Matériel et méthode : C'est une étude descriptive quantitative transversale réalisée entre le 30 avril et le 31 octobre 2014 à l'aide d'un questionnaire manuscrit envoyé au médecins généralistes inscrits au conseil de l'ordre en Pays de la Loire, débutant une nouvelle activité ambulatoire entre le 1er janvier 2008 et le 2 décembre 2013 en zones déficitaires, zones urbaines sensibles, zones de redynamisation urbaine ou zones de revitalisation rurale de la région ou ayant bénéficié d'une aide de l'agence régionale de santé pour la création d'une maison de santé pluri professionnelle. Book excerpt: Avec une baisse des effectifs des médecins généralistes, l'offre de soins de premier recours en France devient problématique. Les inégalités de répartition des praticiens se creusent. Les jeunes médecins généralistes rejettent le modèle d'exercice de leurs ainés. L'âge de la première installation recule. Face aux déserts médicaux, les gouvernements tentent d'apporter des solutions avec les mesures incitatives à l'installation mais celles-ci sont critiquées. Afin d'identifier les facteurs déterminants le lieu d'installation des médecins généralistes et de recueillir leur opinion sur les mesures incitatives, une étude descriptive, observationnelle et transversale a été réalisée entre le 7 janvier et le 9 mars 2014. Un questionnaire électronique commun à deux autres travaux relatifs aux problèmes de démographie médicale a été diffusé aux médecins généralistes lillois ayant passé l'Examen Classant National entre 2004 et 2008, tous modes d'exercice confondus. 310 réponses ont été collectées. Il apparait que la famille était au coeur de la décision du lieu d'installation (compatibilité avec le travail du conjoint, établissements scolaires, services de proximité). Les facteurs professionnels prépondérants étaient la proximité d'un établissement de soin et une opportunité professionnelle (reprise de cabinet, association, poste dans une maison de santé pluridisciplinaire (MSP)). La connaissance de l'offre de soin du secteur était également importante. La plupart des médecins avait entendu parler des mesures incitatives. Les aides logistiques et fiscales étaient les plus citées comme incitant à l'installation. Les principaux freins à leurs recours étaient la priorité laissée à la vie familiale et le sentiment de perte de liberté. Les MSP pourraient augmenter le nombre d'installations de même qu'une meilleure formation théorique à l'installation et davantage de stages ambulatoires. Un tiers des médecins pensaient que les mesures incitatives n'avaient pas d'influence sur les installations. Ainsi, les facteurs privés, environnementaux et professionnels, semblent plus déterminants pour le choix du lieu l'installation que les mesures incitatives qui restent controversées.

Matériel et méthode : nous avons conduit une étude quantitative au travers d'un questionnaire anonyme et auto-administré, adressé aux médecins installés depuis 2017 dans 3 territoires recouverts de zones sous-denses : l'Ariège, le Gers et le Comminges. Résultat : 38 questionnaires furent analysés, les principaux déterminants retrouvés étaient : la possibilité de s'installer dans un groupe de professionnels, avoir des opportunités professionnelles pour le/la conjoint(e) et maintenir une proximité avec la famille et/ou le territoire d'origine. Le parcours de l'internat et l'origine personnelle influençaient fortement le lieu d'installation. Les aides financières revêtaient une importance secondaire. Discussion : les facteurs assurant un environnement de travail propice au maintien de l'équilibre entre vie familiale et vie professionnelle apparaissent prioritaires par rapport aux critères financiers. Augmenter la part de médecins formés issus de territoires sous-denses et y développer l'accueil des internes sont des leviers d'action efficaces. Ces résultats convergent avec les travaux déjà réalisés sur le sujet. Conclusion : cette étude confirme l'importance d'agir pour l'amélioration des conditions de travail ainsi que dans la formation des médecins généralistes pour favoriser leur installation en zones sous-denses.
