This was nowhere more evident than in South Africa. This book explores how the state responded to its responsibilities to defend and protect (human) security. Linking this to the role of the state as sovereign protector and provider of security, it applies the findings to the broader re-interpretation of sovereign responsibility in the 21st Century. This book does not seek to absolve the South African state of its responsibility to respond. Moreover, it argues that although the state, the government, before, during, and after the transition to democracy, was aware of and acknowledged the threat - political, economic and social - posed by the epidemic, it nonetheless chose not to make the epidemic a priority policy issue. As a result, it argues that the South African HIV/AIDS case illustrates the tension inherent between a state’s ultimate sovereign responsibility to respond and its tactical dependence on external contributors to meet the demands of all of its constituents.

The uncertainty HIV/AIDS creates with regard to its origin triggers conceivable concern and simultaneously an adamant wish to finally being able to attribute its origin and aggressive spread to a certain source. Hence, in order to give a name to what has been happening to mankind for the last decades, people clasp at straws to make the phenomenon understandable in accordance to their personal world-view. Since the unrelenting spread of the virus is to be reported from all over the world, every society and culture, depending on context, tries to make sense of the issue for itself, which means, finding reasons for HIV/AIDS that seem most probable against the background of the respective nation and its personal history. Particular explanations, when having been maintained and repeatedly used over a longer period of time, may then sometimes develop into outright myths that bear witness of a larger social and cul-tural movement of reasoning within the respective nation. Considering their foundation in a nation’s history and its experience with other countries, those myths cannot be regarded as mere expressions of phantasm though but rather as cultural and social constructs that attempt to explain the world people live in. Those myths and their respective functions shall be pursued and analysed in the following essay, setting the primary focus on South Africa with its idiosyncratic set of beliefs on the one hand and the West as its perceived opponent on the other. In some countries with high HIV prevalence rates, life expectancy has declined by more than a decade and in a few cases by more than two decades. Even in countries with HIV prevalence of around 5 percent (close to the average for sub-Saharan Africa), the epidemic can reverse gains in life expectancy and other health outcomes achieved over one or two decades. This volume highlights work conducted under the umbrella of a World Bank work program on The Fiscal Dimension of HIV/AIDS, including country studies on Botswana, South Africa, Swaziland, and Uganda. It covers four aspects of the fiscal dimensions of HIV/AIDS: First, it aims for a comprehensive analysis of the fiscal costs of HIV/AIDS, with a wider scope than a costing analysis focusing on only the policy response to HIV/AIDS. Second, it embeds the analysis of HIV/AIDS costs in a discussion of the fiscal context, and interprets these costs as a quasi-liability, not a debt de jure, but a political and fiscal commitment that binds fiscal resources in the future and cannot easily be changed, and very similar to a pension obligation or certain social grants or services. Third, it develops tools to assess the (fiscal dimensions of) trade-offs between HIV/AIDS policies and measures that take into account the persistence of these spending commitments. Fourth, most of the fiscal costs of HIV/AIDS are ultimately caused by new infections, and this study estimates the fiscal resources committed (or saved) by an additional (or prevented) HIV infection. Building on these estimates, the analysis here is able to assess the evolving fiscal burden of HIV/AIDS over time.
