Given the individual and population health burden of lung cancer, especially when it is diagnosed at later stages, there has been a push to develop and implement screening strategies for early detection. However, many factors need to be considered for broad implementation of lung cancer screening in clinical practice. Effective implementation will entail understanding the balance of potential benefits and harms of lung cancer screening, defining and reaching eligible populations, addressing health disparities, and many more considerations. In recognition of the substantial challenges to developing effective lung cancer screening programs in clinical practice, the National Academies of Sciences, Engineering, and Medicine held a workshop in June 2016. At the workshop, experts described the current evidence base for lung cancer screening, the current challenges of implementation, and opportunities to overcome them. Workshop participants also explored capacity and access issues; best practices for screening programs; assessment of patient outcomes, quality, and value in lung cancer screening; and research needs that could improve implementation efforts. This publication summarizes the presentations and discussions from the workshop.

Book excerpt: "BACKGROUND (1) Despite advances in technology and screening, 5 year survival for lung cancer has not seen major improvement. Currently, surgical resection is the primary treatment for early stage lung cancer. Postoperative readmission is known to be a strong predictor of 90-day mortality and directly related to postoperative complications, which are a huge negative predictor of survival. Current models are largely inaccurate in lung cancer specific risk prediction. (2) Smoking cessation intervention with lung cancer screening has been shown to further reduce lung cancer mortality. The efficacy of smoking cessation services such as phone counselling in lung cancer screening populations has yet to be established. OBJECTIVES (1) To predict 90-day postoperative readmission in lung cancer patients treated by lobectomy, compare predictive performance between Random Forests and logistic regression and assess predictor importance using random forests (2) To assess the effectiveness of smoking cessation by phone counselling in the McGill lung cancer screening population. Also, to compare quit rates and smoking behaviours of lung cancer screening referred participants to healthcare worker referred participants. METHODS (1) This is a retrospective cohort study of lung cancer patients resected by lobectomy at the MUHC from 2016-2019. Data was extracted from NSQIP, MUHC Datawarehouse and MUHC Pulmonary Function Testing database. Our primary outcome was 90-day postoperative readmission. We compared accuracy, sensitivity, specificity, false positive rate and AUC in logistic regression and random forests. The logistic regression used Age, Gender, BMI, history of severe COPD and Charlson comorbidity index. The random forest model used all available predictors (2) This is a retrospective cohort study of active smokers referred to Quebec's smoking cessation phoneline in three groups: self-referred participants, healthcare worker referred participants and lung cancer screening referred participants. Variables used were demographics, smoking history, history of mental health disorder and quit intentions. Our primary outcome was self-reported 30-day abstinence rates at 6 months, assessed by logistic regression. RESULTS (1) The logistic regression reported that the most important predictors of our outcome were Gender (OR:2.38, CI:1.20-2.87, p0.05), history of severe COPD (OR:2.1, CI:1.16-3.2, p0.05) and Charlson comorbidity index (OR:1.29, CI:0.9-1.4, p>0.05). The Random Forests ranked predictors by importance (in descending order): Age, WBC count, Hematocrit, Albumin, Sodium, Creatinine, BMI, Gender, Hypertension, Charlson comorbidity index, Active Smoker Within 1 Year of surgery and PFT availability. Dyspnea, history of severe COPD and diabetes all had zero importance and were ranked last. The Random Forests also assessed the relationship between all predictors and our outcome via visualization of its decision trees. The Random Forests had better performance on all assessed metrics, most significantly on AUC (0.72) compared to logistic regression (0.59). (2) Lung cancer screening referred participants and the lowest 6 month quit rates amongst all groups (12%, CI:5-19%). They were less likely to quit compared to self-referred participants (OR:0.37; CI:0.17-0.8), while healthcare worker referred participants were twice as likely to quit (OR:2.16; CI:1.3-3.58), despite adjusting for smoking intensity and quit intentions. CONCLUSION (1) Random forests may be a better model than logistic regression for predicting clinical outcomes like readmission. The random forests predictor importance ranking indicates using preoperative laboratory data for risk assessment may be worth further validation. (2) Phone counselling alone has limited benefit in a lung cancer screening program and lung cancer screening referred participants differ significantly from those otherwise referred by healthcare workers"--
