We extend frequentist-based hypothesis tests previously developed for non-informative coarsening and propose a novel Bayesian method based on comparing a posterior parameter transformation with its expected distribution under the null hypothesis of equal cumulative incidence functions. Both methods use estimates derived by extending previously published estimation procedures to accommodate censoring by death. The data structure and analysis goal are exemplified by the AIDS Link to the Intravenous Experience (ALIVE) study, where researchers are interested in comparing incidence of human immunodeficiency virus seroconversion by risk behavior categories. Coarsening in the forms of interval and right censoring and censoring by death in ALIVE is thought to be informative; thus, we perform a sensitivity analysis by incorporating elicited expert information about the relationship between seroconversion and censoring into the model.

The cumulative incidence function (CIF) is a particularly useful parameter as it explicitly quantifies clinical prognosis. Common issues in competing risks data analysis on the CIF include interval censoring, missing event types, and left truncation. Interval censoring occurs when the event time is not observed but is only known to lie between two observation times, such as clinic visits. Left truncation, also known as delayed entry, is the phenomenon where certain participants enter the study after the onset of disease under study. These individuals with an event prior to their potential study entry time are not included in the analysis and this can induce selection bias. In order to address unmet needs in appropriate methods and software for competing risks data analysis, this thesis focuses the following development of application and methods. First, we develop a convenient and exible tool, the R package intccr, that performs semiparametric regression analysis on the CIF for interval-censored competing risks data. Second, we adopt the augmented inverse probability weighting method to deal with both interval censoring and missing event types. We show that the resulting estimates are consistent and double robust. We illustrate this method using data from the East-African International Epidemiology Databases to Evaluate AIDS (IeDEA EA) where a significant portion of the event types is missing. Last, we develop an estimation method for semiparametric analysis on the CIF for competing risks data subject to both interval censoring and left truncation. This method is applied to the Indianapolis-Ibadan Dementia Project to identify prognostic factors of dementia in elder adults. Overall, the methods developed here are incorporated in the R package intccr.

Book excerpt: Abstract: To analyze competing risks survival data, a commonly used approach employs a Cox model to assess the covariate effects on the cause-specific hazards treating competing risks as censoring. Subdistribution models directly assess the covariate effects on the cumulative incidence function (CIF) treating the competing risk as a second event. We study the relationship of regression coefficient estimates from the Cox model and from the subdistribution model through extensive simulations. We observe the coefficient estimates from the two models are very different when the probability of competing events is above 0.1. The magnitude and directions of the differences depend on the coefficients of both events. Generally, the subdistribution model is more appropriate and performs better. However, when the probability of competing events is low, the simpler Cox model can be used. Furthermore, when the probability of competing events is below 0.4 and the censoring rate is above 0.5, the subdistribution model displays some bias in coefficient estimates potentially due to small numbers of competing events. Survival analyses of diseases of the elderly such as dementia often use survival age as the outcome, and account for left truncation. In such scenarios, we may consider death as a competing risk instead of censoring event. An existing SAS macro estimates the CIF of a disease and the remaining lifetime risk (LTR) conditional on survival alive and event-free to a specified age. However, the macro doesn't compare the whole LTR curves for different groups and doesn't measure the effects of risk factors on the LTR. We study a method which uses the product of the inverse probability of censoring weight and the inverse probability of left truncation weight. We further extend it to model the LTR using the survival ages as the outcome and excluding those events that occurred prior to the event-free base age. We present two SAS macros to model the CIF using survival time as the outcome and the LTR using survival age as outcome and accounting for the left truncation and competing risks, respectively. We illustrate the use of these SAS macros through real datasets.
