Book excerpt: Staphylococcus aureus résistant à la méticilline (SARM) présente une incidence élevée en France et est responsable d'infections nosocomiales. Notre objectif a été d'évaluer le dépistage et la prise en charge du SARM dans une unité de moyen séjour gériatrique de Purpan. Cette étude rétrospective a révélé une incidence élevée du nombre de SARM importés, soulignant l'intérêt d'un dépistage à l'entrée, en particulier chez les patients provenant de secteur hospitaliers comme les réanimations, ou encore ayant eu des séjours hospitaliers récents et prolongés, des facteurs de risque généraux (diabète) et locaux (plaies cutanées). Cependant, les mesures de prévention de la transmission croisée dans les secteurs de gérontologie se heurtent à certaines difficultés spécifiques de ces secteurs, nécessitent des adaptations et deviennent très délicates à observer dès lors qu'une proportion importante de patients est porteur, l'accent sera alors mis sur les patients porteurs d'une infection symptomatique et l'hygiène des mains sera l'élément essentiel de la prévention. Les programmes de prévention de la diffusion du SARM doivent en permanence être adaptés aux réalités du terrain et faire l'objet de rappels constants par les membres Comité de Lutte contre les Infections Nosocomiales (CLIN) et les services d'hygiène des établissements de soins.

Book excerpt: Screening methicillin-resistant Staphylococcus aureus (MRSA) carriers at the time of hospital admission is justified by the necessity of an early identification of asymptomatic carriers in order to implement control measures as soon as possible. Nevertheless, screening programmes induce extra-costs and screening strategies are controversial, particularly in non-intensive care units. In the present work, we studied different aspects of screening strategies. In addition to conventional risk factors, we identified home care as an independent risk factor of MRSA carriage on admission, and the possibility of hospital-acquired MRSA transmission from carriers to households. Therefore, it seems that the definition of patients with risk of carriage should be revised. We demonstrated that screening on admission to two non-intensive care wards (internal medicine and emergency room) can be interesting for control. We also defined a strategy concerning the choice of anatomical sites for screening. The impact of the screening strategy on the values of certain surveillance indicators was also evaluated. Finally, we presented the evolution of MRSA indicators in two hospitals with contrasted screening strategies. Screening seems to be essential in MRSA control programmes. However, screening would not be effective without the implementation of efficient barrier measures, whose certain are still controversial or not sufficiently validated.

L'analyse des résultats confirme l'intérêt de cette stratégie, avec notamment une bonne efficacité de la chimiodécontamination nasale et cutanée sur peau saine, permettant une levée plus rapide de l'isolement. Les conséquences, en terme de morbidité, apparaissent limitées, avec cependant un taux de colonisation et une fréquence de la résistance à la méticilline dans l'espèce Staphylococcus aureus élevés. Le travail a été complété par un rappel sur l'épidémiologie de SARM.

Book excerpt: Objectifs : Les objectifs de notre travail sont de connaître la prévalence des infections à Staphylococcus aureus résistant à la méticilline (SARM) chez les patients admis aux urgences et de définir les caractéristiques des patients porteurs de ce germe. Patients et méthodes : Entre Janvier 2003 et Décembre 2004, tous les patients admis aux urgences, pour lesquels une infection à staphylocoques a été documentée, ont été inclus dans cette étude. L'analyse des dossiers a permis de recueillir les facteurs de risque associés au portage du SARM, le diagnostic établit aux urgences ainsi que le traitement effectué. Les comptes rendus du laboratoire de bactériologie ont permis de décrire les sites d'isolement et le profil de résistance des staphylocoques aux antibiotiques. Deux groupes de patients ont été comparés selon la présence d'une infection à SARM ou à SASM. Résultats : 238 patients ont été inclus avec pour 93 d'entre eux une infection à SARM et 145 une infection à Staphylococcus aureus sensible à la méticilline (SASM). Les patients avec SARM ont un âge médian plus élevé que ceux avec SASM (74 vs 61 ans, p=0.0001), ont une perte d'autonomie plus importante (selon l'indice de Knauss) et présentent plus de facteurs de co-morbidité. Neuf patients présentant une infection à SARM, avaient moins de 40 ans, aucun facteur de co-morbidité et facteur associé au portage du SARM. Pour les infections à SARM et SASM, le site d'isolement le plus fréquent est d'origine cutanée dans 40% et 65% des cas, respectivement. Quel que soit le groupe de patients, 28% des patients présentent une bactériémie. Le profil de résistance de ces SARM aux antibiotiques met surtout en évidence une résistance à la kanamycine et tobramycine et à un moindre degré aux macrolides. L'antibiothérapie de première intention n'est que rarement adaptée en cas d'infection à SARM. Conclusion : Il existe une forte prévalence des infections à SARM parmi les infections staphylococciques diagnostiquées aux urgences nécessitant une attitude rationnelle de la prescription des antibiotiques en cas d'infection de ce type.
