These challenges persist into adulthood, which can lead to poor outcomes in postsecondary education, employment, and independent living. Although social skills can be taught, the majority of evidence-based practices addressing social skills deficits have only been tested with young children with ASD. As such, there is an urgent need to test the use of evidence-based practices to teach social skills with older students with ASD. Pivotal Response Treatment (PRT) is an evidence-based practice that has been used to improve the social and communication skills of children with ASD. However, it has primarily been implemented with young children and in the clinical setting. This concurrent multiple baseline design study across dyads investigated the use of PRT in the secondary school setting with adolescents with ASD. Specifically, it examined the impact of PRT on one aspect of social interaction: question-asking. Education providers (n = 3) were trained to implement PRT with a secondary student with whom they already worked. Results of the study found no functional relation between education provider-implemented PRT and targeted question-asking behavior (id est, questions beginning with who, what, or where) in secondary students with ASD. In part, this finding was attributed to lack of fidelity of implementation: only one of three education providers was able to achieve the criterion level for fidelity. Nonetheless, two of the students exhibited clear effects. Implications for practice and research, limitations, and directions for future research are discussed.

"Pivotal response training" ("PRT") is an intervention designed for children with autism spectrum disorders. This practice focuses on pivotal (core) areas affected by autism, such as communication and responding to environmental stimuli. "PRT" sessions typically begin with a parent or teacher providing clear instructions to a child, having the child help choose a stimulus (such as a toy), and focusing the child's attention. The parent or teacher then encourages the desired behavior (for example, asking for the toy or choosing "toy" from a list of words) by providing rewards if the child implements or attempts to implement the desired behavior. The complexity of the required responses increases as training progresses. Parents, teachers, and peers collaboratively implement the practice at school, at home, and in the community. "PRT" can be used with autistic children aged 2-18. "PRT" is also known as "Pivotal Response Therapy," "Pivotal Response Treatment"ʼ, or "Natural Language Paradigm." "PRT" was found to have no discernible effects on communication/language competencies for children and students with an autism spectrum disorder. The WWC identified three eligible group design studies and 37 eligible single-case design studies that investigated the effects of "PRT" on children and students with an autism spectrum disorder. The WWC reviewed three eligible studies against group design standards. Book excerpt: Dissemination of evidence-based interventions for autism spectrum disorders (ASD) is a major challenge facing the field. Collaboration between researchers and practitioners aimed towards developing and adapting interventions with strong scientific support may aid in the widespread adoption of evidence-based intervention in community classrooms. The following three studies were motivated by teacher feedback on the use of one evidence based intervention, Pivotal Response Training (PRT), in the classroom. Two intervention components were selected for examination based on teacher report of difficulty with use and subsequent omission and observed difficulty with implementation: response to multiple cues and turn taking. Response to multiple cues was evaluated by administering a discrimination learning assessment to typically developing children to determine the age at which it is developmentally appropriate to incorporate conditional discriminations into instruction (Chapter 1). The discrimination learning assessment was also administered to children with ASD to determine the extent to which today's population of children receiving intervention services has difficulty with overselectivity (Chapter 2). Results demonstrated that typically developing children do not consistently respond to simple conditional discriminations until 36 months of age, indicating that response to multiple cues does not need to be incorporated into PRT until children with ASD reach that developmental level. Additionally, a significantly smaller percentage of children with ASD display difficulty with simple conditional discriminations than in previous studies. The turn taking component of PRT was evaluated through a single subject, alternating treatments design that examined the effects of the elements of modeling and contingency on children's communication and play behavior (Chapter 3). Results demonstrated consistent patterns in children's behavior based on the elements of turn taking in use and the functioning level of the child. Avenues for adaptation of the multiple cues and turn taking components and methods of treatment individualization are discussed. Based on these three studies, adaptations to PRT that may support teachers' use of the strategy in the classroom may be made. This research represents a useful model for the iterative and collaborative process of gathering information from front-line practitioners, conducting basic research based on their feedback, and utilizing that basic research to inform clinical practice.

The primary purpose of this dissertation was to examine the effectiveness of pivotal response treatment (PRT) on social behaviors of Korean-American children with autism spectrum disorders (ASD) in their community settings. The secondary purpose was to analyze stakeholders' perceptions (i.e., parents, siblings, community members and the children with ASD) regarding satisfaction and feasibility. A mixed methods design was employed, including a single subject design to evaluate the efficacy of the PRT intervention and a qualitative case study to explain how stakeholders perceived the intervention in terms of its goal setting, procedures, and social behavior changes. Results indicated that peer-mediated PRT effectively increased the social interactions between Korean-American children with autism and their peers in community settings. Additional findings implied that stakeholders were satisfied with the intervention procedures and outcomes, particularly for the intervention's flexibility and applicability as well as children's natural social experiences with peers. Recommendations and implications for practitioners and future researchers are presented.

Book excerpt: The increase in the rate of autism diagnoses has created a growing demand for teachers who are trained to use effective interventions. Research supports behavioral interventions as effective methods for teaching children with autism, however dissemination of these methods has been problematic. Ineffective training and lack of ongoing support after formal training ends may be to blame. The train-the-trainer (ttt) model, which involves training supervisors to train others, may be ideal for providing cost-effective training and ongoing support to teachers. This study assessed the benefits of using the ttt model to disseminate Pivotal Response Training (prt), an evidence-based practice for educating children with autism, to school settings. A multiple baseline design was conducted across three training groups, each consisting of one school staff member (trainer), three special education teachers, and six students. During baseline, each trainer observed interactions between teacher/student dyads and provided feedback to the teachers. During treatment, trainers learned to implement and assess prt themselves and conducted a workshop to educate teachers on prt. After the teacher-training workshop, trainers continued to observe interactions between teacher/student dyads and provide feedback to teachers during weekly classroom observations. A follow-up assessment was conducted three months after training was complete. Assessments included trainer and teacher ability to implement prt, trainer ability to assess prt and provide feedback, and student language and behavioral changes. All trainers conducted the teacher-training workshop with high adherence to training protocol and met mastery criteria in their ability to implement prt, assess implementation of prt, and provide feedback to teachers. Both trainers' assessment of prt and feedback to teachers were variable during post-workshop classroom observations. Six of the nine teachers mastered all components of prt. The remaining three teachers implemented 89% of the prt components correctly. The majority of trainers and teachers maintained their abilities at follow-up. Students demonstrated limited behavioral change, although this was not unexpected as the intervention was minimal and exposure to other interventions was not controlled for as part of the study. These results provide support for the use of the ttt model as an effective method of disseminating evidence-based practices in school settings.
