Head and neck cancer is a biologically diverse group of cancers that bear a common hallmark - evasion of host immune surveillance through innate or acquired mechanisms. The etiological association between the Human Papilloma virus (HPV) and some squamous head and neck cancers, the Epstein-Barr virus (EBV) and nasopharyngeal cancer has provided further impetus for evaluating immunotherapy in this group of cancers. The successful development of anti-programmed cell death protein-1 (PD-1)/ ligand (PD-L1) and CTLA-4 antibodies in solid tumours has gradually brought immunotherapy into mainstream oncological practice in recent years. Besides immune-checkpoint proteins inhibitors, other forms of immunotherapy such as vaccines, EBV or HPV-targeting therapies and cellular therapies are actively being investigated in clinical trials, either alone or in combination with other conventional treatments such as radiotherapy, chemotherapy and surgery. In clinical setting, the practicing oncologist need to be familiar with some unusual patterns of immunological response such as pseudo-progression and hyper-progression in patients with head and neck cancers who are undergoing treatment with immune-checkpoint inhibitors. Furthermore, the unique side effects of immune-checkpoint inhibitors such as autoimmune toxicities need to be recognized early and treated expediently. The development of biomarkers in predicting response to immune-checkpoint inhibitors has played pivotal roles in selecting patients for immunotherapy in practice or as an enrichment strategy in clinical trials. There are now emerging data on the clinical utility of biomarkers such as PD-L1 expression (Combined Positive Score), gene signatures and tumor mutational burden. Book excerpt: Head and neck cancer (HNC) is a highly aggressive disease that arises from the mucosal surfaces of the aerodigestive tract and impacts millions of people worldwide, with over 900,000 new cases diagnosed globally each year. Unfortunately, the prognosis for late-stage disease remains poor despite the development of new treatments like immunotherapies.In order to escape the immune system and promote tumor growth, HNC cells alter their own proteome to manipulate the tumor immune microenvironment (TIME). Typically, the TIME in HNC is considered immunosuppressive and is infiltrated by anti-inflammatory cell types such as regulatory T cells, myeloid-derived suppressor cells, and M2-polarized macrophages. The TAM family of receptor tyrosine kinases (RTKs) is known to be heavily involved in regulation of both macrophage polarization status and the innate immune system in the non-cancer setting. The TAM family of RTKs (Tyro3, Axl, MerTK), beyond their role in innate immunity, have been shown to be involved in cancer progression. Previous studies in our laboratory have demonstrated that Axl is a critical driver of proliferation, survival, metastasis, and therapeutic resistance in HNC, and that MerTK is, at least in part, functionally redundant to Axl. In the non-cancer setting, the TAMs regulate portions of the innate immune response, and, in the context of HNC, the TAMs drive cancer progression. Presented within is the discovery of TAM regulation of the immune compartment in the HNC TIME. Genetic targeting of Axl resulted in a more pro-inflammatory TIME via increased recruitment and activation of natural killer cells. Simultaneous pharmacologic targeting of Axl and MerTK increased the levels of pro-inflammatory cells in the TIME, slowed tumor growth, and increased the efficacy of immune checkpoint inhibitors (ICIs). Thus, targeting the TAMs in HNC, alone or in combination with existing ICIs, may improve ICI response rates and result in more positive clinical outcomes for HNC patients.

Inflammation and cancer have been recognized to be linked in another context for many years, i.e., with regards to pathologies resembling chronic lacerations or 'wounds that do not heal.' More recently, the immunology of wound healing has given us clues as to the mechanistic link between inflammation and cancer, in as much as wounds and chronic inflammation turn off local cell-mediated immune responses and switch on growth factor release as well the growth of new blood vessels - angiogenesis. Both of these are features of most types of tumours, which suggest that tumours may require an immunologically shielded milieu and a growth factor-rich environment.
