Book excerpt: In 1991 the Health Care Financing Agency (HCFA) changed the reimbursement system for hospital capital costs from a costs-paid basis to one with a prospective basis, as part of the ongoing battle to control health care costs. This followed the 1984 change for patient costs from a Fee-For-Service basis to one based on DRGs, a prospectively determined amount for specific diagnosis categories. The capital cost reimbursement allowances in 1996, for example, had a $462 base amount, which was pro-rated with an adjustment for patient-specific DRGs. Previous empirical research has shown that hospitals increased net fixed capital when the proportion of Medicare discharges increased, and as the market became less concentrated. In this paper we develop a model of hospital behavior in which non-profit hospitals maximize some combination of quantity, quality and profits; for-profits maximize the more traditional economic concept of profit. Hospitals differ by the number of Medicare and Medicaid patients, concentration of the market in which they are located, and other hospital-specific factors. Using annual Medicare Cost Report data, provided by the Center for Medicare and Medicaid (CMS), we estimate the annual change in real fixed assets, or hospital net investment, in a fixed effects model for Pennsylvania hospitals. These estimates are then used as inputs to measure both the effect of the percentage of Medicare discharges and market concentration on hospital net investment in the 1980s and 1990s. Our results show that hospital capital formation responded in a directly (i.e., positively) to more competitive markets, even after capital reimbursement was restricted/tightened intentionally, via the 1991 payments system change.

Available in PDF, EPUB and Kindle. Book excerpt: This new volume illuminates the growing corporate in-roads into the health care system and its probable consequences, especially for physicians and other practitioners. Its fourteen contributors examine both the delivery and supply functions in the health sector in America. Ambulatory care, hospitals, health maintenance organizations, and health promotion activities are each critically dissected. A major thrust of the investigations focuses upon implications for the medical profession, principally how the increased scrutiny over clinical decision making by corporate purchasers and payors threatens the traditional role and relative autonomy of physicians. Varying theoretical perspectives are debated, with an additional Canadian perspective offered.

Available in PDF, EPUB and Kindle. Book excerpt: Hospital City, Health Care Nation recasts the story of the U.S. health care system by emphasizing its economic, social, and medical importance in American communities. Focusing on urban hospitals and academic medical centers, the book argues that the country's high level of health care spending has allowed such institutions to become vital, if often problematic, economic anchors for communities. Yet that spending has also constrained possibilities for comprehensive health care reform over many decades, even after the passage of the Affordable Care Act in 2010. At the same time, the role of hospitals in urban renewal, in community health provision, and as employers of low-wage workers has contributed directly to racial health disparities. Guian A. McKee explores these issues through a detailed historical case study of Baltimore's Johns Hopkins Hospital while also tracing their connections across governmental scales--local, state, and federal. He shows that health care spending and its consequences, rather than insurance coverage alone, are core issues in the decades-long struggle over the American health care system. In particular, Hospital City, Health Care Nation points to the increased role of financial capital after the 1960s in shaping not only hospital growth but also the underlying character of these vital institutions. The book shows how hospitals' quest for capital has interacted with structural racism and inequality to shape and constrain the U.S. health care system. Building on this reassessment of the hospital system, its politics, and its financing, Hospital City, Health Care Nation offers ideas for the next steps in health care reform.

Book excerpt: Paying the hospital analyzes in depth and compares the methods of paying hospitals across the principal developed countries - France, Germany, England, Holland, Switzerland, Canada, and the United States. It goes on to analyze the comparative effects of different payment methods - either within individual countries or across many countries - on administration, cost cointainment, and clinical performance. The volume is organized by topic, not country-by-country description, and offers information in the following sequence: methods of cost accounting, how budgets are screened and how rates are described, units of payment, how capital is distributed for buildings and equipment, deciding wages of hopital doctors, the organization of medical staffs, the work of financial managers, effects of different methods on levels of costs, volume, types of services, quality of care, and so on.
