L’influence du poids sur le pronostic des principaux sous-types de cancers du sein est moins bien connue. Aucune étude n’a encore analysé l’impact de l’obésité ou de la variation de poids sur l’efficacité de la chimiothérapie et la survie des patientes recevant du trastuzumab pour un cancer du sein surexprimant HER2, en situation néoadjuvante. L’objectif de ce travail est d’évaluer 1) l’influence de l’index de masse corporelle (IMC) ou de la variation de poids sur la réponse à la chimiothérapie néoadjuvante à base de trastuzumab, chez des patientes présentant des tumeurs surexprimant HER2 et 2) l’influence de ces paramètres sur la survie. Patientes et méthodes De mars 2003 à janvier 2014, 49 patientes ayant reçu au CHRU de Tours ou au CH de Chinon une chimiothérapie néoadjuvante associée au trastuzumab pour un cancer du sein surexprimant HER2 ont été sélectionnées de façon rétrospective. Leur poids a été recueilli au début et à la fin de la chimiothérapie néoadjuvante et du trastuzumab adjuvant. L’influence de l’indice de masse corporelle et de la variation de poids au cours de ces traitements sur la réponse pathologique a été analysée par régression logistique. L’impact de ces paramètres sur la survie sans progression et la survie globale a été analysé par la méthode de Kaplan Meier. Résultats L’âge médian de cette population était de 50 ans, 61% des patientes étaient en surpoids ou obèses au diagnostic. Au cours de la chimiothérapie néoadjuvante, 55% des patientes gardaient un poids stable ou avaient une prise de poids alors que 45% présentaient une perte de poids. Pendant le trastuzumab adjuvant, 80% des patientes avaient un poids stable ou une prise de poids, et 20% une perte de poids. L’IMC au début du traitement n’était pas prédictif de la réponse pathologique et n’était pas associé à la survie. La perte de poids pendant la chimiothérapie néoadjuvante était associée à une moins bonne réponse pathologique tumorale (OR=1,31; IC 95% 1,04 – 1,65 ; p=0,02). En post opératoire, la perte de poids au cours du trastuzumab adjuvant était associée à une diminution de la survie sans progression (p=0,04) et de la survie globale (p=0,03), après un suivi médian de 35 mois. Conclusion Cette étude met en évidence que la perte de poids au cours de la chimiothérapie néoadjuvante ou du trastuzumab adjuvant est un facteur de mauvais pronostic pour les cancers du sein HER 2+. Ces résultats suggèrent que le contrôle du poids au cours du traitement pourrait améliorer le pronostic de ce sous type de cancer du sein.

Utilisée initialement afin de pouvoir envisager un traitement conservateur pour les tumeurs localement avancées, elle est de plus en plus utilisée pour les petites tumeurs opérables d'emblée, notamment les tumeurs TNBC et HER2-positives. Non délétère sur la survie, elle permet en outre, de tester la chimio-sensibilité tumorale par l'évaluation de la réponse histologique. La réponse histologique complète est un facteur pronostique majeur, associé à une meilleure survie globale et sans récidive. L'intérêt est de repérer les patientes qui répondent moins bien à la chimiothérapie néo-adjuvante afin d'adapter les modalités du traitement post néo-adjuvant avec des thérapies ciblées au profil moléculaire du reliquat tumoral. L'étude des facteurs prédictifs de la réponse au traitement est donc un sujet primordial en cours d'exploration. Nous allons étudier spécifiquement l'impact de la taille initiale tumorale sur la réponse histologique et le taux de RCH.MATERIELS ET METHODES : Cette étude rétrospective a été réalisée sur une cohorte de 714 patientes prises en charge à l'institut Curie à Paris en France entre 2002 et 2012. Elles présentaient un cancer du sein Tl-T3NxM0, traité par chimiothérapie néo-adjuvante suivie de chirurgie puis de radiothérapie. Le critère de jugement était le score RCB utilisé en linéaire ou en classe, allant de la réponse histologique complète à la maladie résiduelle étendue. La réponse histologique complète (RHC) a été définie comme l'absence de cellules cancéreuses invasives résiduelles dans la pièce de mastectomie et les ganglions axillaires (ypTOis/ypNO).RESULTATS : Nous avons retrouvé une relation linéaire entre la taille tumorale et le score RCB en variable continu et ce dans les 3 sous-types. Quand le RCB était considéré en classe, cette relation linéaire n'était retrouvée que pour les tumeurs luminales. Les petites tumeurs de moins de 2 cm ne répondaient pas mieux à la chimiothérapie néo-adjuvante, avec une grande proportion de RCB IL Le taux global de réponse histologique complète de notre population totale était de 28 % principalement obtenue pour les sous types triple négatifs (39%) et HER2- positives (38%). Enfin, elle était indépendante de la taille tumorale et ce, quel que soit le sous type.CONCLUSION : Même s'il existe une relation linéaire entre la taille tumorale et le score RCB, la taille tumorale initiale n'est pas un facteur prédictif de réponse histologique complète. Les petites tumeurs ne répondent pas mieux à la CNA et en particulier les TNBC et /7FR2-positives. Ces résultats incitent à traiter davantage les tumeurs de moins de 2 cm par chimiothérapie néo-adjuvante afin d'adapter au mieux leurs traitements post néo¬adjuvants en cas de non RHC.
