This is mostly due to patient-specific requests for metal-free treatment and concerns about adverse reactions titanium particle corrosion. Therefore, manufacturers are increasingly introducing zirconia implants with different features and designs. While most systems offer one-piece implant design, little is known about the applicability and features of two-piece implants.Aim of the study (300)This review aimed to summarize the preclinical knowledge about two-piece zirconia implant systems. Focus was given on design features, mechanical resistance of the implants and of the implant-restoration complex as well as peri-implant tissue response.Material and Methods (1000)An electronic search of the literature until March 2017 was performed to identify all articles related to preclinical research on two-piece zirconia implant systems. The search was conducted using MEDLINE (National Library of Medicine), Pubmed without restrictions concerning the date of publication. The search terminology included: zirconia implant, two-piece, mechanical properties, fracture resistance, strength, osseointegration, bone-to-implant contact, soft tissue, histology, histomorphometry (connecting different keywords with AND, OR). Hand-searching of the bibliographies of all full-text articles and related reviews, selected from the electronic search, was also performed. The literature search was limited to only English language articles. References appraised in related systematic reviews were also considered.Results (1000)Of the initially-identified 1026 titles, 58 papers were selected for full-text evaluation. After exclusion 53 articles, a total of 6 article were finally selected and analysed. The mechanical resistance was evaluated in 4 studies, where a clinical situation of implants placed in the anterior region was simulated. Interestingly, most studies used titanium abutment screws for evaluation. Irrespective of connection type (screw- or cement-retained), implant fractures were mostly reported in the implant-abutment connection, with the weakest component to be abutment screw, abutment and implant, respectively. In general, two-piece zirconia implant systems showed higher failure rates and lower resistance than one-piece zirconia implants as well as titanium implants.Peri-implant tissues were evaluated in 2 studies. Regardless of implant type (one- or two-piece), a significant marginal bone loss was recorded 6months after placement (0.19-1.11mm). The height of buccal peri-implant mucosa and bone-to-implant contact ranged, respectively, between 2.64-3.03mm and 78.6-87.9% with no statistically significant differences at follow-up evaluations.Conclusion and Clinical implications (500)Irrespective of connection type and the design, two-piece zirconia implants show sufficient strength to withstand normal forces applied in the anterior region. Hard and soft tissue response around two-piece zirconia implants seem to be similar to one-piece implants as well as titanium implants. Although they have the potential for clinical application, the limited knowledge about two-piece zirconia implants does not support their clinical application yet.

Clinical investigations using zirconia ceramic implants over the last couple of years have brought up some new developments and questions. Can we be confident in placing zirconia ceramic implants given the recently published data? Is there a difference in the application of one- and two-piece implants? Recent Findings Systematic reviews on preclinical investigations of zirconia implants revealed that one-piece zirconia implants (> 4 mm) are sufficiently stable for clinical use. The same is true for some clinically available two-piece implant systems. Osseointegration and soft tissue integration are, according to the reviews, similar between titanium and zirconia implants with similar surface topographies. Regarding the clinical outcome, a meta-review exists evaluating systematic reviews. The findings of the systematic reviews and the meta-review are that there are good short-term clinical results for one-piece zirconia implants. However, the data for two-piece implants is not robust. Summary In certain applications (single tooth restorations and small bridges), the results of zirconia implants are comparable with titanium implants in short-term studies. Some mid-term investigations support the short-term results. However, according to the current scientific data available, zirconia implants cannot yet be considered an alternative to titanium implants because there are many areas where there is a lack of clinical studies on zirconia implants. Currently, they are an addendum to the titanium implant armamentarium for situations where they are useful (patient request, known hypersensitivity to titanium, or questions of esthetics when titanium might appear inappropriate for a certain situation/condition), but long-term studies are needed. Without a doubt, there is a need for two-piece zirconia implants, but limited research exists to support their clinical use at the moment

For bone tissue evaluation, linear mixed models with random intercepts were fitted. Twenty-seven patients received one implant for an SC and 13 patients received two implants for a 3-unit FDP. Untextured (S-YTZP) discs were used as controls. SB-AE protocol was applied to all samples to achieve a mean roughness ranging between 2.49 u00b1 0.3u00b5m and 2.03 u00b1 0.48u00b5m (p>0.05). Human osteoblasts were cultured on discs for 14 days. Morphology and cellular adhesion were observed using scanning electron microscopy (SEM) after 1 day. Cell viability and proliferation were evaluated at 1,3,7 and 14 days using a resazurin-based method. Alkaline phosphatase (ALP) activity was evaluated at 7 and 14 days and Collagen type I, Osteopontin and IL-1u03b2 were evaluated at 3 and 7 days using ELISA. Todayu2019s computer-assisted design/computer-assisted manufacture (CAD/CAM) technology allows for fabrication of bars made from titanium but also from zirconium dioxide (ZrO). However, ZrO bars have not been in clinical use on a standard basis. Thus, scientific data with focus on ZrO bar reliability and peri-implant bone resorption are missing.Aim/Hypothesis: To report on the clinical short-term outcomes of CAD/CAM ZrO bars and respective mandibular IODs, and on the radiographical crestal bone level changes after 12-months. The null-hypothesis was that the bar fracture rate would be 5% and no significant crestal bone resorption would occur.Materials and Methods: Fifteen edentulous patients (53% men) with a mean age of 62 years were treated with a total of 30 implants, i.e. two interforaminal standard diameter implants (NobelReplace Select RP) per patient. After a healing time of six weeks, all patients received a mandibular IOD supported by a CAD/CAM ZrO bar milled from one homogenous block. All bars had a rigid design (Dolder) with bilateral 7mm extensions and were retained with 35Ncm directly on the implant, using a specific screw, and without titanium base at the interface. Female parts in the IOD were prefabricated gold attachments. The maxilla was rehabilitated with a complete denture and a lingualised occlusion concept was applied. Panoramic radiographs were taken at delivery and at the one year appointment. Prosthetic complications such as bar fractures and the need for maintenance service of the anchorage system, as well as biological outcomes including implant survival and crestal peri-implant bone level changes were recorded.Resultats: All the 30 implants osseointegrated without complications and were successfully in use at the 12-months recalls. One patient showed generalised hyperplasia of the mucosa around the bar. Radiographic measurements of the crestal bone resorption at mesial and distal implant sites revealed bone level changes of -2.9mm to +1.4mm (mean +0.2mm) after 12 months. While 10% percent of the sites showed bone levels with an increase of 1mm and 62% between 0 and 1mm, 27% had a loss of maximally 1mm and one site was measured -2.9mm without clinical signs of inflammation. All the 15 ZrO bars and corresponding IODs were successfully in use without fracture or any technical complications. No special maintaining service was recorded other than removal of superficial bar discoloration at lingual sites.Conclusions and Clinical Implications: None of the 15 ZrO bars and IODs showed a fracture or a prosthetic complication after 12 months. Survival and success rate of the 30 implants were 100% with stable crestal bone levels. These short-term results of this prospective study showed that the use of ZrO bar with its unique biocompatibility and, due to its white colour, easiness to perform and control good cleansability for elder patients is possible with a high predictability and without technical or biological complications.

Book excerpt: The objective of this study was to evaluate the in vivo performance of the hydroxyapatite (HA) coating and hydrofluoric acid (HF) etching zirconia (ZrO) implants and to compare the result with titanium (Ti) implants treated in a similar manner. A total of four different implant types were tested in this study. Threaded zirconia implants with HA coating (Test 1) and zirconia implants with HF-treated surfaces (Test 2) were used to compare to the same size of titanium implants treated in identical fashion (control 1 and control 2). All implants measured about 3.5 mm at the thread diameter and 7.0 mm in total length. Each rabbit received two zirconia and two titanium implants treated in the same manner (either HA-coated or HF-etched). The samples were implanted into the rabbit tibias and retrieved at 6 weeks. Upon retrieval, 24 specimens (6 samples for each group) were fixed and dehydrated. The samples were then embedded undecalcified in PMMA for histomorphometry to quantify the bone-to-implant contact (BIC). Another 24 samples were kept in 0.9% saline and were evaluated using removal torque (RT) analysis to assess the strength of the implant-to-bone interface. The histomorphometric examination demonstrated direct bone-to-implant contact for all four groups. HA particle separation from the implants surface was seen in a majority of the HA-coated samples. No signs of inflammation or foreign body reaction were found during examination. Due to the HA particle smear contamination in the ZrO-HA group, no data was collected in this group. The mean BIC at the first three threads of the Ti-HA, Ti-HF and ZrO-HF were 57.78"18.22%, 46.41"14.55% and 47.41"14.05%, respectively. No statistically significant difference was found pair-wise among these three groups. When comparing the BIC data with the machined-surface implants, a statistically significant difference was found between the Ti-HA versus Ti implant group and the Ti-HF versus Ti implant group. Book excerpt: Introduction: The purpose of this study was to measure the force required to remove individual CAD/CAM monolithic 1.5mm thick zirconia copings cemented with a definitive or a provisional cement on zirconia and titanium stock implant abutments after single application of erbium-doped yttrium scandium gallium garnet (Er:YSGG) laser; comparing them to non-lased controls. Further, the temperature changes were assessed during laser application on cemented crowns, through thermocouples attached at the collars of 2 implants embedded in a stereolithographic model of the mandible. Materials and Methods: Resin- modified Glass ionomer (Rx; RelyX Luting Plus) and zinc oxide eugenol (Tb; TempBond) were used to cement a total of thirty-six monolithic zirconia (Zr; Zenostar® Zr Translucent) copings on new stock zirconia (Zr; ZirDesignTM) and titanium (Ti; TiDesignTM) abutments in this study. These abutment-coping specimens were randomly divided into 12 groups of 3 each, based on laser application (or control; C), abutment type, cement used, and storage conditions (D; 24 hour dry vs. W; 7 day water storage). Er:YSGG laser was applied at 6W, 30% water-60% air and 20 Hz (300mJ/pulse energy), post cementationfollowing a defined pattern. Force required to remove all the cemented copings from their abutments was measured on a universal testing machine (Instron Corp model 4204). Descriptive statistics, multi- factor analysis of variance (ANOVA) and Post Hoc Tukey’s HSD tests (a = .05) were performed. The temperature readings were recorded during laser application on a separate set of cemented crowns, with a four channel data logger thermometer (OMEGA Engineering Inc model HH309A).

Book excerpt: Abstract: The intention of this 5-year prospective cohort investigation was to clinically and radiographically investigate the outcomes of a one-piece zirconia implant system for single tooth replacement. Sixty-five patients received a total of 66 single-tooth implants. All implants immediately received temporary restorations and were finally restored with all-ceramic crowns. Follow-ups were performed at the prosthetic delivery, after 1, 3, and 5 years. Peri-implant and dental soft-tissue parameters were evaluated and patient-reported outcomes recorded. To monitor peri-implant bone remodelling, standardised radiographs were taken at the implant insertion and at the 1-, 3-, and 5-year follow-ups. In the course of 5 years, 14 implants were lost, resulting in a cumulative implant survival rate of 78.2%. The mean marginal bone loss from the implant insertion to the 5-year follow-up amounted to 1.12 mm. Probing depth, clinical attachment level, bleeding, and plaque index increased over time. In 91.5% of the implants, the papilla index showed levels of 1 or 2, respectively. At the end of the study, the patient satisfaction was higher compared to the pre-treatment measurements. Due to the low survival rate after five years and the noticeably high frequency of advanced bone loss observed in this study, the implant has not met the launch criteria, as it would have not been recommended for routine clinical use
