Book excerpt: The underlying goal of all five of the chapters in this manuscript is to better understand how language facilitates or impedes care for gender nonconforming (GN) people, individuals who experience and/or express aspects of gender that do not conform to social expectations, through (1), access (2), knowledge, (3), quality of care, and (4) education. GN individuals are more likely to need access to mental health treatment, yet many avoid seeking care out of fear of further mistreatment. This study seeks to understand how language may play a role in the care process and access to care. Chapter 1 is an introduction to the idea of language as a regulatory process. This chapter is an introduction to the population and reviews relevant literature. Chapter 2 explores institutional language as a key element for access to care by examining intake forms collected from mental health clinics throughout the state of Utah. Chapter 3 examines the role of social work education in preparing students for practice with GN individuals. This exploration is conducted by examining descriptions of gender and GN experience from the perspective of social work students. Chapter 4 explores how social work students ascribe meaning to GN experiences may reflect patterns of communication and influence the therapeutic relationship and working alliance. Chapter 5 is a summary of research findings. This chapter addresses the implications of the research findings and provides specific practice, education, and policy recommendations.

Book excerpt: This study examined the presence and impact of subtle expressions of marginalization, termed microaggressions, in psychotherapy, toward clients who are transgender and gender non-conforming. The design used was a concurrent nested mixed method approach, involving an internet-based survey deployed throughout the United States by community and online programs that serve this population, with a final sample size of 255. In addition to the microaggression categories identified by Nadal, Skolnik, and Wong (2012), two additional themes were identified as Expecting Education and Ignoring Gender. A sub-theme, Expecting Binary Transition Norms, was identified under the category previously identified by Nadal et al. (2012) as Endorsement of Gender Normative and Binary Culture or BehaviorsThe most frequent types of microaggressions reported were assumption of universal transgender experience, discomfort/disapproval of transgender experience, endorsement of gendernormative and binary culture or behaviors, and use of transphobic and/or incorrectly gendered terminology. The same microaggression types that did the most damage to the therapeutic alliance also correlated the most with decisions to terminate therapy prematurely. The microaggression types that most related to these variables were expressions of disapproval and discomfort, pathologization of clients' gender identities, and invasions of bodily privacy. Alliance ruptures were repaired most significantly when therapists, rather than clients, addressed microaggressions. The impact of microaggressions, identified through thematic analysis of text responses, fell into three major categories: (a) self-perception, which included two sub-categories, self-doubt and decreased value for oneself; (b) perception of the therapist and therapy, which involved diminished understanding, safety, openness, and value; (c) perception of other relationships involved accepting excessive responsibility for the feelings and actions of others, isolating from and distrusting people, and expecting their negative judgment.

They can make their way into intentionally safe spaces such as therapy rooms, as wellintentioned mental health providers may express them towards clients. In the general context of therapy, microaggressions negatively impact aspects of the therapeutic process (e.g., therapeutic alliance, satisfaction, and psychological wellbeing) and contributed to early termination from therapy. Nevertheless, questions remain about how microaggressions are experienced by transgender and gender nonconforming (TGNC) therapy clients and how microaggressions affect their therapy experiences. Thus, this cross-sectional online study examined the types and impacts of microaggressions committed by mental health providers towards TGNC clients, expanding upon the categories of microaggressions examined in previous studies. Further, this study looked at the mediating effects of therapists’ attempts to address microaggressions (a type of “rupture repair”) on the relationships between microaggressions and therapeutic alliance, satisfaction with therapy, and premature therapy dropout. Participants were 220 adults who identified as gender diverse and had seen a mental health provider. Most participants experienced TGNC microaggressions from their most salient mental health provider (83.6%), which was associated with reduced ratings for therapeutic alliance, satisfaction and premature dropout from therapy. Respondents also considered all categories of microaggressions as harmful. Further, the mediation analysis revealed that when therapists addressed microaggressions, those attempts to repair ruptures to the therapeutic process mediated (suppressed) negative effects microaggressions had on the therapeutic alliance, satisfaction with therapy and the therapist, and reduced premature client dropout. Study results suggest that attempting to address microaggressions in therapy may not only mitigate the negative effects of therapy, but may also be associated with improving the therapeutic process. Future research and clinical implications of these findings are discussed.
