Book excerpt: The authors report results from a randomized evaluation comparing three school-based HIV/AIDS interventions in Kenya: (1) training teachers in the Kenyan Government's HIV/AIDS-education curriculum; (2) encouraging students to debate the role of condoms and to write essays on how to protect themselves against HIV/AIDS; and (3) reducing the cost of education. Their primary measure of the effectiveness of these interventions is teenage childbearing, which is associated with unprotected sex. The authors also collected measures of knowledge, attitudes, and behavior regarding HIV/AIDS. After two years, girls in schools where teachers had been trained were more likely to be married in the event of a pregnancy. The program had little other impact on students' knowledge, attitudes, and behavior, or on the incidence of teen childbearing. The condom debates and essays increased practical knowledge and self-reported use of condoms without increasing self-reported sexual activity. Reducing the cost of education by paying for school uniforms reduced dropout rates, teen marriage, and childbearing.

Far from describing the Tswana as a closed or timeless ‘society’, he locates the people in their political and economic context, and in so doing, has left behind an extraordinary record. The largely medical solutions funded by major donors have had little impact in Africa, the continent hardest hit by AIDS. Instead, relatively simple, low-cost behavioral change programs—stressing increased monogamy and delayed sexual activity for young people—have made the greatest headway in fighting or preventing the disease's spread. Ugandans pioneered these simple, sustainable interventions and achieved significant results. As National Review journalist Rod Dreher put it, Rather than pay for clinics, gadgets and medical procedures—especially in the important earlier years of its response to the epidemic—Uganda mobilized human resources. In a New York Times interview, Green cited evidence that partner reduction, promoted as mutual faithfulness, is the single most effective way of reducing the spread of AIDS. That deceptively simple solution is not merely about medical advances or condom use. It is about the ABC model: Abstain, Be faithful, and use Condoms if A and B are impossible. Yet deeply rooted Western biases have obstructed the effectiveness of AIDS prevention. Many Western scientists have attacked the ABC approach as impossible and moralistic. Some Western activists and HIV carriers have been outraged, thinking the approach passes moral judgment on their behaviors. Book excerpt: Few people realize that the familiar HIV/AIDS global statistics are actually estimates. For example, UNAIDS estimated that the Republic of South Africa had 140,000 HIV/AIDS deaths in 1997. However, after tabulating all deaths for 1997, the Republic of South Africa attributed only 6,635 deaths to HIV/AIDS. Such discrepancies are rarely noted. The Republic of South Africa (RSA) stands as the exemplar of these discrepancies, and is reputed to have the world's largest AIDS epidemic with an estimated 5.6 million people living with HIV/AIDS (PLWH) in 2008. Such PLWH estimates, as with the estimates of HIV/AIDS deaths, are highly questionable. The reasons behind these discrepancies are clarified by describing two common misunderstandings of HIV infection that contribute to poor mathematical modeling outcomes. Unfortunately, the health authorities in the Republic of South Africa grant more validity to computer-generated estimates than to their own empirical death counts. The author discusses why these modeled estimates, and the HIV sero-prevalence surveys upon which they are based, are simply implausible. Presented with full references are raw numerical data on: the tabulated number of HIV/AIDS deaths in the RSA; the number of AIDS cases detected by RSA disease surveillance systems; UNAIDS/WHO estimates for AIDS deaths in the RSA; and UNAIDS/WHO estimates for the number of people living with HIV/AIDS in the RSA. The total cumulative HIV/AIDS cases in the United States and Africa are also presented for comparison, and to place the African and RSA data within appropriate epidemiological context. Overall, these data span from 1981 to 2009. Altogether, these data, plus additional information detailing the nature of HIV infection and heterosexual HIV transmission rates, explain why the hyperbolic mathematical estimates and HIV antibody test surveys - the primary sources of HIV/AIDS data in Africa - are simply implausible.
