Financial protection against catastrophic expenditures has improved substantially, and legislation has been enacted to improve the quality of physician training and patient care.Despite the progress, substantial challenges remain and include comparatively low resources for the health sector, limitations in the supply of providers at the primary and hospital levels, inefficient payment systems, shortcomings in the quality of maternal and child and adult care, lack of oversight and effective licensing in an expanding private health sector, and ineffective planning for and recruitment and retention of health workers.Given the slow pace in improving health outcomes and limited evidence linking health performance and the health workforce, the need to make more information available about past experiences to inform future policy changes is pressing. Few studies have been undertaken to measure the actual impact of the reforms and the remaining challenges. 'New Insights into the Provision of Health Services in Indonesia: A Health Workforce Study' begins the process, providing real time evidence-based inputs to facilitate the Government of Indonesia's comprehensive health sector review. The authors' analysis of panel data from households and health providers will assist the government's assessment of the impact of past health work force policies and its consideration of policy changes.

Book excerpt: Jacoby and Wang use a competing risk model to analyze environmental determinants of child mortality using the 1992 China National Health Survey, which collects information on cause of death. Their primary question is whether taking into account of cause of death using a competing risk model, compared with a simple model of all causes of mortality, affects conclusions about the effectiveness of policy interventions. There are two potential analytical advantages in using cause of death information: (1) obtaining more accurate estimates and (2) validating causal relationships. Although the authors do not find significant differences between estimates obtained from the competing risk model and those from simpler hazard models, they do find evidence supporting the causal interpretations of the effect of access to safe water on child mortality. The authors' analysis also suggest that a respondent-based health survey can be used to collect relatively reliable information on cause of death. Modifying future demographic and health survey instruments to collect cause of death information inexpensively may be worthwhile for enhancing the analytical strength of the surveys. This paper--a joint product of Rural Development, Development Research Group, and the Environment Department--is part of a larger effort in the Bank to improve our understanding of the determinants of child mortality and provide policy advice to improve health outcomes in low-income countries.

Environment and health perspective dates back to Hippocrates treatise written 400 B.C.E. In his book On Airs, Waters and Places, Hippocrates described diseases as associated with environmental conditions, “Whoever wishes to investigate medicine properly, should proceed thus: in the first place to consider the seasons of the year, and what effects each of them produces for they are not at all alike, but differ much from themselves in regard to their changes. Then the winds, the hot and the cold, especially such as are common to all countries, and then such as are peculiar to each locality. We must also consider the qualities of the waters, for as they differ from one another in taste and weight, so also do they differ much in their qualities. In the same manner, when one comes into a city to which he is a stranger, he ought to consider its situation, how it lies as to the winds and the rising of the sun; for its influence is not the same whether it lies to the north or the south, to the rising or to the setting sun”. There has been a greater emphasis in the last four decades on understanding environmental factors which affect human health, after United Nations established Intergovernmental Panel on Climate Change (IPCC) in 1988 aimed at to evaluate research on changing environmental condition, particularly climate change and its impacts on human wellbeing, including human health, as consequences of extreme heat waves conditions, sea level rise, forced migration, air pollution, droughts, and wildfires. From these studies, risk levels of vulnerable populations and regions can be assessed and level of resilience of healthcare infrastructure that may be used in environmental health policy and equity of these countries.
