Dans cette étude. nous proposons un protocole dïntervention indirecte à des patients avec un trouble spécifique des apprentissages versants lecture et orthographe et nous en mesurerons l'efficacité. Plus précisément nous mesurons lïmpact de cette approche thérapeutique sur la qualité de vie des patients en évaluant leur estime de soi, leur bien-être et leur fonctionnalité en langage écrit. L ·étude compare également l'auto-évaluation de leur qualité de vie par les enfants et l'hétéroévaluation de leur qualité de vie par les parents et les enseignants. Les résultats ont montré une tendance à l'amélioration du bien-être et de la fonctionnalité par rapport au trouble pour r ensemble des sujets. Ils montrent également une tendance à r amélioration de r estime de soi, de la fonctionnalité en langage écrit et pour les devoirs pour trois sujets sur quatre. Enfin les résultats font apparaître des discordances importantes entre les évaluations des enfants et celles des parents et des enseignants. différences qui persistent après le suivi orthophonique. Cette étude renforce I ïntérêt de la complémentarité entre interventions directe et indirecte prônée par de nombreux orthophonistes pour tout type de pathologies. Elle sïnscrit dans une réflexion sur révolution de la place du patient et de rorthophoniste dans le soin.

Nous avons souhaité ainsi démontrer le rôle fondamental de l'environnement de l'enfant dans les processus de construction et d'étayage des compétences linguistiques. Nous avons ensuite analysé les conceptions médicales portant sur les difficultés de langage de l'enfant et nous les avons confrontées aux points de vue de l'école et des parents afin de nous représenter la communication de l'enfant dans son ensemble, telle qu'elle s'actualise concrètement à travers les interactions de sa vie quotidienne. Nous avons notamment tenté de comprendre les différentes représentations véhiculées autour du langage de l'enfant en fonction des attentes de milieux spécifiques: les milieux médicaux, scolaires et familiaux. En nous basant sur les considérations précédentes nous avons souhaité rechercher des outils et des dispositifs permettant d'intégrer les approches interactives et pragmatiques du développement du langage au champ de l'intervention orthophonique. Nous présentons donc dans notre mémoire quelques moyens pratiques de tenir compte des caractéristiques pragmatiques de la communication de l'enfant souffrant d'un trouble du langage dans l'élaboration du projet thérapeutique. Nous avons de plus sollicité par questionnaire un certain nombre d'orthophonistes du Bas-Rhin afin de d'interroger la problématique de notre recherche par rapport aux pratiques professionnelles sur le terrain.

Ils sont dits "à risque" de troubles du langage écrit, mais seule la lecture commence à être étudiée de manière approfondie. Nous avons choisi de nous intéresser à l'orthographe de ces enfants car il semblerait que les troubles observés soient fréquents et résistants. De plus, le carrefour des troubles du langage oral et des troubles du langage écrit soulève des questions dans la littérature : ces troubles sont-ils similaires ? Dans quelle mesure ? Quand les poupulations avec TSL et les populations avec trouble du langage écrit (Dyslexie-Dysorthographie) sont comparées, c'est toujours sur le versant de la lecture. Nous avons choisi de les comparer sur le versant orthographique, afin d'obtenir des données plus complètes sur l'orthographe des enfants avec TSL, et de fournir des données comparatives à ce niveau entre les deux populations. Nous avons proposé trois épreuves d'orthographe (dictée de mots, dictée de texte, production semi-dirigée) à 16 enfants âgés de 9 ans et demi à 11 ans et demi. Les résultats indiquent que tous les enfants avec TSL ont un déficit orthographique modéré à sévère, caractérisé par des difficultés en orthographe phonétique et en morphosyntaxe au premier plan. Les enfants avec DL ont un déficit plus important en orthographe d'usage, et on retrouve moins d'erreurs syntaxiques dans leur corpus. Ces élèments vont dans le sens d'une dysorthographie particulière à la population avec TSL. Cela implique en théorie que le TSL et la DL-DO ne sont pas tout à fait identiques, et que les axes de travail dans la prise en charge des difficultés rencontrées en orthographe ne seront pas les mêmes.

Brown’s writing is clear and aims to describe the various theoretical ideas as plainly as possible. Detailed clinical material is given in most chapters to illustrate the theoretical perspectives. Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
