Les végétariens français sont encore très minoritaires, mais des compromis s'envisagent de plus en plus : les flexitariens, eux, représentent 24% de la population. Ces régimes d'exclusion se voulant bénéfiques pour l'environnement, trouvent de plus en plus d'adeptes. Il convient donc aux professionnels de santé de s'informer sur le sujet, de manière à correctement conseiller et orienter leurs patients. Certains de ces sujets végétariens ont des enfants ou prévoient peut-être d'en avoir : ils peuvent se demander si leur propre régime conviendra à leurs enfants sans prendre le risque qu'ils soient carencés. Il est essentiel de considérer que l'enfant est un sujet à part : son alimentation doit être adaptée pour un développement harmonieux. Des précautions sont de mise, à n'importe quelle étape de la croissance.

Book excerpt: Contexte : les régimes végétariens et végétaliens sont de plus en plus fréquents et représentent en 2018 5,4% de la population française d'après le CREDOC, et plus particulièrement chez les 18-34 ans, potentiels parents ou futurs parents. Objectif : l'objectif de ce travail est d'analyser l'impact sur le développement de l'enfant des régimes végétariens et végétaliens depuis la diversification, pour faire un état des lieux des connaissances et pouvoir adapter le suivi de ces enfants. Matériel et méthodes : une revue systématique de la littérature a été réalisée en interrogeant les bases de données Pubmed, Cochrane, Web of science, Cismef, EM premium, Lissa, et Sudoc. Seules les études de haut niveau de preuve, traitant du développement de l'enfant ayant suivis régime végétarien ou végétalien depuis la diversification, ont été incluses. Deux auteures ont indépendamment sélectionné les études, évalué leur qualité et extrait les données. Des grilles d'évaluation ont permis d'apprécier la qualité de chaque article. Résultats : 22 articles ont été inclus. Les résultats de ces articles apparaissent globalement hétérogènes. Cependant, la croissance en poids et taille sont similaires entre végétariens, végétaliens et omnivores, ainsi que l'apport en fer, avec cependant une ferritine plus basse chez les végétariens par rapport aux omnivores, sans impact sur la numération formule sanguine. En revanche, les végétaliens présentent une carence en calcium et en vitamine B12, s'ils ne sont pas supplémentés. Cela impacte leur développement neurocognitif et leur motricité. L'apport en énergie est suffisant, mais provient davantage des glucides que des protéines dans les deux régimes. Les marqueurs de l'inflammation sont moins présents dans leur prise de sang, ce qui serait protecteur contre les maladies chroniques. Le renouvellement osseux semble plus important chez les végétariens, cependant les résultats des ostéodensitométries sont différents suivant les études. La puberté apparait en moyenne au même âge pour tous les régimes étudiés. Conclusion : les études sélectionnées sont hétérogènes, avec des critères de jugement principalement anthropométriques ou biologiques sans mesure du développement global. Il est donc difficile d'élaborer des conseils, cependant les résultats semblent plutôt rassurants. Des études de suivi à plus long terme seraient intéressantes.

En revanche on trouve peu de données sur le régime végétarien et le risque d'érosion chez l'enfant. L'objectif de cette étude descriptive est d'évaluer la prévalence de l'érosion chez les enfants végétariens. Pour ce faire, les sujets seront recrutés en milieu scolaire. Un questionnaire sur les habitudes alimentaires sera réalisé ainsi qu'un examen clinique de chaque enfant. L'analyse des résultats et la comparaison avec les données retrouvées dans la population infantile générale permettra de déterminer si les enfants végétariens forment un groupe à risque plus important d'érosion. En effet celle-ci constitue un véritable problème de santé publique. Le chirurgien-dentiste doit donc se montrer vigilant vis-à-vis des groupes à risque afin de mettre en place des thérapeutiques préventives et un suivi régulier dès le plus jeune âge.
