Book excerpt: The problem facing Westmont Fire Department is how to implement an Automated External Defibrillator (AED) program within the Village of Westmont and the Westmont Fire Department for the purpose of increasing Advanced Life Support (ALS) services to the community. The Westmont Fire Department is challenged on a daily basis to keep pace with the changes in the emergency medical services of the fire service today. The purpose of the Applied Research Project (APR) is to investigate the effect of having Automated External Defibrillators on fire department apparatus and vehicles as well as the development of an operational plan for AEDs within the Village of Westmont. The research process was facilitated using action and evaluation research to answer the following questions: 1. Would an AED program improve emergency services to the community? 2. Would the development and implementation of an AED program be cost effective? 3. How effective are AEDs in actual field use? 4. What are the advantages of having an AED program? The process used to answer the above questions included a ten-question survey mailed to a total of forty fire departments, twenty in the State of Illinois and twenty to the Executive Fire Officer participants across the United States (Appendix A). Thirty-three of the forty questionnaires (82.5%) were completed and returned (Appendix B). The Illinois Fire Chiefs Association and Executive Fire Officer participant departments were chosen because they represent a sample selection of the fire departments found in Illinois and across the nation. The results of this project demonstrated positive effects for the implementation of an AED program within the Village of Westmont Fire Department. The results also indicated the need to strategically implement a plan to place AEDs in businesses, organizational groups, and schools, all of which have high probability for sudden cardiac arrests. It is the author's recommendation that an Automated External Defibrillator program to improve Emergency Medical Services and increase the survival rate of sudden cardiac arrest (SCA) victims be implemented for the Village of Westmont. Book excerpt: Abstract: Automated External Defibrillator (AED) and Cardiopulmonary Resuscitation (CPR) training has become increasingly more common among non-medical individuals. Past studies have shown the benefits of placing AEDs in public settings for use by non-medical first responders and that AED/CPR training of responders is an important factor in resuscitation of victims of cardiac arrest. However, potential users must be aware of AED locations and must be willing to use them and perform CPR. There is currently no location listing of AEDs on the OSU main campus. This naturally leads to questions about community knowledge and awareness of AED locations on campus and willingness and knowledge about their use. A web-based questionnaire with two sets of questions was administered to a sample of faculty and staff and a sample of administrators from The Ohio State University. The goal of the study was to determine if these members of the OSU community know where AEDs are located on campus and to what extent these members are ready or willing to use them. Information about the current training and experience levels of faculty and staff and their interest in participating in AED/CPR training, as well as administrators' experience with AEDs and CPR and their interest in purchasing an AED or providing training was collected through the questionnaire. The questionnaire was answered by 32 staff members, 55 faculty members, and 42 administrators over a two week period. The results from the questionnaire suggest that few faculty, staff, or administrators have knowledge about the presence of AEDs in or near their work locations on campus. The results from the questionnaire showed that while about two-thirds of respondents had been trained in CPR at some point in time, few were current in their training (10.7 percent of those who were trained at some point). Very few had received AED training at any point in time. However, there was a widespread interest in participating in training in use of AED and CPR (95.2 percent of all respondents). Administrators were generally interested in purchasing an AED for their building (80.0 percent responded "yes" or "maybe") and providing training for employees (80.0 percent responded "yes" or "maybe"). Currently there is no system in place for providing AEDs in public areas at The Ohio State University and it is unknown exactly how many AEDs are currently on campus. However, based on this sample there is interest in obtaining AEDs for buildings on main campus that are not part of the medical campus and interest in receiving training in the use of AEDs and performing CPR. Many other college campuses are moving toward comprehensive AED and CPR plans as part of their emergency management programs. Their experience and the interest on the part of administrators, faculty and staff who participated in this study could support the development of an AED program at The Ohio State University.

Book excerpt: Out-of-hospital cardiac arrest (OHCA) is an often fatal, time-sensitive emergency event. OHCA survival decreases significantly for every minute without treatment. Defibrillation, an effective means to resuscitate OHCA victims, can be safely delivered by untrained bystanders using an automated external defibrillator (AED). The dissemination of AEDs in public areas for bystander use has been shown to reduce the time to defibrillation and triple OHCA survival. However, bystander AED use and OHCA survival remain low despite the increasing amount of resources dedicated to AED placement. Identifying the limitations of existing AED network design and exploring new strategies for AED deployment are crucial to ensure efforts and resources spent on AED placements directly translate to improved OHCA outcomes. To address these questions, this thesis focuses on increasing the understanding of spatiotemporal OHCA risk and developing novel tools to improve the assessment and design of AED networks, using optimization and machine learning techniques, with the ultimate goal of increasing bystander AED use and OHCA survival. Specifically, we analyzed AED placement guidelines, existing AED networks, bystander response records, and real OHCA patient data from Toronto, Canada, and Copenhagen, Denmark and identified limited temporal AED accessibility as a key factor responsible for low AED use rates. Motivated by this finding, we developed a spatiotemporal AED placement optimization model that maximizes spatial and temporal AED accessibility. We showed our model reverses the effects of limited temporal AED accessibility in both Toronto and Copenhagen, two vastly different cities with distinct geographies, populations, and EMS systems. This demonstrated the effectiveness and generalizability of our optimization approach. To validate the clinical value of our optimization model, we developed a novel in silico trial framework that computationally evaluates AED networks in a real-life deployment situation. Using this framework, we conducted two in silico trials and established the superiority of optimization approaches to current practices and established placement guidelines, which represent the gold standard of evidence-based AED placement recommendations, in improving estimated OHCA outcomes. In conclusion, our findings identify the importance of temporal AED accessibility, data-driven AED placement optimization models, and centralized decision making in improving AED use and OHCA survival.

Descriptive, observational, cross-sectional study carried out over three years, which evaluates and compares the results of the various training programs (6 hours, 3 hours, recertification and demonstration workshops) in cardiopulmonary resuscitation (CPR) and AED use in a sample of 326 participants. The data collection instruments are a questionnaire and a high-fidelity manikin. Contingency tables, chi-square and Kruskal-Wallis test have been used for the statistical analysis. Opening the airway is done correctly by 59.6% in the 6h course and 52.2% in the recertification course. However, a large percentage of participants from the 3h course (72.7%) and especially from the workshops (95.3%) do not open the airway during AED use. What can the Channahon Fire Protection District do to educate the public first responders on the proper use of automatic external defibrillators? The principal procedures utilized to complete the research project were a review of the fire service literature contained in magazine articles, fire service journals, and resources of the United States Fire Administration. Also referenced was literature available through the American Medical Association. The major results of this research were that successful early defibrillation programs involved delivery strategies that focused on rapid intervention as time was the major component to quality in an automatic external defibrillator program (Browett, 1998). Experts believed that more than three hundred lives a day could be saved if the maximum time to defibrillation were in the five to seven minute range (Newman,1997). Survival rates for out-of-hospital cardiac arrest are as low as 1.4% (Olson, Fontanarosa,1999) with current intervention techniques. Other factors that might influence the placement of AED's included call volume, response-area demography and the distribution of advanced cardiac life support (ACLS) and basic life support (BLS) response times (Becker,Mandell,1991). The recommendations resulting from this research included the Channahon Fire Protection District developing a comprehensive plan and framework for implementing an early defibrillation program, specific to its resouces and particular needs.
