Book excerpt: Contexte: La stratification des patients est un enjeu majeur de la prise en charge de l'insuffisance cardiaque systolique. Elle permet au praticien d'orienter vers la transplantation ou l'assistance les patients les plus sévères. L'objectif de notre travail est d'étudier l'impact pronostique de paramètres cliniques (facteurs de risque cardiovasculaire et comorbidités) chez des patients stables porteurs d'une dysfonction systolique du ventricule gauche. Méthode: Nous avons inclus rétrospectivement les patients porteurs d'une insuffisance cardiaque avec dysfonction et/ou dilatation ventriculaire gauche, bénéficiant d'un traitement médical optimal et hospitalisés pour bilan pronostique au CHRU de Lille entre janvier 1991 et décembre 2012. Chaque patient a été évalué sur le plan clinique Ils ont également bénéficié d'un électrocardiogramme, d'une échocardiographie et d'une épreuve d'effort métabolique. Les patients étaient classés selon l'étiologie de la cardiopathie. Le critère de jugement principal était la survie sans transplantation cardiaque urgente ou assistance ventriculaire gauche. Résultats: Sur les 2264 patients ayant bénéficié d'un bilan pronostique entre 1991 et 2012, l'étiologie était ischémique chez 1012 patients (44,7%) et non ischémique chez 1230 patients (54,3%). On dénombrait 796 décès cardiovasculaires et 209 décès non cardiovasculaires. En analyse multivariée, les facteurs prédictifs indépendants de mortalité globale sont l'âge, la FEVG, le stade NYHA de la dyspnée, la présence d'une artérite oblitérante des membres inférieurs ou d'une pathologie pulmonaire, l'étiologie ischémique, un rythme en fibrillation atriale et l'IMC. Ces facteurs sont prédictifs de mortalité globale et de mortalité cardio vasculaire. Conclusion: En plus des paramètres de pronostic bien connus (âge, étiologie, NYHA, FEVG, fibrillation atriale), notre étude a mis en évidence deux puissants paramètres indépendants de pronostic, la présence d'une pathologie respiratoire et pour la première fois à notre connaissance dans cette population, la présence d'une artérite oblitérante des membres inférieurs. Des études complémentaires sont nécessaires pour définir puis valider un score clinique simple qui nous permettrait une stratification rapide des patients en consultation.

Notre objectif était d'étudier les facteurs pronostiques de l'insuffisance cardiaque systolique chez des patients porteurs d'une dysfonction ventriculaire gauche chronique stable, et de comparer leur impact selon l'étiologie ischémique ou non ischémique de leur cardiopathie. Méthode : Ont été inclus l'ensemble des patients porteurs d'une insuffisance cardiaque avec une altération de la FEVG et/ou une dilatation du ventricule gauche, qui ont bénéficié d'un bilan pronostique entre janvier 1991 et décembre 2012 au CHRU de Lille. Chaque patient a été évalué sur le plan clinique, biologique, morphologique et a bénéficié d'une épreuve d'effort métabolique. Le critère de jugement principal était la survie sans transplantation cardiaque ou assistance ventriculaire gauche. Résultats : Sur les 2245 patients ayant bénéficié d'un bilan pronostique entre le 1 er janvier 1991 et le 31 décembre 2012, 1008 (45%) présentaient une insuffisance cardiaque d'étiologie ischémique, 1213 une insuffisance cardiaque d'étiologie non ischémique (55%). L'étiologie ischémique était associée à un pronostic péjoratif en analyse univariée. L'âge, le stade NYHA de la dyspnée, le taux de BNP, le pourcentage du pic de V02, le traitement bétabloquant et le traitement hypolipémiant sont des facteurs pronostiques indépendants de l'insuffisance cardiaque systolique communs aux étiologies ischémiques et non ischémiques. En cas d'étiologie ischémique, la FEVG, le taux de noradrénaline, et la fréquence cardiaque de repos sont des facteurs prédictifs indépendants de mortalité globale. En cas d'étiologie non ischémique, ceux sont la natrémie, la clairance de la créatinine et la prise d'antivitamine K qui sont des facteurs pronostiques spécifiques en analyse multivariée. Conclusion : L'évaluation de l'étiologie est primordiale car certains facteurs pronostiques de l'insuffisance cardiaque systolique diffèrent selon l'étiologie ischémique ou non ischémique de la cardiopathie La création d'un nouveau score de risque intégrant ces différences permettrait d'affiner encore la stratification pronostique des patients.
