De mieux en mieux connus, les facteurs épigénétiques viennent s'ajouter aux déterminants génétiques et environnementaux du surpoids. Ainsi, il semblerait que seuls certains enfants soient prédisposés à devenir obèses. Un dépistage précoce du surpoids est alors indispensable pour repérer ces enfants et optimiser leur prise en charge. Nous avons conduit une étude qualitative, prospective et monocentrique parmi les enfants de plus de deux ans consultant au Service d'Urgence Pédiatrique (SUP) de Nancy. L'objectif principal était l'étude de la faisabilité d'un dépistage du surpoids et de la mise en place de mesures de prévention dans une structure d'urgence pédiatrique. Des conseils ont été délivrés aux parents des enfants dépistés. L'évaluation de l'efficacité de ces conseils a été réalisée par un entretien téléphonique ultérieur. 86 familles sur 123 ont accepté de participer à l'étude. 25% des enfants qui présentaient un surpoids ont été dépistés grâce à ce repérage et la mise en place de cet outil de prévention aux urgences est apparue réalisable et acceptable. Les entretiens ultérieurs par téléphone ont permis de constater que la fiche de conseils a permis d'améliorer les connaissances des parents, comme par exemple, le nombre de repas recommandé (p=0,036). Elle a donné lieu à l'initiation d'un suivi médical dans 12,5% des cas. Cette étude a montré que la sensibilisation au surpoids a bien une place au sein du Service d'Urgence Pédiatrique. Ce travail se poursuit actuellement par l'étude des déterminants périnataux de l'obésité infantile au sein du SUP qui est une structure propice à de telles études.
