Book excerpt: "Achieving equity in health requires a commitment to monitoring health inequalities which, in turn, necessitates strong, equity-oriented health information systems. High-quality data and robust monitoring systems ensure that efforts can be targeted appropriately and that progress can be tracked. Countries must strengthen health information systems to generate better data and evidence to measure progress. Integral to the health inequality monitoring process is the task of reporting data in a meaningful way. This State of inequality report exemplifies effective reporting practices, featuring the topic of reproductive, maternal, newborn and child health. The report addresses the challenge of how to best communicate a large and complex body of data in a manner that is comprehensible, flexible and appealing to a wide readership. Feature stories provide an in-depth look at the state of inequality for selected indicators and highlight key observations in reproductive health interventions, maternal health interventions, care-seeking for sick children, childhood immunization, child malnutrition and child mortality. Perhaps most notable, however, is the innovative use of electronic visualization technology. Story-point dashboards, for instance, guide the reader through a succession of visuals where readers can use interactive tools to further explore, sort and filter the data. Similarly, interactive maps and tables engage readers in customizing how data are viewed. The report reveals that significant inequalities exist in low- and middle-income countries in the area of reproductive, maternal, newborn and child health. The good health of women, infants and children is essential for sustainable development, and there is still much work to be done. Discussions will increasingly call into question how efforts to improve reproductive, maternal, newborn and child health can achieve early and accelerated progress among those who are falling behind"--Pages viii-ix.

Book excerpt: The problems related to the process of industrialisation such as biodiversity depletion, climate change and a worsening of health and living conditions, especially but not only in developing countries, intensify. Therefore, there is an increasing need to search for integrated solutions to make development more sustainable. The United Nations has acknowledged the problem and approved the “2030 Agenda for Sustainable Development”. On 1st January 2016, the 17 Sustainable Development Goals (SDGs) of the Agenda officially came into force. These goals cover the three dimensions of sustainable development: economic growth, social inclusion and environmental protection. The Encyclopedia of the UN Sustainable Development Goals comprehensively addresses the SDGs in an integrated way. The Encyclopedia encompasses 17 volumes, each one devoted to one of the 17 SDGs. This volume addresses SDG 3, namely "Ensure healthy lives and promote well-being for all at all ages" and contains the description of a range of terms, to grow a better understanding and foster knowledge. Ensuring healthy lives and promoting the well-being for all at all ages is essential to sustainable development. Significant strides have been made in increasing life expectancy and reducing some of the common killers associated with child and maternal mortality. Major progress has been made on increasing access to clean water and sanitation, reducing malaria, tuberculosis, polio and the spread of HIV/AIDS. However, many more efforts are needed to fully eradicate a wide range of diseases and address many different persistent and emerging health issues. Concretely, the defined targets are: Reduce the global maternal mortality ratio to less than 70 per 100,000 live births End preventable deaths of newborns and children under 5 years of age, with all countries aiming to reduce neonatal mortality to at least as low as 12 per 1,000 live births and under-5 mortality to at least as low as 25 per 1,000 live births End the epidemics of AIDS, tuberculosis, malaria and neglected tropical diseases and combat hepatitis, water-borne diseases and other communicable diseases Reduce by one third premature mortality from non-communicable diseases through prevention and treatment and promote mental health and wellbeing Strengthen the prevention and treatment of substance abuse, including narcotic drug abuse and harmful use of alcohol Halve the number of global deaths and injuries from road traffic accidents Ensure universal access to sexual and reproductive health-care services, including for family planning, information and education, and the integration of reproductive health into national strategies and programmes Achieve universal health coverage, including financial risk protection, access to quality essential health-care services and access to safe, effective, quality and affordable essential medicines and vaccines for all Substantially reduce the number of deaths and illnesses from hazardous chemicals and air, water and soil pollution and contamination Strengthen the implementation of the World Health Organization Framework Convention on Tobacco Control in all countries, as appropriate Support the research and development of vaccines and medicines for the communicable and non-communicable diseases that primarily affect developing countries, provide access to affordable essential medicines and vaccines, in accordance with the Doha Declaration on the TRIPS Agreement and Public Health, which affirms the right of developing countries to use to the full the provisions in the Agreement on Trade-Related Aspects of Intellectual Property Rights regarding flexibilities to protect public health, and, in particular, provide access to medicines for all Substantially increase health financing and the recruitment, development, training and retention of the health workforce in developing countries, especially in least developed countries and small island developing states Strengthen the capacity of all countries, in particular developing countries, for early warning, risk reduction and management of national and global health risks Editorial Board Mohamed Walid AbdullahMeherun AhmedMonica de AndradeMasoud MozafariGiorgi PkhakadzeTony WallCatherine Zeman/div/div

Book excerpt: According to the most recent data available , 4.5 billion people were not fully covered by essential health services in their countries in 2021. Likewise, in 2019, the total population experiencing financial hardship was estimated to be 2 billion people. To scale up action to leave no one behind, WHO’s Fourteenth General Programme of Work, 2025-2028 (GPW 14) has an explicit and strong commitment to equity. In support of this, WHO has released a handbook with methods for the identification of barriers to effective coverage with health services. The 8-module handbook applies mixed method research approaches -- grounded in the Tanahashi framework for effective coverage -- to focus on barriers experienced by potential users and non-users of health services. The handbook facilitates the capturing of evidence on the interface between the population and the services. The handbook has four objectives. 1.To orient national authorities and partners on key concepts, definitions, frameworks and principles relevant to barriers assessments. 2.To build the capacity of national authorities and partners to design a research plan and apply diverse methods (e.g. informant interviews, literature reviews, quantitative analysis and focus groups) in barriers assessments. 3.To provide guidance for reporting on barrier assessment findings in a clear and actionable manner and convening key stakeholders to deliberate next steps. 4.To adapt the methods for humanitarian contexts, accounting for the need for differentiated approaches.

Book excerpt: Within the continuum of reproductive health care, antenatal care provides a platform for important health-care functions, including health promotion, screening and diagnosis, and disease prevention. It has been established that, by implementing timely and appropriate evidence-based practices, antenatal care can save lives. Endorsed by the United Nations Secretary-General, this is a comprehensive WHO guideline on routine antenatal care for pregnant women and adolescent girls. It aims to complement existing WHO guidelines on the management of specific pregnancy-related complications. The guidance captures the complex nature of the antenatal care issues surrounding healthcare practices and delivery, and prioritizes person-centered health and well-being --- not only the prevention of death and morbidity --- in accordance with a human rights-based approach.

Book excerpt: Having a child remains one of the biggest health risks for women worldwide. Fifteen hundred women die every day while giving birth. That's a half a million mothers every year. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work. The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.

Book excerpt: Social determinants of health – broadly defined as the conditions in which people are born, grow, live, work and age, and people’s access to power, money and resources – have a powerful influence on health inequities. In 2021, the Seventy-fourth World Health Assembly adopted resolution WHA74.16 on addressing social determinants of health, which encouraged Member States to integrate social determinants of health into public policies and programmes and adopt multisectoral approaches. The resolution requested the Director-General to develop an “Operational framework ... for the measurement, assessment and addressing, from a cross-sectorial perspective, of the social determinants of health and health inequities”. This Operational framework for monitoring social determinants of health equity provides countries with critical guidance on monitoring the social determinants of health and actions addressing them, and using data for policy action across sectors to improve health equity. The publication is meant as a resource for national governments and their partners. It was drafted under the guidance of an expert group and with internal contributions across the three levels of WHO, as well as through formal consultation with Member States, and was endorsed at the Seventy-sixth World Health Assembly.

Book excerpt: The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: Optimizing outcomes for women in labor at the global level requires evidence-based guidance of health workers to improve care through appropriate patient selection and use of effective interventions. In this regard, the World Health Organization (WHO) published recommendations for induction of labor in 2011. The goal of the present guideline is to consolidate the guidance for effective interventions that are needed to reduce the global burden of prolonged labor and its consequences. The primary target audience includes health professionals responsible for developing national and local health protocols and policies, as well as obstetricians, midwives, nurses, general medical practitioners, managers of maternal and child health programs, and public health policy-makers in all settings.
