Changes made to medications in the time between Primary Care Providers office visits also create a potential for medication error. Research has indicated there are multiple areas for potential breakdown in the medication reconciliation process. Some of these areas include when a patient is admitted to the hospital from home, moving from department to department in the inpatient setting, discharging a patient home after hospital stay and from home to the Primary Care Providers follow-up visit. Addressing medication reconciliation at the Primary Care Provider (PCP) level and implementing a standardized process to address the steps for obtaining the information pertinent to medication reconciliation is imperative to the success of reducing error and potential harm to patients in the outpatient setting. Education of staff in the process of obtaining medication information as well as audit tools for assessing the compliance of following the process at each level can insure effective medication reconciliation at the PCP level, thereby reducing error and enhancing patient safety with medication.

Book excerpt: Background and Significance: According to the Institute of Medicine (IOM), medication-related errors lead to mortality and adverse effects such as allergic reactions, avoidable side effects, drug interactions, and unnecessary cost (IOM, 2000). Even with the predominant use of computerized provider order entry (CPOE) and clinical decision support systems (CDSS), medication discrepancies are still problematic today. There is substantial evidence available to demonstrate the benefits of using a transition of care (TOC) team for timely, appropriate medication reconciliation (MR) utilizing multiple intervention modalities. Purpose: This quality improvement (QI) project attempted to determine if the implementation of a TOC team utilizing a telephone-based MR system in a geriatric primary care setting improved identification of medication discrepancies and improved seven-day follow-up (FU) appointments following an inpatient admission. Methods: The TOC team at the geriatric primary care clinic was educated about the improved MR intervention tool and data collection process via a PowerPoint presentation. The data was collected via an Excel spreadsheet and analyzed for improvement upon identified medication errors and follow-through with timely FU appointments. The plan-do-study-act (PDSA) cycle was utilized for implementation. Results: A QI project was implemented in a geriatric primary care setting including 74 patients, 18 of which received the intervention. The implementation was carried out over a total of 12 weeks. Error identification increased from 0.71 errors per patient in the control group to 1.22 in the intervention group. Of the patients contacted via MR phone calls, 88.8% scheduled FU appointments; only 38.8% were within the goal time frame of seven days. The average time to FU during the implementation period was 9.1 days. Conclusion: The TOC team utilizing a telephonic MR improved identification of medication errors and overall FU rates. However, the TOC team did not positively impact the seven-day FU during MR from an inpatient hospitalization visit back to the outpatient setting. Results also indicated an improvement in the time spent per phone call. As an preop/postop outpatient unit, we see an average of 50 patients daily and perform medication reconciliations regularly. The problem is our nurses perform this task inconsistently and often incomplete, leading us to potential medication errors. During my CNL project, I worked with a committee of staff champions and developed a standard work guide for nurses to use in order to effective reconcile patient medications. We used fishbone diagrams, audit tools, surveys and led regular discussions in our huddles in order to develop this guide. Once implementation is complete, we hope to have at least 80% of our medication reconciliations to be accurate and a decrease in medication error related safety incident reports to no more than 21 reports (a 50% improvement) this fiscal year, ending in August 2016. In order to maintain sustainability, our staff champions will serve as resources for staff and we will implement our standardized medication reconciliation into our orientation and annual skills competency review.

Book excerpt: It is estimated, that ambulatory care settings have a 25% adverse drug events (ADEs) rate, and 39% of those event were preventable errors (Tache, Sonnichsen, and Ashcroft, 2011). Considering many adverse drug events are related to medication errors, preventing medication errors is fundamental to improving patient safety and outcomes. Medication reconciliation is the process of identifying and resolving medication discrepancies that occur, during transitions in care. Patient participation is a key component to the medication reconciliation process. With the intent to improve patient participation, a patient awareness intervention was implemented in the cardiology outpatient clinic. Data was collected using microsystem assessments, staff /patient medication reconciliation questionnaires. The intervention includes the use of patient posters, brochures and pre-appointment phone call reminders to bring in their medications. The barriers to implementing the patient awareness intervention in this clinic were in part related to resistance to change and lack of understanding of the medication reconciliation process. The barriers to this process will be further discussed, in this paper. The patient pre-appointment phone calls resulted in a 7% increase in patients bringing in their medications. As a result, the care providers were able to verify and reconcile the patient medications at the appointment.

Book excerpt: Soon after the 1999 Institute of Medicine report, To Err Is Human, was released, the Department of Defense (DoD) began an aggressive examination of medical errors and the strategies for minimizing them. A primary goal was the creation of a standardized medication event reporting system, including a central registry for the compilation of reported data. This paper describes important experiences gleaned from the DoD's transition to a standardized medication error reporting system. MEDMARXSM, an Internet-based commercial reporting application, was selected by the DoD leadership as the standard tool for medication event reporting. MEDMARX was implemented initially at five military hospitals in fall 2000 as part of a patient safety pilot project, and was later made available to all 143 military treatment facilities worldwide. Medication errors represent approximately 50 percent of all patient safety events reported by military health care facilities. Although the challenges associated with the implementation of a standardized error reporting system were considerable in number and scope, the long-term benefits to the DoD are significant.
