Book excerpt: De nos jours, l’implantologie est de plus en plus utilisée en médecine dentaire. Cependant demeure la question de l’interaction de l’implant avec le parodonte. Deux méthodes sont préconisées pour procéder à la pose de l’implant: Système enfoui, système non-enfoui: L'école suédoise (Branemark) préconise la mise en nourrice des implants pendant la phase d’ostéointégration. Puis, une deuxième intervention les connecte au milieu buccal. En revanche, pour Schroeder, une seule intervention est nécessaire pour assurer l’ostéointégration de l’implant: l’enfouissement de l’implant pendant la phase de cicatrisation osseuse n’est plus ainsi considérée comme une condition sine qua non à l’ostéointégration. La technique en deux temps permet d’enfouir l’implant afin de le protéger des bactéries et des contraintes mécaniques de la cavité buccale lors de la phase initiale de l’ostéointégration, de plus elle assure une marge de sécurité en cas de rétraction gingivale lors de la pose du pilier de cicatrisation, cependant elle nécessite -un temps de mise en place plus long. Par ailleurs, de nombreuses études portent vers une implantologie plus rapide avec des protocoles simplifiés: les implants non-enfouis correspondent à une simplification de cette procédure chirurgicale. A l’issue de ces constats, plusieurs problématiques semblent se poser: existe-il une différence au niveau des tissu péri-implantaire à utiliser les implants non-enfouis? L’ostéointégration et la cicatrisation des tissus mous se passent-elles de manière différente entre les deux techniques? Existe-t-il une différence en terme de taux de survie entre les deux techniques? C’est pour répondre à l’ensemble de ces questions que ce thème a été choisi. Pour y parvenir, les généralités relatives au tissu péri-iinplantaire feront l’objet de la première partie puis dans une deuxième partie la comparaison entre les deux méthodes sera analysé et enfm pour finir dans une troisième partie nous verrons les incidences cliniques de ces deux types d’approche chirurgicale

Its subject, reflected in the title "Sciences and Empires", is not a currently-accepted sub-discipline of the history of science; rather, it refers to a set of questions which found autonomy only recently.
