Book excerpt: This document consists of candu zr-2.5nb pressure tube inspection and operating data primarily for ontario hydro reactors. the data presented here represent the current contents of the in-reactor deformation database developed and maintained at ohrd/1/. it is intended for use in the understanding of the deformation behaviour of fuel channels in power reactors and in the derivation and fine-tuning of material creep and growth constants for the new deformation equations. in compiling and presenting the deformation and operating data of fuel channels the databse provides a statistical sample of the actual operating behaviour of components in candu power reactors. this sample may be used to detect any abnormal or unexpected effects and to provide a means of comparison between different power reactos. with the future addition of 600 mw and experimental reactor data, this information will play a key role in the development of a broader understanding of fuel channel behaviour in the ongoing process of extending the operating lives of candu reactors. this document represents the first issue of the information in the database in a 3-ring binder format. it is felt that this format will avoid the need to re-issue the entire document when new information becomes available. it will be the responsibility of ohrd to provide updates on a regular basis (most likely annually). the information is divided into three sections: the first section contains diametral expansion inspection data for individual fuel channels, the second contains the sag inspection data, and the third contains the reactor operating data. within each section are three subsections: a legend describing the tables, information on the source and processing of the data in the tables, and the tables of the data.

Book excerpt: The life cycle of a power plant involves many complex phases, each requiring attention to the specific human factors (hf) issues relevant to various project personnel (e.g. operators, maintainers, construction, commissioning + decommissioning personnel). it also includes an ongoing process of change as technology evolves, as new requirements emerge, and as experience reveals new challenges and opportunities to maintain or improve performance. in order to meet these wide ranging and ongoing requirments, a comprehensive, but flexible program is needed that is established and committed to early in the course of any project or any change during the life of the plant. such an approach has been developed in the form of this human factors engineering program plan (hfepp) development guide. the hfepp concept was developed for military applications and is a well established approach in that domain. there is growing awareness now in the power industry, both within canada and abroad, of the potential for human performance improvements through an hfepp. for ontario hydro, there is ample evidence of the scope for improvements from several sources: the significant event reports (sers) from nuclear stations, the recent decline in worker safety record, the recurring recommendations from external reviews such as the recent hare commission and osart (and others before them) and increasing emphasis on human performance by the atomic energy control board (aecb). the regulatory context for work relating to nuclear stations provides a special incentive. it is clear that the aecb will soon require more thoroughly documented and auditable efforts in the area of hf than in the past. a regulatory policy statement requiring much broader and better documented hf than ever before is expected soon. in addition the aecb has let several contracts to external hf consultants in recent years, and is expanding it's hf staff.

Book excerpt: Simulations of ultrasonic inspection of engineered components have been performed at chalk river laboratories of aecl for over 10 years. the computer model, called ewe for elastic wave equations, solves those equations using a novel finite difference scheme. it simulates the propagation of an ultrasonic wave from the transducer to a flaw, the scatter of waves from the flaw, and measurement of signals at a receive transducer. regions of different materials, water and steel for example, can be simulated. in addition, regions with slightly different material properties from the parent material can be investigated. the two major types of output are displays of the ultrasonic waves inside the component and the corresponding amplitude or a-scans. the ewe model has been applied to the inspection of large pipes in a nuclear plant, gas pipeline welds and steam generator tubes. most recent work has dealt with the ultrasonic inspection of pressure tubes in candu reactors. pressure tube inspections can reliably detect and size defects; however, there are improvements that can be made. for example, knowing the sharpness of a flaw-tip is crucial for effective fitness-for-service assessments. computer modelling of the ultrasonic inspection of flaws with different root radii has suggested inspection techniques that provide flaw tip radius information. a preliminary investigation of these methods has been made in the laboratory. the basis for the model will be reviewed at the presentation. then the results of computer simulations will be displayed on a pc using an interactive program that analyzes simulated a-scans. this software tool gives operators direct access to the results of computer simulations.

Denting is a circumferential deformation of the tube in the support region as a result of the formation of a magnetite, Fe3O4, corrosion product on the carbon steel tube support plate in the crevice between the support plate and the tube wall. The effects of denting on eddy current inspection are twofold: (1) The detection and measurement of defects in the dented region is hampered by the distortion of the eddy-current response signals caused by the dents and (2) large dents may prevent the passage of high fill factor probes forcing the inspection teams to use a smaller probe in undented regions. Unless appropriate measures are taken, less reliable detection of defects and estimation of their depth can result. The report presents an experimental evaluation of existing eddy-current ISI methods when used to inspect dented regions. Tubes were mechanically dented to simulate various degrees of service induced denting. These tubes were then inserted in carbon steel tube support plates and the crevice region between the support plate and tube was subsequently packed with powdered magnetite. Diametral dent sizes considered during this program varied between 1 and 10 mils. Eddy-current signal patterns and strip chart recordings were obtained using a variety of test parameters, such as frequency, phase rotation and coil configuration, on tube samples containing various degrees of denting and with several types of defects in the dented system.

Book excerpt: Steam Generators (SGs) are a critical component of nuclear reactors, employing thousands of SG tubes to convert heat generated in the reactor core into useable energy. SG tubes are supported at numerous locations by Broach Support Plates (BSPs) that have trefoil shaped holes, which prevent excessive tube vibrations, while allowing water to easily flow through the support structures. A number of degradation modes occur in SGs, such as SG tube fretting, cracking or denting, requiring periodic inspection. Currently, conventional Eddy Current Testing (ECT) is used to non-destructively assess the condition of SG tubes and components. However, as reactors age, new modes of degradation will likely appear that may be difficult to detect and characterize using conventional ECT, such as wall loss in BSPs and build-up of corrosion products, which typically form as a hard sludge called magnetite. Pulsed Eddy Current (PEC) technologies are an emerging technique that is presented in this work as a method to further advance inspection techniques used in CANDU® nuclear reactors. A PEC probe was designed to inspect the unique shape of the trefoil shaped hole to detect and characterize wall loss and the presence of magnetite in A516 carbon steel BSPs with trefoil shaped holes from within 15.9 mm (5/8") Alloy-800 SG tubes. PEC was also used to observe how measurements of wall loss were affected by the presence of magnetite. This work presents Finite Element Method (FEM) simulations and experimental results collected to observe these degradation modes. The probe was demonstrated to be capable of detecting far side wall loss as low as 20%, locating and characterizing the relative permeability of magnetite, and of detecting wall loss when magnetite was present. FEM simulations and experimental results were found to be in good agreement, suggesting that additional investigations of the effects of BSP degradation on PEC signal response may also be performed using FEM models.
