African Americans between the ages of 18 and 24 are disproportionally affected by unplanned pregnancy and sexually transmitted infections, which are associated with risky sexual behaviors, including sex without a condom. The risks and stress associated with living at the intersection of both African American risk factors and college risk factors may play a role in the sexual behavior of African American college students. The purpose of this study was to examine the determinants of condom use among African American undergraduates at predominantly White Institutions (PWIs). This study used the constructs of the Theory of Planned Behavior (TPB) to examine the factors that contribute to condom use. An added factor the study examined was the relationship between different types of stress and condom use. The relationship between stress, intention to use condoms, and actual condom use was also investigated. The study employed a cross-sectional design and used surveys to collect data on African American college students between the ages of 18 and 24 years old at PWIs. The survey was disseminated through Qualtrics online survey software. The sample of 202 students engaged in a range of sexual behaviors (vaginal, oral, and anal sex) and had inconsistent condom use during these activities. The study found that constructs of the Theory of Planned Behavior, namely intentions and attitudes, were independently significant at predicting condom use. However, the interaction between intentions and overall stress was more significant in predicting condom use among African American college students attending PWIs over the past 30 days. The study findings have promising implications for health education practitioners, university stakeholders, and researchers who are interested in reducing sexual health disparities. Coordinated efforts are needed to reduce the risk factors that contribute to unsafe sexual behaviors among college students, especially among those at greater risk such as African American college students at PWIs.

The author offers factual information to draw conclusions that can be used to develop HIV/AIDS prevention programs specifically tailored for the African-American community. The first chapter provides an introduction, rationale, and overview of the study. Basic information about the prevalence of AIDS among various African-American populations are presented. Then, Johnson describes information about the subjects, measures of sexual behaviors, drug use, attitudes about the use of condoms, knowledge about AIDS, and perceived susceptibility of being exposed to HIV/AIDS. Next, Johnson describes the sexual attitudes and behaviors of African-American males and females who are currently involved with multiple partners and those who have been previously treated for sexually transmitted diseases. He then describes the characteristics of African-Americans with HIV/AIDS. The epilogue summarizes the major findings and presents suggestions for AIDS prevention activities for African-American young adults.

Book excerpt: Adolescent risky sexual behaviors such as early sexual debut, low rates of condom use, and high rates of partner change are associated with a number of negative health and social outcomes including contracting sexual transmitted infections and unintended pregnancy. Utilizing a sample of 529 African American adolescents, this study examines the contributions of parenting behaviors and family structure as factors associated with engagement in risky sexual behavior. Participants reported few differences in mother's parenting across family structures. While fathers and relatives provided greater responsiveness on average than stepfathers. Variation in parenting fully explained differences in risky sexual behavior for males. Yet family structure differences persisted for females, with females in mother step-father homes continually engaging in greater mean risky sexual behavior than those in mother-father households even after accounting for parental responsiveness and demandingness. Implications for prevention and intervention are discussed.

Book excerpt: African American (AA) adolescents are currently at disproportionate risk for HIV infection, with AA youth ages 13 through 19 accounting for 56% of all new HIV cases, a rate of infection 8 times that of Caucasian youth. Prevention researchers have targeted this population with a sense of urgency, resulting in numerous studies that have investigated predictors of sexual risk behaviors among AA youth. The results of these studies have yielded findings in a number of domains including environmental, individual, and interpersonal factors. However, reported findings are often inconsistent and synthesis of findings in this area is lagging. The present study synthesized data from 79 studies examining 11 domains of variables in association with 3 outcome variables: Unprotected Sex, Condom Use, and Multiple Sex partners. Intentions and partner variables were most predictive of sexual risk behaviors in this population. Further research is needed to investigate the role of partner-related influences in predicting sexual risk behaviors.

Book excerpt: Sex on television has nearly doubled since 1998 while sexual risk and responsibly messages have decreased. During this time frame, television watching has become one of the most popular pastimes in the United States, especially among Black/African Americans. Black youth are more likely to report higher levels of satisfaction with television images and view characters as more realistic than the average youth. Here, we examine how viewing higher amounts of television affects certain sexual behaviors of Black female adolescents using data from the 2007 Youth Risk Behavioral Surveillance Survey. Results indicate that the number of television hours viewed on an average school day was significantly related to initiating sexual intercourse before the age of 14, condom use, and contraceptive use to prevent pregnancy for all adolescent females in the sample. Though the number of television hours viewed on an average day was not significantly related to the sexual behaviors of Black adolescent females, this group was more likely to initiate sex before 14, report having 3 or more lifetime sexual partners, and more likely to report using condoms at last intercourse than their female counterparts in any other racial group. Black female adolescents who watched 5 or more hours of television, however, were almost four times more likely to report using any form of contraceptive to prevent pregnancy than their white counterparts. These findings suggest that the amount of television watched on an average school day does not influence the sexual behaviors of Black female adolescents in high school. More research is needed to determine the effect television has on Black female adolescents.

Book excerpt: African American college women are experiencing sex-related negative consequences (e.g., contracting sexually transmitted diseases or human immunodeficiency virus, having an unplanned pregnancy) at disproportionate rates in comparison to Caucasian college women. Furthermore, African American college women are likely engaging in risky sexual behaviors (e.g., unprotected anal, vaginal, oral sex) that may be placing them at a greater risk for experiencing sex-related negative consequences. Research suggests that increased alcohol consumption is predictive of more risky sexual behavior among college women. Additionally, sex-related alcohol expectancies, or beliefs about the effects of alcohol on sexual behavior, are positively associated with increased alcohol consumption and risky sexual behavior and therefore, may attenuate the association alcohol use has with risky sexual behaviors among African American college women. Because of the underrepresentation of African American college women in research examining the aforementioned factors, the purpose of the present study was to examine the link between sex-related alcohol expectancies (i.e., enhancement, sexual risk taking, disinhibition), alcohol consumption, and risky sexual behaviors exclusively among a sample of African American college women at a midsized Southern university. Multiple and hierarchical linear regression analyses yielded the following results: (a) enhancement sex-related alcohol expectancies predicted increased risky sexual behavior, (b) sex-related alcohol expectancies did not predict increased alcohol consumption and (c) did not moderate the relationship between alcohol consumption and risky sexual behaviors. The research on adolescent sexual behavior finds that youth are more likely to engage in sexual activity at an earlier age and have more partners when they are not raised in two-parent households. This is a concern for African-Americans because they are less likely to live in two-parent families and are also more likely to be sexually active. Using the National Longitudinal Survey of Adolescent Health, we find that adolescents in cohabitating and step-parent households are more likely to engage in sexual activity and have more partners, compared to their counterparts in two-parent households. We also find differential effects by gender, with family structure and context having a stronger effect on young females. This is an important result because it shows the negative effects of family disruptions in the African-American community.

Book excerpt: The purpose of this study is to explore ethnic identity as a protective factor against sexual activity among a sample of African American adolescents. The study sample consisted of 1084 African American adolescents who were administered a survey comprised of questions about their health, risk behaviors, and their backgrounds. The variables examined were sexual intercourse, ethnic identity, parent-child communication regarding specific topics, mother-adolescent conflict and negative communication. The study also examined family structure, educational aspirations, attitudes about sex, self-esteem, refusal skills, lifetime substance use, perceived peer lifetime sexual behavior, age and gender. Analyses conducted examined the relationship between sexual activity and the independent variables as well as associations between two nominal variables. Relationships between interval/ratio variables were investigated. Multiple logistic regression analyses were used to assess whether the relationship between ethnic identity and lifetime sexual intercourse persisted, controlling for individual, family, and social factors. The findings indicate ethnic identity, and individual, family, and peer factors were related to lifetime sexual intercourse. The effects were stronger for substance use and the influence of peers on sexual behavior. When taking into account the other individual, family, and peer factors, the relationship between ethnic identity and lifetime sexual intercourse was non-significant. Additionally, ethnic identity did not moderate the risk factors explored in this study. Gender differences are clearly delineated in the study with more males having sexual intercourse than females. The results of this study recommend multifaceted interventions that begin as early as elementary school and involve families, schools, communities, and organizations working to promote responsible sexual behavior of adolescents. In addition, the findings of other studies provide support for intervention programs that facilitate the development of ethnic identity, such as youth mentoring programs with a focus on Rites of Passage programs. Interventions should also be developed to include peer groups that are comprised of friends, because of the influence peers have on sexual activity and the engagement in other risk behaviors such as substance use.-- Abstract.
