Book excerpt: Background: Recent studies indicate a rise in the prevalence of obesity in children of various age groups. Obesity was officially recognized as a disease by the American Medical Association (AMA) in June 2013. It has been acknowledged as a progressive epidemic public health crisis in the United States for the past 20 years. This is a health care concern that needs examination and the development of new and more effective prevention and treatment modalities for obesity in children. Environmental, hereditary, behavioral, and socioeconomic factors play a significant role in the prevalence of obesity (Rooney, Mathiason, & Schauberger, 2011). Childhood obesity can generate serious emotional and physical consequences, thereby compromising the quality of life among children. Purpose: The purpose of this capstone project was to assess health care providers' cultural competency by assessing the management of Hispanic parents with children diagnosed with obesity. A culturally diverse survey was developed to evaluate pediatric health care providers' daily practice and management of Hispanic parents of children with obesity. Theoretical Framework: Madeleine Leininger's Theory of Culture Care Diversity and Universality and the Health Belief Model constitute the theoretical framework of this capstone project. Methods: The research project gained support from the private pediatric primary care office for the completion of a questionnaire to assess health care providers' knowledge of childhood obesity. Results: The data analysis revealed that the health care providers surveyed had at times omitted the evaluation of parameters that were vital in managing the health care of obese or overweight children. Certain parameters, such as weight, were always considered. The data analysis revealed gaps in the care of Hispanic pediatric overweight or obese patients and Hispanic caregivers. As a result of the findings, a better understanding of the importance of providing culturally competent health care was gained. Conclusion: This survey increased health care providers' awareness of the importance of taking culture into consideration. It identified the gaps, and measures are now being evaluated to improve the quality of care provided to these children.

Book excerpt: This graduate project evaluated The Whole Child program in order to better understand whether the program components effectively and equitably addresses the prevalence of obesity in the Hispanic community. The Whole Child (TWC) is a grant funded program in the Service Planning Area (Spa 7) in Los Angeles, providing mental health, housing, parent enrichment and nutrition services in a predominantly Hispanic community. The prevalence of obesity in children within the Hispanic community has been increasing rapidly in Los Angeles County. Hispanics in Los Angeles bear a disproportionately higher incidence and prevalence of being overweight and obese, and also have higher rates of diet related chronic disease. Research has demonstrated that health and nutrition education programs are effective in decreasing the incidence of diet related chronic disease in children, however, evaluating cultural relevance, efficacy of early intervention, and effects of socioeconomic status for Hispanic families are key components for program success. TWC was evaluated to better understand how effective addressing whole health is in supporting the nutritional health of children in this marginalized community. The researchers utilized the Logic Model to identify outputs, outcomes, external factors and assumptions of the program in conjunction with short-term and long-term projected goals. Research implications and recommendations for future programs were identified.

Book excerpt: In the past 10 years, obesity rates have increased at an alarming rate among children ages 2-5. Hispanic populations are especially susceptible to obesity. Surveillance data complied by the Center for Disease Control (CDC) Pediatric Nutrition Surveillance System (PNSS) on children participating in federally funded health and nutrition programs revealed among low-income, preschool-age children, the obesity prevalence remained at 14.6% in 2008. The research indicates a multitude of social and physiological factors affect childhood obesity. Parenting factors influencing obesity in children include level of physical activity, healthy nutritional resources, involvement in social activities, and achievement in educational settings. These factors are influenced by the cultural practices, beliefs, and values passed on from one generation to the next. Few studies have involved understanding cultural practices affecting parents and caretakers as they strive to raise healthy Hispanic children. Studies conducted along the U.S/Mexico border are few to none. Through a qualitative descriptive study using Leininger's Culture Care Diversity and Theoretical Model, focus groups were conducted, using five interview questions to explore perceptions on obesity prevention. The sample included 37 adult Hispanics presently responsible for the care of healthy preschoolers. Demographic data provided a rich, in-depth pictorial of the participants. Themes and sub-themes emerged from the transcripts and were confirmed by the key informant. Patterns of similarities and differences were validated through transcriptions in both English and Spanish. The themes ultimately corresponded with Leininger's Sunrise Model dimensions. The themes abstracted from the data analysis were: 1. General agreement existed among caretakers about requirements for physical activity and balanced nutrition: however, methods used to meet those requirements differed. 2. Participants presented a variety of useful resources with information which assisted them raise their children in a healthy manner. 3. The contrast between urban and rural living environments provided challenges to these caretakers' attempts to raise healthy children. Implications for nurses providing culture care and its inclusion in nursing education were discussed.

Book excerpt: Abstract: Background: According to the National Center for Health Statistics, 15.3% of all American children between the ages of 6 and 11 are overweight and the prevalence of overweight children is disproportionately higher among Mexican-American children (1). The percent of overweight Mexican-American children ages 6 to 11 is 23.7%, significantly higher than the 11.8% for non-Hispanic whites (1). Overweight and obesity lead to an increase risk of many severe health problems and morbidities including diabetes, high blood pressure, and heart disease. Jump into Food and Fitness (JIFF), an intervention program targeted for elementary school-aged children, was developed by the 4-H Youth Development team at Michigan State University Extension is designed to educate children ages 8 to 11 about healthy food choices, nutrition, and physical fitness, and is modifiable for younger or older children (2). JIFF was not designed specifically for Latino children, and its implementation to the Latino community is a recent application in community nutrition research. Purpose: The purpose of this research is to measure the success of the intervention with a group of Latino children, as well as to develop and pilot an evaluation tool for this intervention. Procedure: In order to investigate the impact of this intervention in Latino children, the program has been applied to a group of children whose parents attended free nutrition and exercise classes through Centro Esperanza Latina on the West side of Columbus. The children and their parents were a part of another research project conducted by researchers at The Ohio State University. For seven weeks the primary investigator, with the help of a regular childcare provider, applied JIFF to a group of 12 children for once a week for one hour. The piloted evaluation tool included a pre and post food behavior and physical fitness questionnaire created by the primary investigator. It also included seven individual evaluations to be conducted before and after each lesson in order to measure the impact of each individual lesson. The evaluations included asking questions and conducting activities for the children in order to measure acquired knowledge and skills, as well as behavior change. The classes, handouts, and parent newsletters were translated into Spanish. Findings: The piloted evaluation tools were successful in measuring acquired knowledge and short-term behavior change for this sample population. Variations of the evaluation tool may also be successful in measuring long-term behavior change with future implementations of other curriculum. The evaluation tools indicate that the sample populations did not consistently retain knowledge acquired during each lesson of the JIFF curricula and pre and post behavior change was inconsistent for this sample. Conclusions: Education through school, after school programs, or other types of intervention could benefit the health of many overweight children; however, evaluation of these programs is critical. The evaluation tool could potentially be used for assessment of the impact of JIFF on other populations and variations of the piloted evaluation tool could also be used to measure the success of a different health intervention targeting children.

In many communities, for example, fresh produce is not available or affordable, streets and parks are not amenable to exercise, and policies and economic choices make fast food cheaper and more convenient than healthier alternatives. Community efforts to combat obesity vary in scope and scale; overall, however, they remain fragmented, and little is known about their effectiveness. At the local level, communities are struggling to determine which obesity prevention programs to initiate and how to evaluate their impact. In this context, the Institute of Medicine held two workshops to inform current work on obesity prevention in children through input from individuals who are actively engaged in community- and policy-based obesity prevention programs. Community perspectives were elicited on the challenges involved in undertaking policy and programmatic interventions aimed at preventing childhood obesity, and on approaches to program implementation and evaluation that have shown promise. Highlights of the workshop presentations and discussions are presented in this volume.
