Book excerpt: Background Emergency Department (ED) crowding has led to the practice of caring for patients in the ED who are admitted to the hospital, but are waiting for inpatient beds; a practice known as boarding. Boarding in the ED has been shown to negatively affect patient outcomes. ED nurses need preparation to care for the critically ill, boarded patients in the ED. There is little evidence that describes how to best educate ED nurses to care for this patient population. Objectives The purpose of this pilot study was to describe the nursing care and interventions performed by ED nurses, caring for critical care, boarded patients in an academic medical center. This information could then be used to develop critical care education targeted to ED nurses. Methods This study used a non-experimental, descriptive design, and retrospective chart review of electronic health records to collect data on critical care patients boarded in the ED for [greater than/equal to] 3 hours (n=35). Results The majority of patients were male with a median age of 60 years. The primary diagnosis was sepsis, with many of the nursing interventions focused on guideline-recommended therapy. Patients [greater than/equal to] 55 years had higher acuities, required more critical care interventions, and had more in-hospital mortality. Future studies with a larger, more inclusive sample, and more variables of interest would be beneficial to best inform a critical care education program for ED nurses.

Compounding this issue is often the lack of inpatient beds for critical patients. While the ED staff focuses on caring for the influx of patients seeking treatment, as well as stabilizing patients in the ED, critical intensive care boarding patients are left with even fewerresources. Both ED nurses and intensive care unit (ICU) nurses are highly skilled in their specialties, their approaches to care being different. ICU nurses focus on improving the condition of the ICU patient, while ED nurses focus on stabilization and transferring the patient to the appropriate level of care. Objectives: The purpose of this pilot study was to determine if the early implementation of ICUspecialized nursing care reduced ED, ICU, hospital length of stay (LOS) and ED boarding time. Methods: This was a retrospective study that compared patients admitted directly from the ED tothe ICU during similar time periods before and after implementation of the SICU/ ED intervention program in February 2017 at a Level 1 Trauma Center. Data were abstracted from an electronic health record and analyzed using SPSS v 26. Results: The findings showed that the ED LOS in the pre-implementation group was longer whencompared with the post-implementation group of patients receiving care from ICU nurses (U=911.000; p=0.01). ED boarding times in the pre-implementation group were longer when compared with the post-implementation group (U=837.500; p=0.002). ICU and hospital LOS and total hospital time did not show a statistical significance in change in the pre- and post-intervention groups in this pilot study.
