This qualitative study examines key factors that influence the quality of the diet consumed by men who are experiencing homelessness. The first major research question explores factors that potentially affect the quality of the diet served by shelters. The second investigates individual characteristics which potentially affect the selections that are made by the homeless. One-on-one interviews were conducted in three mission settings with 24 subjects, men aged 20-65 years. Subjects were comprised of two groups, persons who were homeless and the mission staff. The research sites were in three different locations, two inner-city and one rural. Purposive sampling methods were used. Data analysis was completed through the development of themes from the coding of data. The quality of the diet for the homeless is often inadequate due to limited food resources, limited staff knowledge regarding nutrition, as well as to particular foods offered and to the final choices made. Missions depend on donors for 80-90% of their food supply. Individuals who are homeless receive approximately 80% of their diet from charitable organizations. For these reasons, both the missions and donors have a tremendous opportunity to positively impact the nutritional status of their clientele. The key factors that were discovered include: the priority of nutrition to the kitchen manager, as well as his/her knowledge of nutrition and ability to apply this; various means to increase the variety of food provided; and limiting the sweets/sugar offered. Two other important nutrition-related factors examined include the nutritional knowledge base of the staff and the homeless, and their attitude towards healthy eating. Weight gain is a factor that is explored as well due to the subjects emphasis on this topic. Foods found in short supply were milk, fresh fruits and fresh vegetables, cheese, eggs and fish. Whole grains were available but often weren't consumed. Meals could be improved by increasing the food temperature, decreasing the sweets made available and by increasing the variety. Nutrition education is one means suggested by this research with the potential to greatly influence both what is served, selected and finally consumed.

Book excerpt: For many Americans who live at or below the poverty threshold, access to healthy foods at a reasonable price is a challenge that often places a strain on already limited resources and may compel them to make food choices that are contrary to current nutritional guidance. To help alleviate this problem, the U.S. Department of Agriculture (USDA) administers a number of nutrition assistance programs designed to improve access to healthy foods for low-income individuals and households. The largest of these programs is the Supplemental Nutrition Assistance Program (SNAP), formerly called the Food Stamp Program, which today serves more than 46 million Americans with a program cost in excess of $75 billion annually. The goals of SNAP include raising the level of nutrition among low-income households and maintaining adequate levels of nutrition by increasing the food purchasing power of low-income families. In response to questions about whether there are different ways to define the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, USDA's Food and Nutrition Service (FNS) asked the Institute of Medicine (IOM) to conduct a study to examine the feasibility of defining the adequacy of SNAP allotments, specifically: the feasibility of establishing an objective, evidence-based, science-driven definition of the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, as well as other relevant dimensions of adequacy; and data and analyses needed to support an evidence-based assessment of the adequacy of SNAP allotments. Supplemental Nutrition Assistance Program: Examining the Evidence to Define Benefit Adequacy reviews the current evidence, including the peer-reviewed published literature and peer-reviewed government reports. Although not given equal weight with peer-reviewed publications, some non-peer-reviewed publications from nongovernmental organizations and stakeholder groups also were considered because they provided additional insight into the behavioral aspects of participation in nutrition assistance programs. In addition to its evidence review, the committee held a data gathering workshop that tapped a range of expertise relevant to its task.

This report fulfills the final reporting requirement of the grant, and presents a summary of activities for the grant period from December 1, 1994 through May 31, 1999. During this grant period, the CMNR has met from three to six times each year in response to issues that are brought to the committee through the Military Nutrition and Biochemistry Division of the U.S. Army Research Institute of Environmental Medicine at Natick, Massachusetts, and the Military Operational Medicine Program of USAMRMC at Fort Detrick, Maryland. The CMNR has submitted five workshop reports (plus two preliminary reports), including one that is a joint project with the Subcommittee on Body Composition, Nutrition, and Health of Military Women; three letter reports, and one brief report, all with recommendations, to the Commander, U.S. Army Medical Research and Materiel Command, since September 1995 and has a brief report currently in preparation. These reports are summarized in the following activity report with synopses of additional topics for which reports were deferred pending completion of military research in progress. This activity report includes as appendixes the conclusions and recommendations from the nine reports and has been prepared in a fashion to allow rapid access to committee recommendations on the topics covered over the time period.
