Book excerpt: All across the United States, individuals, families, communities, and health care systems are struggling to cope with substance use, misuse, and substance use disorders. Substance misuse and substance use disorders have devastating effects, disrupt the future plans of too many young people, and all too often, end lives prematurely and tragically. Substance misuse is a major public health challenge and a priority for our nation to address. The effects of substance use are cumulative and costly for our society, placing burdens on workplaces, the health care system, families, states, and communities. The Report discusses opportunities to bring substance use disorder treatment and mainstream health care systems into alignment so that they can address a person's overall health, rather than a substance misuse or a physical health condition alone or in isolation. It also provides suggestions and recommendations for action that everyone-individuals, families, community leaders, law enforcement, health care professionals, policymakers, and researchers-can take to prevent substance misuse and reduce its consequences.

Available in PDF, EPUB and Kindle. Book excerpt: This collection of studies by experts on drug abuse treatment represents the state of the art in research examining what works best and why in a nationwide effort to improve and expand treatment to counteract the epidemic use of cocaine and the spread of AIDS. Investigators at the National Institute on Drug Abuse describe 15 projects that test intervention strategies to improve client recruitment, retention, performance, and treatment outcomes in a range of community-based programs relating to different populations, treatment settings, and research designs addressing real world issues. The findings, analysis, and recommendations should counteract misunderstanding and misinformation about the nature of drug abuse among 5.5 million patients and the effectiveness of current treatment programs. This report is meant for students, scholars, and professionals dealing with public health, criminal justice, America's social problems and governmental policies. This professional report describes innovative programs for improving the treatment of drug abuse. The first part of the book relates to methadone programs limiting the spread of HIV among high-risk narcotics addicts, improving retention on methadone maintenance, and enhancing treatment through a mobile health service. The second segment of the book deals with residential programs, assessing their impact, evaluating a specific outreach project, providing a longitudinal evaluation of dually diagnosed men in different community programs. Case management programs described in the third section of the study include an assessment of a model for intravenous drug users, an assertive community treatment project for hard-to-reach populations, and problems with and remedies for treatment drop-outs and noncompliers. The fourth part of the report concerns system-evaluation of strategies for dealing more effectively with drug abuse today. It covers multi-modality and other eclectic programs, such as a therapeutic community day treatment model for methadone clients, a behavioral model for treating cocaine addicts, treatments for AIDS risk reduction, and a drug day treatment using the therapeutic community model.

Book excerpt: This collection reports on the progress of the intervention programs first described in Innovative Approaches in the Treatment of Drug Abuse: Program Models and Strategies (Inciardi and Fletcher, 1993). By examining the implementation of treatment initiatives, this study focuses on an area often neglected in the research literature: the context in which research is conducted. Applied researchers, particularly those who study users of illicit drugs, face many obstacles that investigators working in more controlled settings, or with more predictable and compliant subjects, often do not encounter. These accounts demonstrate the challenges in producing rapid improvement in treatment for the vulnerable and underserved population of drug abusers. A close study of these efforts will be useful to other researchers in planning for and solving implementation problems that can be anticipated, and in providing guidelines and strategies to overcome those that cannot.

Book excerpt: Each year, more than 33 million Americans receive health care for mental or substance-use conditions, or both. Together, mental and substance-use illnesses are the leading cause of death and disability for women, the highest for men ages 15-44, and the second highest for all men. Effective treatments exist, but services are frequently fragmented and, as with general health care, there are barriers that prevent many from receiving these treatments as designed or at all. The consequences of this are seriousâ€"for these individuals and their families; their employers and the workforce; for the nation's economy; as well as the education, welfare, and justice systems. Improving the Quality of Health Care for Mental and Substance-Use Conditions examines the distinctive characteristics of health care for mental and substance-use conditions, including payment, benefit coverage, and regulatory issues, as well as health care organization and delivery issues. This new volume in the Quality Chasm series puts forth an agenda for improving the quality of this care based on this analysis. Patients and their families, primary health care providers, specialty mental health and substance-use treatment providers, health care organizations, health plans, purchasers of group health care, and all involved in health care for mental and substanceâ€"use conditions will benefit from this guide to achieving better care.

Book excerpt: This TAP presents seven papers submitted to the 2004 National Rural Alcohol and Drug Abuse Network (NRADAN) Awards for Excellence. Each paper describes effective and innovative models of treatment and prevention services in rural populations. This publication seeks to promote and showcase research addressing the unique and special challenges of providing treatment services to individual in rural areas and their families. The first place paper, "Creating and Sustaining an Adult Drug Court: Avoiding Burial in Grant's Tomb," (Glade F. Roper and Dee S. Owens) describes the effectiveness of a self-funded drug court in Tulare County, California; lessons learned; and disadvantages and advantages of the self-funded approach. The second place paper, "Empower for Recovery: An Innovative Approach to Assist Sustained Recovery in Rural Iowa," (Deborah K. Rohlfs) describes the effectiveness of a strength-and homebased substance abuse treatment and recovery support program in rural Iowa, as well as the challenges, problems, and solutions related to program implementation. The third place paper, "Delivering a Maternal Substance Abuse Intervention Program along the Rural Route," (Trudee Ettlinger) describes the development and effectiveness of an indicated-level substance abuse prevention program for lower income mothers living in rural Vermont, as well as the lessons learned and recommendations for improving implementation. "Socio-Demographic Profiles and Treatment Outcomes of Methamphetamine Abusers in Rural and Urban Areas" (Kazi A. Ahmed, and Careema Yusuf), is the fourth paper, and compares methamphetamine abusers in rural and urban areas, specifically their socio-demographic characteristics, actual use behavior, and drug use and nondrug treatment outcomes. " An Environmental Scan of Faith-Based and Community Reentry Services in Johnson County, Iowa" (Janet C. Hartman, Stephan Arndt, Kristina Barber, and Thomas Wassink) is fifth in the series, and focuses on community corrections, substance abuse treatment providers and faith-based organizations supporting reentry clients. Sixth in this group of papers, " Substance Abuse among Rural and Very Rural Drug Users at Treatment Entry" (Marlies L. Schoeneberger, Carl G. Leukefeld, Matthew L. Hiller, and Michael Townsend) examines the demographic distinction between rural and very rural drug users. Implications are discussed and recommendations are presented for substance abuse treatment providers and policymakers. The seventh and final paper, "Making the Addiction Severity Index User Friendly: An Electronic Display of Client Outcomes Using Shareware" (James E. Sorensen, James Elzey, and Faith Stuart), attacked a root problem in the use of outcome instruments: the inability to conveniently summarize and display client outcomes. Each paper provides references. "Resources on Rural Substance Abuse Issues" provides descriptions of programs, and contact information. (Contains 14 tables and 1 figure.).

Book excerpt: Problems stemming from the misuse and abuse of alcohol and other drugs are by no means a new phenomenon, although the face of the issues has changed in recent years. National trends indicate substantial increases in the abuse of prescription medications. These increases are particularly prominent within the military, a population that also continues to experience long-standing issues with alcohol abuse. The problem of substance abuse within the military has come under new scrutiny in the context of the two concurrent wars in which the United States has been engaged during the past decade-in Afghanistan (Operation Enduring Freedom) and Iraq (Operation Iraqi Freedom and Operation New Dawn). Increasing rates of alcohol and other drug misuse adversely affect military readiness, family readiness, and safety, thereby posing a significant public health problem for the Department of Defense (DoD). To better understand this problem, DoD requested that the Institute of Medicine (IOM) assess the adequacy of current protocols in place across DoD and the different branches of the military pertaining to the prevention, screening, diagnosis, and treatment of substance use disorders (SUDs). Substance Use Disorders in the U.S. Armed Forces reviews the IOM's task of assessing access to SUD care for service members, members of the National Guard and Reserves, and military dependents, as well as the education and credentialing of SUD care providers, and offers specific recommendations to DoD on where and how improvements in these areas could be made.
