Book excerpt: By late 1997, a number of studies had shown that early administration of the drug zidovudine (AZT) may have some potential to prevent HIV infection following an exposure to the virus. This intervention, known as post-exposure prophylaxis (PEP), has since become widely available as a potential means of averting HIV infection among workers who have been exposed to the virus in hospitals and other health care settings, as well as in emergency, law-enforcement, custodial, and paramedical services. Consideration of non-occupational PEP, such as following instances of sexual exposure or injection drug use, has raised a number of concerns and difficulties. This report summarizes proceedings of a conference held to provide information on what is known about PEP, to discuss the issues involved in non-occupational PEP and their complexity, and to make recommendations for determining future action and decision making. The first part of the conference consisted of a plenary session addressing two general themes: types of exposure and risk assessment (including clinical studies, counselling issues, communication and education issues, access to treatment), and legal and ethical issues (including economics) relating to PEP. This part was followed by interdisciplinary groups that discussed unresolved issues and barriers to PEP, and proposed solutions and recommendations. The final session was devoted to action planning in stakeholder-specific groups: government and policy makers, health care providers and researchers, and the community and consumer sector.

Book excerpt: The purpose of this study was to analyze how international HIV/AIDS treatment providers in Australia, Kenya, and Lebanon navigate systematic social and political barriers to ethically meet the needs of their clients living with HIV through grounded theory methodology. Data collection methods include the use of interviews, filed notes, observations, historical media publications, and scholarly literature on barriers to ethical decision-making for treatment providers. Data collection took place with key informants, agency care workers, and community respondents from July 15, 2015 to August 4, 2015 in Melbourne, Australia, Beirut, Lebanon, and Nairobi, Kenya. After data collection, three clear opportunities for meaningful analysis emerged to inform international HIV social work practice, including: 1) a case study from Lebanon evaluating the utility of the four quadrants model to ethical decision-making; 2) an evaluation of the core challenges to consensual HIV disclosure and ethical-decision making for treatment providers and their clients; and 3) an examination of the new challenges from diminishing funding to ethical decision-making by HIV treatment providers. These three research papers are presented in a potentially publishable format and bookended between introduction and conclusion chapters. Implications for international social work practice and policy recommendations are discussed at length.
