Nous avions dans un 1er temps étudié les caractéristiques des patientes ayant reçu une chimiothérapie néo adjuvante. Méthodes : les patientes prises en charge à l'institut Curie durant la période 2003-2012 étaient incluses. Les données ont été collectées de façon prospective. Les critères d'exclusion étaient les maladies métastatiques d'emblée, les patientes n'ayant pas reçu de chimiothérapie et les cancers unifocaux. Les caractéristiques des patientes ainsi que les données sur les récidives et les décès étaient colligés. Résultats : 14 154 patientes atteintes d'un cancer du sein étaient suivies à l'institut Curie entre 2003 et 2012. Après exclusion de 13 110 patientes, 1044 patientes atteintes d'un cancer du sein multifocal ont été incluses. Parmi elles, 165 (16%) patientes ont reçu une chimiothérapie néo adjuvante. Après traitement néo adjuvant, 47% (n=78) des patientes ont eu un traitement chirurgical conservateur et 53% (n=87) un traitement radical. L’objectif secondaire est l’étude du rôle et des indications de la radiothérapie après chimiothérapie néo-adjuvante et traitement chirurgical conservateur. Matériels et méthodes : Analyse rétrospective de 290 patientes prises en charge, de 1990 à 2004, à l’Institut Bergonié à Bordeaux, pour un cancer du sein non métastatique traité par une chimiothérapie première suivie d’un traitement chirurgical conservateur et d’une radiothérapie. Etude des taux de rechute locorégionale en fonction de plusieurs facteurs pronostiques potentiels. Résultats : L’analyse univariée a permis d’identifier cinq facteurs pronostiques de rechute locorégionale, statistiquement significatifs : l’âge au diagnostique inférieur à 40 ans (p = 5.69x10-5), la négativité des récepteurs hormonaux (p = 0.0001), l’envahissement ganglionnaire clinique initial (p = 0.006), l’envahissement ganglionnaire au curage axillaire, après chimiothérapie (p 0.0008), la présence de carcinome canalaire in situ dans la pièce opératoire (p = 0.0006). En analyse multivariée, trois de ces facteurs sont indépendants : la négativité des récepteurs hormonaux (RR = 4.015 IC 95% [2.057-7.837], l’âge initial inférieur à 40 ans (RR = 2.801 IC 95% [1.592-4.926]), et l’envahissement ganglionnaire au curage axillaire (RR = 2.302 IC 95% [1.248-4.246]). Conclusion. Dans notre série, la négativité des récepteurs hormonaux, l’âge inférieur à 40 ans et l’envahissement ganglionnaire au curage axillaire sont des facteurs pronostiques de rechute locorégionale après chimiothérapie première et chirurgie conservatrice. Cela soulève la question de la prise en charge thérapeutique optimale pour ces patientes à haut risque : faut-il proposer une mastectomie, ou peut-on intensifier le traitement locorégional et notamment la radiothérapie afin de limiter ce risque de rechute ?

Utilisée initialement afin de pouvoir envisager un traitement conservateur pour les tumeurs localement avancées, elle est de plus en plus utilisée pour les petites tumeurs opérables d'emblée, notamment les tumeurs TNBC et HER2-positives. Non délétère sur la survie, elle permet en outre, de tester la chimio-sensibilité tumorale par l'évaluation de la réponse histologique. La réponse histologique complète est un facteur pronostique majeur, associé à une meilleure survie globale et sans récidive. L'intérêt est de repérer les patientes qui répondent moins bien à la chimiothérapie néo-adjuvante afin d'adapter les modalités du traitement post néo-adjuvant avec des thérapies ciblées au profil moléculaire du reliquat tumoral. L'étude des facteurs prédictifs de la réponse au traitement est donc un sujet primordial en cours d'exploration. Nous allons étudier spécifiquement l'impact de la taille initiale tumorale sur la réponse histologique et le taux de RCH.MATERIELS ET METHODES : Cette étude rétrospective a été réalisée sur une cohorte de 714 patientes prises en charge à l'institut Curie à Paris en France entre 2002 et 2012. Elles présentaient un cancer du sein Tl-T3NxM0, traité par chimiothérapie néo-adjuvante suivie de chirurgie puis de radiothérapie. Le critère de jugement était le score RCB utilisé en linéaire ou en classe, allant de la réponse histologique complète à la maladie résiduelle étendue. La réponse histologique complète (RHC) a été définie comme l'absence de cellules cancéreuses invasives résiduelles dans la pièce de mastectomie et les ganglions axillaires (ypTOis/ypNO).RESULTATS : Nous avons retrouvé une relation linéaire entre la taille tumorale et le score RCB en variable continu et ce dans les 3 sous-types. Quand le RCB était considéré en classe, cette relation linéaire n'était retrouvée que pour les tumeurs luminales. Les petites tumeurs de moins de 2 cm ne répondaient pas mieux à la chimiothérapie néo-adjuvante, avec une grande proportion de RCB IL Le taux global de réponse histologique complète de notre population totale était de 28 % principalement obtenue pour les sous types triple négatifs (39%) et HER2- positives (38%). Enfin, elle était indépendante de la taille tumorale et ce, quel que soit le sous type.CONCLUSION : Même s'il existe une relation linéaire entre la taille tumorale et le score RCB, la taille tumorale initiale n'est pas un facteur prédictif de réponse histologique complète. Les petites tumeurs ne répondent pas mieux à la CNA et en particulier les TNBC et /7FR2-positives. Ces résultats incitent à traiter davantage les tumeurs de moins de 2 cm par chimiothérapie néo-adjuvante afin d'adapter au mieux leurs traitements post néo¬adjuvants en cas de non RHC.

Cette étude rétrospective unicentrique, inclue 80 patientes présentant des carcinomes mammaires uniques traités par chimiothérapie néoadjuvante en vue d'une chirurgie conservatrice. 45 patientes ont bénéficié d'une IRM mammaire à la fin de la chimiothérapie pour évaluer la réponse tumorale. L'IRM mammaire permet une évaluation correcte du résidu tumoral en fin de chimiothérapie néo-adjuvante avec un coefficient de corrélation de 0,503 avec la taille histologique effective (p= 0,0003). Cependant cette technique d'imagerie présente parfois des surestimations et des sous-estimations importantes notamment en cas de carcinome lobulaire infiltrant. Par comparaison, 35 patientes ont bénéficié d'une évaluation de la réponse en fin de chimiothérapie néoadjuvante par échographie mammaire. Ces patientes, en effet, n'auront peut-être pas le temps d'envisager une reconstruction différée avant que n'apparaissent des métastases.
