Hearing loss is associated with quality of life (i.e., social support, mental health, general health, and physical functioning), but it is unclear if using hearing aids is positively correlated with quality of life. The current study tested three groups, those with: normal hearing, hearing loss with hearing aids, and hearing loss without hearing aids. Hearing thresholds were established with pure-tone audiometry. Total sample size was 100 participants aged 60 and older. It was hypothesized those with normal hearing would have significantly better quality of life than those with hearing aids, who would have significantly better quality of life than those with hearing loss but without hearing aids. Data were analyzed with MANCOVAs and ANOVAs. Differences in quality of life among the three groups approached significance despite platykurtic data that brought power sharply down. After Winsorization, there was a significant difference among the groups; the group without aids had significantly poorer quality of life than the group with normal hearing, which did not significantly differ from the group with hearing aids. MANCOVA significance was largely due to the significant general health variable. These results were found when 54% of the group with aids wore them for less than eight hours a day, and the group without aids only had mild hearing loss. Further, disease-specific measures suggested decreased handicap for the group with aids. These results call for future investigations of people with at least moderate hearing loss and people who wear aids more consistently.

For others, the appropriate services are difficult to access, or individuals do not know how or where to access them. Others may not want to deal with the stigma that they and society may associate with needing hearing health care and obtaining that care. Still others do not recognize they need hearing health care, as hearing loss is an invisible health condition that often worsens gradually over time. In the United States, an estimated 30 million individuals (12.7 percent of Americans ages 12 years or older) have hearing loss. Successful hearing health care enables individuals with hearing loss to have the freedom to communicate in their environments in ways that are culturally appropriate and that preserve their dignity and function. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. In other cases, hearing loss is much more severe, and people may retreat into a hard-to-reach shell. Yet fewer than one in seven older Americans with hearing loss use hearing aids, despite rapidly advancing technologies and innovative approaches to hearing health care. In addition, there may not be an adequate number of professionals trained to address the growing need for hearing health care for older adults. Further, Medicare does not cover routine hearing exams, hearing aids, or exams for fitting hearing aids, which can be prohibitively expensive for many older adults. Hearing Loss and Healthy Aging is the summary of a workshop convened by the Forum on Aging, Disability, and Independence in January 2014 on age-related hearing loss. Book excerpt: The current study aimed to identify and treat hearing loss in an underprivileged community and evaluate the effect, if any, amplification had on quality of life. A total of 15 adults, between the ages of 67 and 89 years, completed a cognitive screening, using the Six-Item Screener (SIS) and a hearing handicap screening, using the Hearing Handicap Inventory for the Elderly-Screener (HHIE-S) and underwent a peripheral hearing assessment. Seven of these participants were excluded from the analysis, as they did not meet the criteria for this study (4-frequency PTA (.5, 1, 2, & 4 kHz) ≥ 26 dB HL, in at least one ear). Seven participants were fitted bilaterally. One participant, with a unilateral moderate to severe hearing loss, was fit with a contralateral routing of signals (CROS) hearing aid system. All participants fitted with hearing aids completed the HHIE-S questionnaire again, 4-5 weeks after their hearing aid fitting. Improvement in HHIE-S scores (lower scores) indicating an improved quality of life (QOL) after 4-5 weeks of hearing aid use were observed; however, when analyzed it was not a significant finding. The results also revealed a strong positive correlation between right and left ear 4-frequency PTAs and pre-HHIE-S scores. The correlation revealed that the more severe the hearing loss the greater impact on emotional and social quality of life as indicated by the HHIE-S scores. Results from this pilot study indicate that future studies should include more participants, use of the adaptation manager on the hearing aid(s) at the initial fitting, at least one follow-up appointment after the initial hearing aid fitting and before any follow-up questionnaires, and a longer duration between initial hearing aid fitting and administration of the second HHIE-S.

Despite evidence that hearing aids can improve the quality of life of older adults, hearing aid uptake is shown to be low internationally. Internet hearing aid sales have emerged as an alternative model of hearing aid delivery for those unable to afford or access hearing aids. Aims and Objectives: This thesis aims to explore older hearing aid users' experiences of utilising audiology services in New Zealand, and their perceptions towards internet-based delivery of hearing aids. The key objectives are to explore positive and negative experiences of older adults, as well as barriers faced, through their journey of utilising the hearing aid delivery system, and to explore their perceptions of the benefits and shortcomings of internet- based delivery of hearing aids. Methods: A qualitative approach was adopted to accomplish the aims of this thesis. Semi structured interviews were conducted with participants in a face-to-face manner. In order to be eligible to participate in this study, individuals needed to be aged 65 years and over, be a hearing aid user, and be residing within the Auckland region. Participants were recruited from various sources, including the University of Auckland Hearing and Tinnitus Clinic, the Hearing Association, via a mass email sent to members of the University of Auckland's Faculty of Medical and Health Sciences, and via snowball sampling. The interviews were transcribed and analysed thematically for common themes related to the research aims and objectives. Results: Five main aspects of the hearing aid delivery system were discussed. Physical accessibility to the clinic was generally reported to be easy, and the overall journey through the system was mainly described to have progressed seamlessly. Financial troubles were however, reported in meeting the high cost of the hearing aids. The interpersonal and communication skills of the audiologist were commonly discussed. Trust in the audiologist also emerged as a theme, and was influenced by patients' perception towards the audiologist's focus on making a sale versus providing hearing aids suited to the individual's needs, as well as towards the audiologist's competence in their work. A general lack of awareness regarding internet hearing aid sales was found. Lower cost hearing aids and greater convenience were perceived to be the benefits. Numerous concerns and limitations related to this model were also expressed. Concern regarding whether and how clinical procedures, such as hearing tests and adjusting or fine-tuning of the hearing aids would be conducted in this model were expressed. Participants preferred to be treated by an expert with knowledge of hearing loss, and preferred face-to-face contact. A fear of being ripped off on the internet, and a general lack of familiarity and confidence in using the internet for purchasing were further expressed. Conclusions: The findings of this study have highlighted aspects of the hearing healthcare system in New Zealand, that need greater focus in order to enhance hearing healthcare services for older adults. These include addressing difficulties in meeting the cost of the hearing aids, and providing adequate training and education of audiologists in developing therapeutic relationships and involving patients in their healthcare. Exploration of patients' perceptions towards internet-based hearing aid delivery has revealed benefits as well as several concerns. These concerns would likely need to be addressed and could be used for future research in this area as well as to guide the development of an internet-based model that is likely to appeal to older adults. This could potentially improve access to hearing aids. Overall, the findings of this research are relevant to policy-makers, audiology providers, and researchers, and have implications for improvements in hearing aid delivery services and access to hearing aids in older adults.

Screening could identify untreated hearing loss and lead to interventions to improve hearing-related function and quality of life. PURPOSE: To update the 1996 U.S. Preventive Services Task Force evidence review on screening for hearing loss in primary care settings in adults ages 50 years and older. DATA SOURCES: We searched Ovid MEDLINE from 1950 to July 2010, the Cochrane Database of Systematic Reviews, and the Cochrane Central Register of Controlled Trials through the second quarter of 2010 to identify relevant articles. We supplemented electronic searches with reviews of reference lists of relevant articles and solicited additional citations from experts. STUDY SELECTION: We selected randomized trials and controlled observational studies that directly evaluated effects of screening for hearing loss in older (ages 50 years) adults. To evaluate indirect evidence on screening, we also included studies on the diagnostic accuracy of screening tests for hearing loss used in primary care settings, and randomized trials and controlled observational studies that reported clinical outcomes associated with use of amplification. DATA EXTRACTION: One investigator abstracted data and a second investigator checked data abstraction for accuracy. Two investigators independently assessed study quality using methods developed by the U.S. Preventive Services Task Force. DATA SYNTHESIS: Evidence on benefits and harms of screening and treatments for hearing loss was synthesized qualitatively. One large (n=2305) randomized trial found that screening for hearing loss was associated with increased hearing aid use at 1 year, but screening was not associated with improvement in hearing-related function. There is good-quality evidence from 20 studies on diagnostic accuracy that common screening tests for hearing loss can help identify patients at higher risk for hearing loss. The whispered voice test at 2 feet and a single question regarding perceived hearing loss were comparable with a more detailed screening questionnaire or a hand-held audiometric device for identifying at least mild (>25 dB) hearing loss. Negative results using a hand-held audiometric device may be the most useful finding for ruling out at least moderate (>40 dB) hearing loss. One good-quality randomized trial found that immediate hearing aids were effective compared with wait-list control for improving hearing-related quality of life and function in patients with mild or moderate hearing loss and severe hearing-related handicap. We did not find direct evidence on harms of screening or treatments with hearing aids, but harms are likely to be small based on the non-invasive nature of screening and treatment, with no known serious adverse events. LIMITATIONS: We excluded non-English language studies, included studies of diagnostic accuracy in high-prevalence specialty settings, and did not construct outcomes tables. CONCLUSIONS: Additional research is needed to understand effects of screening compared with no screening on health outcomes, and to confirm benefits of treatment under conditions likely to be encountered in most primary care settings.
