In conducting the study, the Legislative Management is to consult with the Insurance Commissioner, Department of Health and Human Services, board of CHAND, and Public Employees Retirement System (PERS). The study must include analysis of the enrollees, transitioning of the current enrollees' plans to potential Affordable Care Act (ACA) plans, Medicaid programs, and PERS plans, and any other options determined by the committee. The study also must include a process to discontinue any new enrollment into CHAND immediately and transition all major medical plans in effect as soon as plan year 2024. The Legislative Management shall report its findings and recommendations, together with any legislation required to implement the recommendations, to the 69 th Legislative Assembly.

When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

In performing the study, the legislative management may consider population shifts, facility needs, personnel needs, rural access, regulatory public health functions, and vulnerable populations; determine the scope of the weakness in the current health care system; take into account the ongoing impact that federal health care reform under the federal Affordable Care Act (ACA) is having on state delivery of health care and on state delivery of Medicaid; and consider how to forge partnerships with federal payers and regulators in order to work toward addressing medical reimbursement system reform. Section 1 of House Bill No. 1034 (2013) directs the Legislative Management to study health care reform options, including the implementation of the ACA if the federal law remains in effect and state alternatives for state-based health care reform if the federal law is repealed. As part of this study, the Insurance Commissioner, State Department of Health, and Department of Human Services shall provide status reports on the state of health insurance and health-related public assistance. Section 3 of House Bill No. 1362 (2013) directs the Legislative Management to consider studying the effects of the ACA, due to the dramatically changing health care system in the state, including alternatives to the ACA and the Medicaid expansion provisions to make health care more accessible and affordable to the citizens of the state, including access, the cost of providing services, the Medicare penalty to the state's providers, and the Medicaid payment system.

The United States pays more for healthcare, has poorer health outcomes, and the highest uninsured rate of the developed world as well as rampant health inequities across race, gender, and class. In an attempt to remedy several of these issues and provide health insurance to low-income Americans, the Patient Protection and Affordable Care Act (ACA) was passed in 2010. As a part of the ACA, Medicaid was expanded to cover individuals living below 138% of the federal poverty line. Given that the Supreme Court determined that states should have the choice to expand Medicaid, only 25 states expanded Medicaid by January 2014. Using comprehensive health data from the Behavioral Risk Factor Surveillance System, this study explores whether the ACA, and particularly, the expansion of Medicaid, reduced racial and ethnic disparities in health insurance, access, utilization, and outcomes. By employing a difference-in-difference strategy, we find that racial disparities in health insurance coverage, financial risk from healthcare costs, and self-reported general health and mental health decrease. However, there are ambiguous effects on health utilization and physical health outcomes. These improvements are not uniform across race: blacks experience far greater improvements than Hispanics, relative to non-Hispanic whites. Additionally, there are no remarkable differences in racial and ethnic disparities in health between expansion and non-expansion states and individuals living below and above 138% of the federal poverty line after the full implementation of the ACA.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Many of them have been pushed out of the mainstream because of an economy and society where the odds are stacked against them; others have chosen to be on the outskirts of twenty-first-century America. These men are disconnected from work, personal relationships, family and children, and civic and community life. They may be angry at government, employers, women, and "the system" in general—and millions of them have done time in prison and have cast aside many social norms. Sadly, too many of these men are unsure what it means to be a man in contemporary society. Wives or partners reject them; children are estranged from them; and family, friends, and neighbors are embarrassed by them. Many have disappeared into a netherworld of drugs, alcohol, poor health, loneliness, misogyny, economic insecurity, online gaming, pornography, other off-the-grid corners of the internet, and a fantasy world of starting their own business or even writing the Great American novel. Most of the men described in this book are poorly educated, with low incomes and often with very few prospects for rewarding employment. They are also disproportionately found among millennials, those over 50, and African American men. Increasingly, however, these lost men are discovered even in tony suburbs and throughout the nation. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.
