Nurses may be trained in two-, three-, or four-year programs, but all are eligible to take the accreditation examination to be licensed as registered nurses (RNs). The question of distinguishing between "professional" nurses from bachelor programs and "technical" nurses from the associate degree programs has become an important and controversial issue in nursing. Advocates have long contended that the associate degree nurse is vital to the American health care system. This study, funded by the W. K. Kellogg Foundation, confirms this view. In recent years the Foundation has invested more than $6.1 million in the development of the ADN, awarded by junior and community colleges. Many participants in the ADN projects for the Kellogg Foundation have noted that, despite the importance of the ADN and the controversy about its place in nursing education, the literature is scattered and hard to identity. The Origins and Rise of Associate Degree Nursing Education and the companion bibliography will provide much-needed information to educators, hospital and nursing administrators, nursing leaders, and public policy makers--all of whom must cope with the growing nursing shortage and increasingly difficult issues in health policy and administration.

With Ewell's (1997) construct of designed curriculum providing the theoretical framework, the primary effects of The National League for Nursing Accreditation Commission, Inc. (NLNAC) program accreditation on the Sinclair Community College Associate Degree Nursing program were studied. An instrumental case study methodology was employed to collect and analyze data. Four effects of accreditation on the designed curriculum were identified: faculty engagement in curriculum changes, sophisticated conceptual framework underlying the curriculum, increased number of curricular revisions near the accreditation timeframe, and systematization of continuous process improvement tools. The research findings indicate a relationship exists between the maturity of the designed curriculum processes and the directness of the intervention of program accreditation.

Book excerpt: This study describes the historical development of the Associate Degree Nursing Program at Portland Community College by women faculty leaders in a male-dominated organization. In-depth interviews of eleven participants were transcribed and analyzed. The participants gave a very rich description of this nursing programthus describing its culture. Six themes were identified: the use of power, governance, informal networks, work ethic, psychological climate, and external influences. A review of the literature affirmed that these topics are conceptually related. Thus, three constructs were reviewed: the use of power, organizational culture, and associate degree nursing education. What this means in terms of a historical study is for one to look at the whole. The influence of prior events upon contemporary social events is still a dynamic issue. Insight into how or why decisions have been made in the organization's past, can illumine contemporary decision making. Past decision making can be viewed both within the historical context and through the contemporary lenses of the use of power in the workplace, organizational culture, and associate degree education. Being aware of the cultural context, both from the broader social perspective and from the microcosm of the organization or the immediate work group, allows insights through these more refined lenses.
