LESSON OVERVIEW According to multiple studies, nearly a third of all people who are over the age of 65, and who live at home, fall. Among those who do fall, half will fall again. A previous fall seems to be a big predictor of the likelihood of another fall. There are many factors that predispose people to falls. These include the use of assistive devices, hazards within the home, chronic illnesses, problems with gait and balance, urinary incontinence, and visual impairments. Most homecare patients receiving home health aide services are in categories which show them to be at high risk for falling. Emergent care for injury caused by fall or accident in the home is one of the adverse event outcomes reported by the Centers for Medicare and Medicaid Services (CMS) to be evaluated by homecare agencies and to be benchmarked against agencies. It is incumbent on all homecare agencies to take measures to reduce the risk of falling. The topic of falls is important for home health aides for many reasons, including the fact that they typically spend more time in the home than other disciplines. Patients are more likely to demonstrate their normal behavior to the aides than to the professionals. There are measures that can decrease the risk of falls in the home. Home health aides can take steps to make living areas safer for patients in addition to reporting problem areas to the nurses and therapists.

OVERVIEW The incidence of workplace injuries is very high among home health workers in general and home health aides in particular. The Bureau of Labor Statistics reports that one in ten home health aides has a work-related illness or injury every year. The resulting absences from work due to injury usually last from one to two weeks. There are shortages of home health aides throughout the United States and time lost from work by aides is troublesome and expensive for homecare agencies. In addition to the loss of worker productivity, the agency is also responsible for paying for medical expenses through its Workers' Compensation Plan. Overall, the injury rate for homecare aides exceeds the injury rate for similar workers in hospitals by 50 percent, and is 70 percent higher than the national average for private industry workers. It is not surprising that there are more frequent work-related injuries in home health for two primary reasons - there is far less control over the environment, and homecare workers travel from place to place either by car or public transportation throughout the work day. The home health aide may encounter a workplace environment that includes hostile animals, rotting steps, uneven floors, and clutter and then may have to brave inclement weather while juggling supplies and papers.The incidence of highway accidents is much higher among homecare workers than other types of health care workers. (Injuries that occur when commuting prior to the start of the workday or after it ends are not counted as work-related injuries.) The injury most common to home health aides is over-exertion, notably while lifting and transferring patients. Other common injuries include body actions such as over-reaching, twisting, or losing balance without falling. Many injuries occurring in the workplace can be prevented. The purpose of this in-service program is to provide information about injuries common to home health aides and to give measures that the aides can undertake to prevent or reduce them.

Care pathways for managing patients with these diagnoses were among the first to be widely used. All too often, care planning for patients with dyspnea pays far too little attention to the home health aide. Many of these patients require personal care assistance from home health aides because of their compromised breathing. Beyond that, the involvement of the home health aide may be limited to maintaining oxygen equipment, taking vital signs, and reporting any abnormalities to the nurse. The very high prevalence of patients with dyspnea coupled with an increased rate of re-hospitalization for these patients mandates that stabilization or improvement in dyspnea should always be a part of a homecare agency's performance improvement activities. Recent establishment of improvement in dyspnea as a potential marker of quality in the OBQI (outcome-based quality improvement) reporting system provides even greater necessity to look at this issue. Home health aides can be very valuable in achieving better outcomes for patients with dyspnea. The purpose of this in-service is to provide information to assist in that goal.

Statistics from the National Institute of Mental Health (NIMH) indicate that almost 20 million American adults suffer from clinical depression. That number is about 9.5% of the adult population. Elderly patients with chronic disease, especially those who lack social support, are particularly at high risk of developing clinical depression. It is more common in females, and in those of a lower socioeconomic status who have recently had stressful life experiences. Given that this profile is common among homecare patients receiving home health aide services, it is not surprising that the incidence of depression among these patients is very high. It is important for the aide to know how to recognize the behavior of depressed patients as well as some measures to take in caring for them. The purpose of this in-service is to provide information and guidance to home health aides who care for patients with clinical depression.

The prevalence among older adults living in the community is estimated at 20% to 38% for women and 10% to 19% for men. Over half of the institutionalized older adults have urinary incontinence. Given the census of the average homecare agency, it is not surprising that many of the patients have some degree of urinary incontinence, particularly those who receive home health aide services. In addition to the psychosocial issues surrounding urinary incontinence, the condition is a contributing factor in several adverse events reported for home health agencies. There is an increased likelihood of falls which may lead to an increase in emergent care for falls. Incontinence predisposes a person to development of skin breakdown and urinary tract infections. In addition, one of the most commonly listed reasons for nursing home placement is urinary incontinence. Therefore, unresolved urinary incontinence could result in an unexpected nursing home admission as well. There are two Outcome-Based Quality Improvement reports (Improvement in Urinary Incontinence, Emergent Care) that could be negatively affected by the presence of urinary incontinence. Home health aides play a crucial role in helping patients improve in urinary incontinence. They also may be important in identification of patients with this condition. Since many patients are very reluctant to discuss incontinence with physicians and nurses, it may be the home health aide, assisting the patient with hygiene, who first identifies the problem. The purpose of this in-service program is to provide information that will enhance the ability of the home health aide to help patients with urinary incontinence.

OVERVIEW While there is some variation, there are commonly six activities considered to be the activities of daily living (ADLs). They are feeding, bathing, dressing, toileting, transferring, and mobility. Studies indicate that the prevalence of personal assistance needs increases with age and by age 65, 4.1% of the total population in the United States require help in one or more ADL. The prevalence is much higher among homecare patients. When home health aide visits are ordered in homecare, it is almost always to assist with ADLs. Historically, except for patients receiving therapy services, most home health aides were assigned to assist with personal care with little emphasis placed on helping patients achieve greater independence in performing ADLs. Home health aides themselves often viewed their roles as providing direct care, not in promoting greater independence. The benchmarking capabilities of the Outcome and Assessment Information Set (OASIS) reports provide agencies with comparative data on patient improvement in performance of ADLs. The data have helped agencies develop quality improvement activities aimed at increasing independence levels of patients. Home health aides are crucial to success in this goal.
