The global financial crisis that began in 2008 compounded the burden of high food prices, exacerbating the problems of hunger and malnutrition in developing countries. The tandem food price and economic crises struck amidst the massive, chronic problem of hunger and undernutrition in developing countries. National governments and international actors have taken a variety of steps to mitigate the negative effects of increased food prices on particular groups. The recent abrupt increase in food prices, in tandem with the current global economic crisis, threatens progress already made in these areas, and could inhibit future efforts. The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions. The compounding effects of the current global economic downturn on nutrition motivated additional discussions on these dual crises, their impacts on the nutritional status of vulnerable populations, and opportunities to mitigate their negative nutritional effects.

The primary estimation strategy adopted is the semi-parametric difference-in-difference with propensity score matching technique. The analysis finds that household health and nutritional behavior indicators do not vary statistically between households that were crisis-affected and households that were not affected by the crisis. However, the analysis finds that crisis-affected poor families curtailed their out-of-pocket health expenditures during and after the crisis more than poor families that were not affected by the crisis did. In addition, crisis-affected vulnerable groups changed their health behavior. In particular, households with low educational attainment of household heads and households with more elderly people changed their health and nutrition behavior response when affected by the crisis. The results are invariant to the propensity score matching techniques and parametric fixed effects estimation models.

This phenomenon is observed globally, not only in low- and middle-income countries but also in high income countries. The report also shows that the world is not on track to meet global nutrition targets, including those on low birthweight and on reducing stunting among children under five years. Moreover, overweight and obesity continue to increase in all regions, particularly among school-age children and adults. The report stresses that no region is exempt from the epidemic of overweight and obesity, underscoring the necessity of multifaceted, multisectoral approaches to halt and reverse these worrying trends. In light of the fragile state of the world economy, the report presents new evidence confirming that hunger has been on the rise for many countries where the economy has slowed down or contracted. Unpacking the links between economic slowdowns and downturns and food insecurity and malnutrition, the report contends that the effects of the former on the latter can only be offset by addressing the root causes of hunger and malnutrition: poverty, inequality and marginalization.

The healthcare becomes more and more subject to higher technologies with a growing number of private, well organized and benefit-oriented hospitals. Researchers found that approximately 50% of health improvements were due to access to better technology, whereas remaining gains resulted from income improvements and better education. Healthcare is actually referenced to as one of the most complex, costly and challenged sector of our contemporary societies. Just as health conditions at any time improve with income level, so too, might adverse income shocks have detrimental effects on health. Economic downturns have been linked to increased morbidity and mortality, and to poor nutrition and mental health. The recent economic crisis through the increasing socioeconomic disparities and difficulties such as unemployment, extreme poverty, homelessness, stigma, discrimination and social isolation and through the budgetary constraints and poor policies for financing prevention and treatment, have been translated to heightened risk behaviors on the individual level and impaired public health response on the population level. The negative impacts can also be observed at the societal level, as all sensitive social indicators have been distorted. In European countries, the late 2000's economic crisis laid to a consistent tendency of European governments to diminish the spending on healthcare. This have led to considerable decrease in the number of people accessing healthcare as in countries with weak health insurance policy people had to use money from their pocket. The economic crisis provokes certainly the need for multifactorial adjustments at all health managerial levels. To powerfully address this issue health managers should have clear understanding of the impacts of economic crisis on their ability to maintain and promote an adequate healthcare provision. The present paper will discuss indeed, the main challenges that a health services manager can face regarding the impact of an economic crisis to a local society.

But questions remain on how best to encourage proper nutrition. The recent global financial crisis has raised concerns about malnutrition and food supplies, but still lack data on the effects and repercussions. The Indonesia study, while it looks at a previous crisis, provides useful lessons into how governments and policy experts can work to support proper mother and child nutrition during times of economic crisis. Although Indonesia had the advantage of an already-existing network of local health clinics and village midwives, who were experienced in handling community health programs, this study makes clear that a targeted feeding program to provide children with special snacks can effectively protect healthy growth. Good nutrition is important to protect and encourage good early childhood development, which is key to giving children the best mental and physical start in life. In turn, children can grow into healthy and productive adults, who themselves have healthy children. Policymakers and development experts concerned about helping support the most vulnerable populations, especially during crises, may want to consider supplementary nutritional feeding programs for pregnant women and young children in order to promote and protect healthy development.
