Based on research studies the significance of breastfeeding exclusively for six months has proven to be a preventable measure against childhood obesity. This paper will educate the reader on the problems related to childhood obesity, the plan to prevent or decrease the rate of childhood obesity, and the author's solutions to the problem. The author will incorporate a nursing theory that supports the solution to the problem, an implementation plan that incorporates proper education to healthcare staff and patients, an evaluation plan to evaluate the effectiveness of the author's proposed change, and the strategy to disseminate the information to key stakeholders and the nursing community. Obesity is a serious public health concern in the United States, the Centers for Disease Control and Prevention (2012) stated that "one-third of adults are obese and prevalence continues to rise and 17% of children and adolescence are obese; this number has tripled since 1980" (CDC, 2012, page 1). Chronic illness and other health-related complications related to obesity include diabetes mellitus, heart disease, liver disease, stroke, hypertension, gallbladder disease, sleep apnea, osteoarthritis, low self-esteem, depression, and cancer (CDC, 2012). "In 2008 147 billion dollars of medical costs were related to obesity, cost were 1,429 dollars higher for person than medical cost for a normal weight person" (Menell, 2012). The significance of obesity to nursing is that it makes care more challenging for the nurses. Nurses are caring for a larger and heavier population and it can cause harm to the nurses and nursing assistance that are caring for these patients. The nurses are not only concerned about their health and risk for injury, but also the increased challenges with patient care. These challenges nurses are facing when it comes to obesity are skin concerns, respiratory challenges, immobility and ambulation, drug absorption, and intravenous access. The health care of the nurse and the patient are significant to the care being received in the hospital and outpatient clinics. More patients are becoming home bound due to obesity and complications associated with obesity that making regular schedules follow-up appointments, and physical exams in almost impossible. Home care nursing will be on the rise to help patients stay safe and well in the home, because they are unable to make it out of the house. The use of emergency medical services to assist patients in emergency situations is on the rise (Menell, 2012). Nurses need to advocate for the patients and request referrals for appropriate treatment and management of obesity, and develop care plans for patients for a healthy lifestyle, and encouraging family support in the process. The solution to manage and decrease obesity in the United States includes coordination of care by a multi-disciplinary team, prevention method, education, and awareness. Fortunately obesity is a preventable disease. The community needs to be educated and become aware of the resources available to everyone when it comes to diet and lifestyle changes. Resources available in the community include weight-loss management programs, and disease management programs. Obesity and risk for obesity should be addressed by the primary care physician in a tactful manner, more needs to be done than suggesting an exercise program and behavioral modification. The ultimate goal for obesity is self-care, attainment, and maintenance of an ideal weight, adequate nutrition, and positive body image.

Book excerpt: The number of children in the United States who are becoming obese has tripled since 1980. Children as young as two years old are at risk for obesity which is a Body Mass Index (BMI) of greater than 85%. Researchers have studied the relationship between infant feeding and the risk for obesity as a child. The American Academy of Pediatrics (AAP), The World Health Organization (WHO), and the National Association of Pediatric Nurse Practitioners (NAPNAP) recommend breastfeeding as the preferred method of infant feeding until at least 6 months of age and ideally for the first year of life. Secondary analysis of large data sets indicated that breastfeeding had a slight effect on the risk for obesity but was not statistically significant for protection against the risk for obesity as a five year old child. The purpose of this project was to explore the relationship between maternal infant feeding practices and the risk for obesity for children at five years of age. Approval was obtained from the University Institutional Review Board (IRB) and the pediatrician whose practice was used for the data collected in this retrospective designed study. The variables of interest included the type of feeding (breast milk or formula) the infant was given, the age of the infant when solid feeding were initiated, and the height and weight of the child at each health visit. Over 900 charts were reviewed for inclusion and 109 charts met the criteria for inclusion in the study. There were 32 breastfed infants and 61 formula fed infants, the 16 remaining infants were simultaneously given breast milk and formula. The BMI was determined at the five year visit from the height and weight documented. The odds ratio for breastfed infants and the risk for obesity was .848 with a confidence interval of (0.349, 2.057) and a p-value of .358 which indicates that there is not sufficient evidence to say that breastfeeding lowers the risk for obesity as a five year old child.
