Book excerpt: Previous research has shown that muscle activation patterns of the anterior thigh influence knee joint mechanics and in particular, patellar tracking. Vastus lateralis and vastus medialis muscles are located on the lateral and medial sides of the quadriceps group respectively. They are the primary muscles responsible for maintaining patellar tracking through the patellofemoral groove in order to prevent knee conditions such as patellofemoral pain syndrome. If either the vastus lateralis or vastus medialis muscle has a predominant level of activation, then the patellar will tend to be drawn to one side of the patellofemoral groove. Rehabilitation protocols for treating patellar maltracking and patellofemoral pain often focus on improving muscle activation of vastus medialis. Vastus medialis is the smallest anterior thigh muscle responsible for leg extension, with lower activation levels often resulting in the patellar being drawn laterally. Although rehabilitation protocols may focus on medial muscle activation, both muscles are innervated by the same nerve, suggesting that improved vastus medialis muscle activation will also result in improved vastus lateralis muscle activation. To combat this problem it has been suggested that turning the foot out to 45 degrees may improve vastus medialis activation due to the interaction of the adductor magnus muscle. Another theory suggests that vastus medialis will be in a relatively higher position than vastus lateralis when seated, and therefore, the muscle needs to work harder to oppose gravity. Three exercises (leg press, squat and terminal leg extension) were assessed in order to determine if commonly employed rehabilitation protocols can improve vastus medialis activation. The exercises were performed with two foot positions (parallel/vertical and turned out to 45 degrees) at 5 0 % of 3 R M and the resultant E M G activity was normalised to a series of isometric M V C trials. Single differential electrodes were placed on the muscle bellies of vastus medialis and vastus lateralis, running parallel to the muscle fibres, with an earth electrode over the patella. Synchronised E M G and video data were collected to allow for the assessment of muscle activation during seated and standing posture exercises, open and closed kinetic chain exercises and for both foot positions to determine if any of these exercise conditions would result in improved vastus medialis muscle activation. The results showed that any improved vastus medialis activation also coincided with improved vastus lateralis activation, with both muscles contracting to perform leg extension motion. Higher levels of muscle activation generally occurred when the foot was in a parallel/vertical foot position, and the squat exercise resulted in higher levels of muscle activation than both of the seated exercises. The two closed kinetic chain exercises also demonstrated higher levels of muscle activation when the leg was in a flexed position, while the open kinetic chain exercise produced higher levels of muscle activation as the leg moved toward an extended position.

Specifically, this study aimed to preferentially recruit VM by introducing a gentle stretch over the muscle, through the combination of thigh adduction and tibial external rotation (with the foot turned out to 45 degrees). Healthy and asymptomatic volunteers (N=29) performed two leg extension exercises, leg press and terminal leg extension, in two different foot positions (feet parallel/vertical, or foot turned out to 45 degrees) and with or without a component of thigh adduction achieved by placing a firm ball between the knees of the subject. Subjects included were participants of team sports. Outcome measures included: Peak EMG activation for VM and VL during the leg extension phase, normalised to MVC; Average EMG activation for VM and VL during the leg extension phase, normalised to MVC; and Knee flexion angle at the time of peak VM and VL activation. Data for each condition were compared using a three factor repeated measures ANOVA. Trends in the data revealed a tendency toward higher muscle activation during the closed kinetic chain exercise (leg press) compared to an open kinetic chain exercise (terminal leg extension). These peak activation values also occurred with the leg in a more flexed position during the leg press compared to the terminal leg extension exercise. However, it was also observed that turning the foot out to 45 degrees does not greatly influence muscle activation in a preferred direction. Furthermore, introducing a gentle stretch over the muscle by adding components of thigh adduction and leg external rotation (with the ball in place) was found to be ineffective in selectively activating VM under these experimental conditions. On the basis of these findings and within the limitations of this research, it was concluded that whilst dynamic quadriceps contractions are undertaken, altered limb or foot position towards tibial external rotation and hip adduction has no influence on preferential activation of VM in asymptomatic individuals.

This study measured peak pressures on the plantar foot and the influences of fatigue on four lower extremity muscles during a lateral hop in those with and without CAI. The results revealed no significant differences in peak pressure between groups for the plantar pressure areas. The muscle measurements demonstrated higher peroneus longus and gluteus maximus activity just prior to landing the lateral hop. There were no statistically significant outcomes for fatigue. Both the muscle activation and the plantar pressure results suggest a pre programmed, feed-forward mechanism adapted by those with CAI. Altered neuromuscular activity in those with CAI has been documented in previous research. These results continue to support this theory, now demonstrated during a functional task which indicates those with CAI are finding methods to land a jump without injury in controlled situations.

Weakness or insufficient coordination of the quadriceps and gluteals can lead to overuse, strain, and compensatory movement patterns such as trunk flexion. The trunk accounts for roughly 50% of a person's mass, so changes in orientation can alter the mechanical demands that are places on the lower extremity. Additional trunk flexion is thought to increase the action of the hip extensors. There is a lack of evidence that isolates quadriceps and gluteal muscle activation during flexion-based running. Objective: To determine the effects of trunk flexion on quadriceps, gluteal, hamstring, and erector spinae (ES) EMG measures in a healthy running population. Design: Descriptive laboratory. Setting: Laboratory. Participants: 18 total participants (10 females, 8 male). Inclusion: recreational runner aged 18-39 who ran an average of 15 miles per week. Exclusion: current lower extremity or low back pathology or one that has caused pain within 6 months of the study. Intervention: The independent variable was the position of the trunk (preferred, flexed, and extended). All participants completed five successful running trials for each trunk position. Main Outcome Measures: EMG measures and 3D trunk and lower extremity kinematics and kinetics were analyzed across the stance phase of the running trials. Results: The activation of the quadriceps and hamstrings exhibited no significance across the conditions. The activation of the gluteals exhibited a significant difference between the flexed and extended position (45.06 ± 13.36% vs. 30.72 ± 12.47%). The activation of the ES exhibited a significant difference between the preferred and extended position (20.50 ± 10.66% vs. 34.78 ± 14.86%). There were no differences detected in knee or hip sagittal plane posture. Conclusion: Sagittal plane trunk positioning has an influence on the activation of the gluteals and ES during free running.
