After discussing sector-specific characteristics, lessons of international experience, and the main set of initial conditions, the authors advocate reforms based on organized public financing for basic care, private financing for supplementary care, pluralistic delivery of services, and managed competition. Policymakers need to achieve a balance, both assuring social solidarity through universal access to basic health services and expanding individual choice and responsibility through voluntary supplemental insurance. The authors also consider the problems that undermine effectiveness of market-based competition in the health sector.

The book considers five domains of exclusion (services; economic; social relations; civic and socio-cultural; and community and spatial domains), with three chapters dedicated to analysing different dimensions of each exclusion domain. The book also examines the interrelationships between different forms of exclusion, and how outcomes and processes of different kinds of exclusion can be related to one another. In doing so, major cross-cutting themes, such as rights and identity, inclusive service infrastructures, and displacement of marginalised older adult groups, are considered. Finally, in a series of chapters written by international policy stakeholders and policy researchers, the book analyses key policies relevant to social exclusion and older people, including debates linked to sustainable development, EU policy and social rights, welfare and pensions systems, and planning and development. The book’s approach helps to illuminate the comprehensive multidimensionality of social exclusion, and provides insight into the relative nature of disadvantage in later life. While the goal to improve average levels of population health is important, there has been an increasing focus on disparities at national and European levels. Improvements have been seen over the past few decades in both health status and living and working conditions has widened tremendously in the European Union (EU) and will continue to do so as it goes through the enlargement process. The diversity in living conditions has translated into diversity in patterns of health across the region. Inequalities in income, education, housing and employment affect population health, both directly (for example, good housing reduces risks associated with poor health) and indirectly through psychosocial factors (such as stress). From the life course perspective, individuals are affected by different sets of risks related to disease and illness; certain diseases and causes of health are more likely to affect young people, whereas the majority are associated with older ages. Investigating differences in health status within and between European countries provides the focus of this report. The relationship between living conditions, socioeconomic factors and health is discussed and analysed with the objective of stimulating a debate and policy action for creating a healthier and more equitable society. We aim to present an overview of key issues and not comprehensive literature review or exhaustive analysis of the topics involved.

Total health expenditures have increased in almost all countries, especially in recent years, and with revenues not keeping pace, huge debts have accumulated in the health sector. Efficiency gains have been few and far between, and with the dynamic nature of technology and demographic changes increasing the complexity of health services and the health marketplace, further reforms are becoming even more difficult. This report takes stock of recent trends in health expenditure aggregates in the public sector and identifies specific areas of health expenditure reform consistent with the objectives of stabilizing the fiscal situation in these countries (the Czech Republic, Estonia, Hungary, Latvia, Lithuania, Poland, Slovakia, and Slovenia) without adversely affecting the production, delivery and utilization of health services.
