le médecin généraliste au centre de la prise en charge du cancer. L'objectif de ce travail était de mieux connaître les conditions d'exercice des médecins généralistes (MG) du secteur de Roubaix et de Tourcoing, leur rôle auprès des patients atteints de cancer et d'appréhender leurs difficultés face au corps médical et aux institutions. Méthode : Nous avons recensé l'ensemble des médecins traitants figurant sur les fiches RCP des patients de l'année 2011. Une zone d 'étude a ensuite été définie pour ne concerner que les médecins exerçant au versant Nord-Est de la métropole lilloise. Les critères d'exclusions étaient l'exercice d'une autre spécialité que la médecine générale et l'arrêt d'exercice. Une enquête quantitative descriptive a donc été menée auprès de 389 MG. Résultats : Le taux de réponse a été de 46,53% soit 181 réponses. Les participants à l'étude étaient majoritairement des hommes âgés de 51 à 60 ans exerçant en cabinet de groupe. 84% d'entre-eux découvrent moins de 10 nouveaux cas de cancer par an. 46% suivent entre 10 et 30 cas. Les médecins disent être impliqués à tous les stades de la maladie notamment le dépistage et la prévention. Mais, seulement 4% s'intéressent aux thérapeutiques. Concernant les institutions, 36% connaissent le RRC contre 7% pour le 3C. Les médecins attendent surtout d'un RRC qu'il permette une meilleure coordination des soins. Sur le plan de la formation, 76% se disent compétents et 75% souhaiteraient continuer à se former de manière classique type DPC. 57% sont satisfaits de la circulation de l'information entre la ville et l'hôpital. Ils sont même 74% à considérer leur relation comme bonne ou excellente avec les oncologues ou les hématologues. Mais le problème récurrent relaté par les médecins reste le délai des courriers. Les MG ne se sentent pas toujours à leur place au sein des RCP et ils ne sont que 33% à souhaiter y participer. Mais, 98% souhaitent en recevoir le compte-rendu. Le DCC inclus dans le DMP permettra dans les prochaines années d'intégrer au mieux le MG dans la prise en charge des patients atteints de cancer.
