OVERVIEW Unfortunately, there is a three to 17 percent incidence of pressure ulcers in elderly patients. Those patients can experience extreme pain, social isolation, and complications, such as infection of the bone or blood, along with a higher mortality rate. Overall, in the United States the cost for the treatment of pressure ulcers ranges from $9.1 to $11.6 billion per year. The best plan for patients in the home setting is to provide care that helps to prevent the development of pressure ulcers. However, some patients are admitted with a pressure ulcer and others, in spite of good care, can develop one. That's when every member of the homecare team, including home health aides, can play a role in treatment. Most existing training on pressure ulcer generally targets practitioners. This lesson focuses on instructing home health aides, new or experienced, on care of the patient with a pressure ulcer.

Overview OVERVIEW Approximately 25% of all post-menopausal women in the United States have or will develop vertebral compression fractures. The incidence continually increases with age, and the prevalence is 40% in women 80 years of age. While only two-thirds of these fractures cause acute symptoms, they are all associated with increased morbidity and mortality. Women who have sustained a compression fracture have a 15% higher mortality rate that those who have not. Vertebral compression fractures often cause chronic pain and physical limitations. In almost all instances, they have a negative effect on function and quality of life. As is the case with many other chronic illnesses, compression fractures can cause psychosocial problems associated with loss of function and/or mobility. Treatment for compression fractures often involves bracing, adherence to a strengthening program, and activity limitations. Many patients with compression fractures receive homecare for various lengths of time and most will need assistance from home health aides for personal care and to assist with the treatment regimen. The purpose of this in-service is to provide information about the care of these patients.

Book excerpt: This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.
