Documented studies show that in rehabilitation units, falls are the leading causes of increased immobility, morbidity and mortality. For fall prevention identifying the high risk patients are essential, in this paper the author is introducing the Morse Fall Risk assessment tool to identify the high risk patients. In this paper researcher has reviewed fifteen articles that focuses on this identified problem and supports the proposed solution. This author has incorporated Henderson's need theory in this project, because this theory explains how a nurse can assist people to increase their independence to promote health and safety in and out of the hospital. The educational materials used in the project are power point presentation, Morse Fall Risk Assessment tool, fall prevention strategies, fall prevention protocol, environmental risk assessment check list, and hourly rounding signup sheets. This project will be evaluated using a simple survey to assess nurse's perception on Morse Fall Risk Assessment tool, and also a fall risk assessment audit tool is used for evaluation. This paper also explains the disseminating method of the study results to the stake holders. The purpose of this paper is to improve the current fall prevention program in the rehab unit by introducing Morse Fall Scale to identify high risk patients, and educate the staff, patients and families the evidence based fall prevention interventions using relevant educational tools.

It is dependent on and consistent with the vision articulated in the WHO Active Ageing Policy Framework. Although not all of the awareness, assessment, and intervention strategies identified in the model apply equally well in all regions of the world, there are significant evidence-based strategies that can be effectively implemented in all regions and cultures. The degree to which progress will be made depends on to the success in integrating falls prevention strategies into the overall health and social care agendas globally. In order to do this effectively, it is necessary to identify and implement culturally appropriate, evidence-based policies and procedures. This requires multi-sectoral, collaborations, strong commitment to public and professional education, interaction based on evidence drawn from a variety of traditional, complementary, and alternative sources. Although the understanding of the evidence-base is growing, there is much that is not yet understood. Thus, there is an urgent need for continued research in all areas of falls prevention and treatment in order to better understand the scope of the problem worldwide. In particular, more evidence of the cost-effectiveness of interconnections is needed to develop strategies that are most likely to be effective in specific setting and population sub-groups.

Abstract: Background: Falls are one of the most common and serious problems facing the elderly and are known to be associated with significant mortality, morbidity, decreased functioning and premature institutionalization. In Hong Kong, the prevalence of falls among community-dwelling older adults is 19.3%. Moreover, the incidence of falls among older people in institutions is almost three times the fall rates for the community-dwelling elderly. Institutional falls are regarded as common adverse events in hospitalized older patients. Significant mortality, morbidity and healthcare costs associated with institutional falls led institutions to recognize falls as a high-priority safety risk for hospitalized patients. This demonstrated the significance of providing the health care providers with an evidenced-based practice guideline of an effective multifactorial fall prevention programme in order to prevent in-patient falls. Objectives: The objectives of the study are to systematically review and present the best evidence for the effectiveness of multifactorial fall prevention interventions in reducing falls in hospitals, to translate the reviewed evidence and to develop evidence-based practice guidelines for the multifactorial fall prevention programme as well as to develop a plan for implementing and evaluating the multifactorial fall prevention programme. Methods: The relevant literature was searched by several electronic databases. The related literature was then retrieved, reviewed and synthesized. The quality assessment of the studies was performed according to the methodological checklist for controlled trials designed by the Scottish intercollegiate Guideline Network (SIGN). Evidenced-based practice guidelines for the multifactorial fall prevention programme were then synthesized according to the findings of the reviewed literature, while the implementation potential being assessed in terms of transferability, feasibility and the cost-benefit ratio. Results: Five studies were identified according to the inclusion and exclusion criteria set. "Evidence-based guidelines of fall prevention programme for hospitalized older patients" were formulated based on the review of the selected studies. Fourteen recommendations of the evidence-based guidelines are formulated and graded according to the grading system of Scottish Intercollegiate Guidelines Network (SIGN). The evidence-based recommendations can offer nurses and other health care professionals the standards and strategies required for implementing multifactorial fall risk assessment and multifactorial fall prevention interventions, including environmental modifications, knowledge, medication reviews and exercise. A communication plan for various parties in hospitals including a pilot test for determining the feasibility of the innovation and an evaluation plan to determine the effectiveness of the fall prevention programme were subsequently developed. Conclusion: This study reviewed evidence for the effectiveness of the multifactorial fall prevention programme in reducing the incidence of falls, translated the reviewed evidence and developed evidence-based guidelines for a multifactorial fall prevention programme, which can provide the health care practitioners with an evidence-based approach in fall risk assessment and management so as to prevent in-patient falls. The aim of the project is to create more awareness among nurses and other care givers about fall risk assessment, fall prevention strategies and modify the caring environment with learned knowledge and skills of the staff. An educational manual development and education program for nurses about the importance of accurate fall risk assessment and multifaceted intervention strategies from evidence can reduce falls at ASECU. Implementation of multifaceted interventions can increase the safety and quality of care associated with decreased health care costs. Spot checks and chart audits daily for two weeks, then weekly on all high risk for fall patients can be done to evaluate the compliance. The success of the program is significant reduction in fall rate and that is evaluated every month, every three month, every six month and then yearly following the implementation. The results are disseminated to nurses and other health care providers inside the hospital through staff meetings and e-letter monthly. To the community it is accessible through magazine H2U, flyers, and brochures. The video-on-demand and website are further tools for dissemination to the stakeholders. Falls can be reduced or prevented with proper and accurate assessments, intervention strategies and following the guidelines at ASECU, at Sunrise Hospital.

Falls are one of the top reported events that occur in hospitals and it is a patient safety concern that requires the implementation of evidence-based practices to reduce falls. This quality improvement project will be developed by a master's prepared clinical nurse leader (CNL) on a medical-surgical unit to improve patient safety. Problem Maintaining patient safety is the most important priority in health care. Health care organizations implement protocols, policies and procedures to ensure that care is provided in a safe manner to minimize preventable harms. However, falls are unexpected incidences that occur in health care settings but are considered to be preventable occurrences. Falls can lead to serious injuries and even death; these events are known as sentinel events (The Joint Commission [TJC], 2013). According to the TJC (2016) inpatient falls are one of the top reported sentinel events occurring in hospitals and are considered a serious problem because it compromises patient safety. According to Walsh et al. (2018) and Zhao et al. (2019) in the U.S. the average fall rate of adults in a medical and surgical (M/S) unit is 3-5 falls per 1,000 patient days, in which 26.1% result in serious injuries including death. The existing problem in a Medical-Surgical (M/S) unit is that there was a 44% increase in falls this year in comparison to the previous year. Therefore, implementing a fall prevention program focusing on patient mobility is imperative. Context The M/S is an inpatient unit that has three floors with a total of 110 beds that serves a diverse population of patients. The M/S unit provides medical services adult patients. The unit provides treatment for acute and chronic medical conditions such as cardiovascular, pulmonary, and renal diseases as well as others. The M/S unit has been diligently working on focusing on purposeful hourly rounding as a method to decrease in patient falls but it has not yielded the desirable outcome of a reduction in falls. Interventions The proposed plan is to implement a fall prevention program that is centered around promoting patient mobility on the M/S unit to decrease patient falls. The CNL will establish a multidisciplinary team known as the mobility/fall task force to collaborate on developing a standardized mobility program that can be modified to meet the needs of each patient. This program will be multifaceted as it will also include performing a fall and mobility assessment and the use of several mobility tools in place along with the development of a patient mobility goal plan tool that will be incorporated into this fall prevention program. The goal is to reduce the incidence of inpatient falls occurring in the M/S unit by 25% by the end of December 2019 and 50% by December 2020. This program is currently underway. Measures To determine the success of this project, several data sets will be collected by auditing nursing documentation of the following: nursing fall risk and functional mobility assessments, patient daily mobility activities, and hourly rounding. Additional data will be collected through the review of incident fall reports as well as the review of monthly length of stay. These measurements will be analyzed first on a weekly basis for two weeks, followed by a bi-weekly basis for four weeks, then monthly for six months, and every three months thereafter. The data collected will then be entered in a Microsoft Excel spreadsheet to graph the information to depict the changes occurring over time. The graphs will reflect if there are any changes indicating positive outcomes through the introduction of this fall prevention program. Results The fall prevention program was initiated in August 5, 2019 and trialed for two weeks on all three floors in the M/S unit. The program officially began on August 19, 2019. The outcomes measured included fall rates, percent of patient being ambulated and length of stay. At this point, there is insufficient data available to determine the trend of the success, but the initial results indicate positive outcomes since the program was implemented. Although it is not the target goal of 25% reduction, data indicates that there has been a 7% reduction in falls. Further, data shows that there also was a decrease in length of stay (LOS). However, the percent of patients being ambulated was below the desired target goal. Conclusion The preliminary results indicate that having the CNL implement and lead the fall prevention program has shown positive outcomes in the reduction of falls occurring in the M/S unit per month. To continue the success of the program, the CNL will continue working closely with staff to assign a unit champion by floor per shift to ensure sustainability. The mobility/fall task force will continue meeting regularly to monitor success of the program and discuss ways to continue preventing falls. The success of this program will indicate that having a CNL collaborate with a team to implement quality improvement projects can lead to improved patient safety and better patient outcomes.

This proposal will delineate an implementation plan for incorporating these three nursing interventions into One South's falls prevention protocol.

Prevention strategies should emphasize education, training, creating safer environments, prioritizing fall-related research and establishing effective policies to reduce risk (World Health Organization, 2014). One solution to falls would be the initiation of hourly rounding. Hourly rounding has been shown to reduce falls as well as improve patient satisfaction and promotes quality use of healthcare worker's time. The implementation of hourly rounding would require a team of workers dedicated to the work of fall prevention. This team would work together to educate professionals on hourly rounding using presentations and pamphlets. Evaluation is key to understand where work can improve in the hourly rounding process and to support findings no matter how small or large. Implementation should be done for a minimum of twelve weeks along with support from leaders and team members. Following implementation and evaluation, dissemination is key to acknowledge the work done and the effort done by staff.

Book excerpt: In this research facility, the falls risk assessment that was previously used was replaced with an evidenced-based tool, the Morse Falls Scale. Fall rates were analyzed for a five month period after implementation of the new scale, from May 1, 2016 – September 30, 2016, and compared to the same months the previous year to see if fall rates decrease with using the evidenced-based scale. Fall prevention is multi-factorial and begins with an assessment of the patients risk for a fall, and interventions which are personalized for each patient. Fall prevention should include all staff, as anyone can make a difference in preventing falls. In this study, fall rates did start to slightly decrease in the months following implementation of the new scale. Further research is needed to see if the fall rates continued to decline long term.

Patient falls are a leading cause of injuries in hospitals, considered to be among the most expensive adverse event, and continue to be a patient safety concern (Trepanier & Hilsenbeck, 2014). Realizing this problem, much research has developed that aims to resolve or at best, reduce the quantity of falls that hospitals experience. Some research looks at education on falls, while others aim to resolve the issue by investigating medications that cause falls. Plenty of research is available to help support health care change in reducing falls, however, not one simple answer is provided in this research. This capstone will aim to provide healthcare workers with education on the research available on fall prevention, as well as include research suggestions in daily practice while keeping track of the implementations used and aim to test if a fall reduction goal of 50% can be met. Problems that bedside nurses encounter while implementing fall preventions are one of the main reasons why health care experiences falls in their facilities. Of course it is impossible to guarantee that all falls will be eliminated, however, there is much that can be done to reduce the risk and improve outcomes of fall prevention. To do this, it is important to understand where the problem begins. Many nurses feel overwhelmed by the abundant amount of tasks mounting up as they go about their day. Is there a simple solution to fall prevention? Can one intervention prove to work better than all the rest? Are bed alarms enough to reduce falls? This paper will show what research found, will educate on best evidence practice, and will propose a project in fall reduction to help meet a company's goal of fall reduction by 50% in a given time.
