Book excerpt: Objectif : L'objectif de cette étude est d'identifier, grâce au CBCT, les lésions osseuses et dentaires associées aux canines incluses maxillaires, et de déterminer en quoi celui-ci doit devenir l'examen de référence dans la gestion des canines incluses. Matériels et Méthodes : 42 patients ont été inclus dans cette étude rétrospective, parmi lesquels 33 femmes et 9 hommes, avec une moyenne d'âge de 16,5 ans. À l 'aide de radiographies panoramiques et des CBCT recueillis auprès des patients inclus dans l'étude, ont été étudiées : la localisation (vestibulaire, palatine, mésiale et distale), la hauteur d'inclusion, l'orientation des canines incluses ; les volumes de résorption des dents adjacentes ont été calculés, à l'aide d'un logiciel d'exploitation des images 3D Osirix Lite, et l'ensemble des lésions osseuses et dentaires visibles sur les CBCT ont été répertoriées. Résultats : Nous avons donc recensé 58 canines incluses maxillaires chez 42 patients. L'inclusion des canines maxillaires touche plus souvent les femmes (78,57%). Les canines sont le plus souvent incluses en palatin (60,34% des cas). Dans 58% des cas, il y a une résorption radiculaire des dents adjacentes (69% pour les incisives latérales, 13% pour les incisives centrales et 18% pour les premières prémolaires). D'autres lésions dentaires et osseuses ont été observées (hypertrophie du sac folliculaire, dilacération radiculaire, odontome, transposition, sinusite). Conclusion : De nombreuses découvertes fortuites sont détectées lors de cet examen, notamment les résorptions radiculaires qui représentent les lésions les plus fréquemment rencontrées. Le CBCT, de par l'ensemble des images dans les trois dimensions de l'espace qu'il apporte, constitue un outil pertinent dans l'étude des canines incluses maxillaires, pour un diagnostic précis et une prise en charge orthodontique, chirurgicale, parodontale, endodontique, la mieux adaptée.

Book excerpt: L'éruption dentaire est un processus physiologique complexe permettant la migration de la dent depuis son site de développement dans l'os alvéolaire jusqu'à sa position fonctionnelle au sein de l'arcade dentaire. Le canal gubernaculaire (CG) représente la voie par laquelle la dent traverse l'os. Il possède un rôle important dans le mécanisme d'éruption de la dent. Il est admis que les canines permanentes maxillaires, après les dents de sagesse, sont les plus touchées par l'inclusion dentaire. L'étude des caractéristiques du CG de canines incluses maxillaires à l'aide d'examens CBCT a été réalisée afin d'identifier les paramètres susceptibles d'engendrer une inclusion de ces dents et également pour clarifier son rôle dans le processus d'éruption dentaire. 81 canines incluses maxillaires issues de 64 examens CBCT ont été analysées. Lorsque le CG était détectable et interprétable, quatre paramètres ont été analysés à savoir : la position de la canine incluse dans le maxillaire, le trajet du CG, le site d'attachement du CG au follicule dentaire et l'ouverture du CG au niveau de la crête alvéolaire. Pour 12 dents (14,8%), la détection et l'analyse du CG n'ont pas pu être réalisées. Dans notre échantillon, plus de la moitié des canines incluses étaient en position palatine (54,5%). 87% des canines incluses maxillaires présentaient au minimum deux paramètres anormaux. 42% d'entre elles présentaient un trajet sinueux du CG et 70% des canines incluses maxillaires possédaient un site d'attachement anormal du CG au follicule dentaire. Pour 81% des canines incluses analysées, l'ouverture du CG au niveau de la crête alvéolaire était normale. Ainsi, il semblerait probable qu'une accumulation de paramètres anormaux et une altération du CG entraineraient une éruption anormale de la dent. D'autres études sur les caractéristiques du CG sont néanmoins nécessaires pour approfondir son rôle dans le processus d'éruption dentaire. De plus, une étude plus approfondie de ces différents paramètres permettrait de contribuer à la prise de décisions thérapeutiques des canines incluses maxillaires, ces dernières ayant un rôle majeur à la fois esthétique mais aussi fonctionnelle au sein des arcades dentaires.

Book excerpt: Trabecular bone structure as well as bone mineral density (BMD) have impact on the biomechanical competence of bone. In osteoporosis-related fractures, there have been shown to exist disconnections in the trabecular network as well as low bone mineral density. Imaging of bone parameters is therefore of importance in detecting osteoporosis. One available imaging device is cone-beam computed tomography (CBCT). This device is often used in pre-operative imaging of dental implants, for which the trabecular network also has great importance. Fourteen or 15 trabecular bone specimens from the radius were imaged for conducting this in vitro project. The imaging data from one dual-energy X-ray absorptiometry (DXA), two multi-slice computed tomography (MSCT), one high-resolution peripheral quantitative computed tomography (HR-pQCT) and four CBCT devices were segmented using an in-house developed code based on homogeneity thresholding. Seven trabecular microarchitecture parameters, as well as two trabecular bone stiffness parameters, were computed from the segmented data. Measurements from micro-computed tomography (micro-CT) data of the same bone specimens were regarded as gold standard. Correlations between MSCT and micro-CT data showed great variations, depending on device, imaging parameters and between the bone parameters. Only the bone-volume fraction (BV/TV) parameter was stable with strong correlations. Regarding both HR-pQCT and CBCT, the correlations to micro-CT were strong for bone structure parameters as well as bone stiffness parameters. The CBCT device 3D Accuitomo showed the strongest correlations, but overestimated BV/TV more than three times compared to micro-CT. The imaging protocol most often used in clinical imaging practice at our clinic demonstrated strong correlations as well as low radiation dose. CBCT data of trabecular bone can be used for analysing trabecular bone properties, like bone microstructure and bone biomechanics, showing strong correlations to the reference method of micro-CT. The results depend on choice of CBCT device as well as segmentation method used. The in-house developed code based on homogeneity thresholding is appropriate for CBCT data. The overestimations of BV/TV must be considered when estimating bone properties in future clinical dental implant and osteoporosis research.

Book excerpt: OBJECTIF : L'objectif de cette étude a été de valider le potentiel des méthodes de reconstruction itérative nouvellement développées en imageries CT à faisceau conique, afin de réduire la dose d'exposition dans le cadre des bilans de localisation des canines incluses. MÉTHODOLOGIE : Quarante examens par imagerie volumétrique à faisceau conique de canines incluses ont été reconstruits à pleine dose (D), demi-dose (D2) et quart de dose (D4). Ces examens ont été analysés par un radiologiste maxillo-facial et par un résident en orthodontie. La cohérence entre les évaluations des critères radiologiques retenus a été évaluée avec les tests de Kappa Cohen. RÉSULTATS : Les résultats de cette étude ont montré de fortes valeurs de Kappa concernant l'évaluation inter-examinateur de la position de la canine impactée avec des scores compris entre 0.606 - 0.839. Les valeurs de Kappa déterminées pour la résorption, l'ankylose et les lésions associées étaient beaucoup plus faibles avec des scores compris entre 0.000 et 0.529. CONCLUSION : Cette étude a permis de montrer que la localisation des canines incluses pourrait potentiellement être possible à faible dose (1/4 dose), comparativement à un dosage conventionnel. Toutefois, le diagnostic de la résorption, de l'ankylose ou encore de certaines lésions associées nécessitent de la haute résolution et donc des acquisitions à pleine dose.

Mais la canine incluse peut être à l'origine d'autres types de lésions, et altérer d'autres catégories de dents comme nous l'avons constaté .en étudiant lès clichés tomodensitométriques les 35 patients, adressés au service de radiologie de l'Hôtel Dieu à Paris, dans le but d'étudier les rapports existant entre leurs dents incluses et les structures avoisinantes . Il en ressort que des déplacements dentaires et des coudures radiculaires des dents voisines sont présents dans la majorité des cas moins graves que les résorptions, ils compliquèrent cependant les futures tractions orthodontiques. De plus nous avons voulu souligner le danger encouru par les premières prémolaires maxillaires ; toujours coudées et/ou déplacées, elles sont aussi parfois résorbées, et ceci n'avait été illustré, à notre connaissance, que par un seul auteur . Nous présentons ainsi, un cas clinique dans lequel les racines des premières prémolaires maxillaires ont été résorbées suite à l'inclusion bilatérales des canines.

The introduction of cone beam computed tomography (CBCT) represents an important technological advance in the context of oral and maxillofacial radiology as it permits the acquisition of high-quality 3D images and dynamic navigation over an area of interest in real time, with a short scan time and lower dose of radiation than conventional computed tomography (CT). The initial indications for CBCT have been extended by the progressive addition of new ones such as evaluation of the extent of osteonecrotic lesions of the jaw due to bisphosphonates, preoperative staging of oral cancer, and planning reconstructive surgery. As a consequence, this radiological technique represents an interesting complement to conventional radiology in those clinical situations in which 3D imaging can facilitate diagnosis and/or treatment.

Book excerpt: Abstract: Objectives: To assess the ability of using cone-beam computed tomography (CBCT) to diagnose small osseous defects at the mandibular condyle, this study investigated: 1) the impact of defect size and CBCT scan resolutions on the accuracy of qualitative and quantitative diagnoses of condylar defects and, 2) the ability to determine a suspected defect to be true/false based on CBCT measurements. Methods: Cylindrical holes simulating condylar defects of varied levels of diameter (=2mm; 2-3mm; >3mm) and depth (=2mm; >2mm) were created at 22 fresh pig mandibular condyles, with defect number/size per condyle/quadrant randomly determined. With soft tissues repositioned, CBCT scans (0.4mm, 0.2mm voxel-size) of the pig heads were obtained. Reconstructed CBCT data were analyzed independently by two calibrated, blinded raters using Dolphin-3D to diagnose osseous defects qualitatively (defect identification/localization) and quantitatively (geometric measurements of diameter/depth), which were compared to physical diagnoses obtained from polyvinyl siloxane impressions. Statistical tests were conducted for the research objectives.
