For children who have already been removed from the care of their biological parents, additional caregiver changes can have negative effects. There is evidence that foster children who change placements frequently are more likely than their peers to suffer from emotional and behavioral problems. While researchers have found that children who change placements have an increased likelihood of using outpatient mental health services, little is known about the relationship between placement change and use of crisis or inpatient psychiatric services. The aim of the present study was to examine the effects of placement change during the first 90 days in foster care on utilization of crisis services and inpatient psychiatric treatment among a sample of 37,699 Californian children and youth who started a new foster care spell between October 1998 and March 2001. The results suggest that the relationship between placement change and use of crisis and inpatient psychiatric services is bi-directional and is dependent on the type of placement change. These findings suggest that foster children with psychiatric problems may be more likely to have certain types of placement changes, and that these changes can be detrimental for their mental health.

Children who enter the foster care system have experienced abuse in the home, which has caused them to develop mental health disorders. Due to gaps in services provided to youth in the child welfare system, Los Angeles County and the State of California was involved in a class action lawsuit in 2002. This lawsuit challenged the State's neglect to deliver mental health services to foster children. The literature review aimed to understand the consequences of inadequate mental health services for foster youth and the harmful effects of multiple foster care placements. John Kingdon's agenda-setting framework was applied to illustrate the child welfare system's response to the Katie A vs. Bonta class action lawsuit. Additionally, this project defined the adverse outcomes of insufficient mental health services and multiple placements on foster youth who age out of foster care. Limitations were present in this study due to time constraints, and recommendations for further research were proposed.

Question: Does foster care have a significant impact on youth in terms of emotional, behavioral or developmental conditions? Methods: Secondary data was utilized for this researcher to determine the impact (or lack thereof) that the welfare system has on California's youth currently in foster care. Results: Overall, 13% of the California youth population have been diagnosed with an emotional, behavioral or developmental condition. Additionally, 47.9% of the United States youth involved with the child welfare system have significant emotional or behavioral problems. Conclusion: Foster care has a significant impact on youth in terms of being diagnosed with an emotional, behavioral or developmental condition. Additionally, 53% of California foster youth leave foster care due to reunification, adoption or emancipation. And while African American youth make up only 6% of the California population; they make up 21% of the population of foster youth which is about 3.5 times more compared to Whites and Hispanics.

Book excerpt: Children involved with the foster care system after experiencing parental neglect or abuse are known to face numerous physical, mental, and developmental challenges. Compounding these concerns, youth with a history of maltreatment experience toxic stress that can extend into adulthood. In light of these health risks, timely and thorough physical and mental health care is critical to their well-being. Health care utilization among children in foster care (i.e., out-of-home care), however, is often inconsistent and fragmented, varying based on factors such as a child’s age, gender, race/ethnicity, out-of-home caregiver, and specific health needs. The long-term, predictive effects of these child-level predisposing, enabling, and need characteristics on physical and mental health service use among children in extended foster care was investigated for this thesis research. This was a retrospective longitudinal analysis of data from the second National Survey of Child and Adolescent Well-being. The sample consisted of 451 children who were zero to 17 years old and were in out-of-home care during the 36-month study period. Generalized estimating equations were used to construct population-averaged models while specifying a within-subject working correlation matrix to yield unbiased regression estimates. Younger age and placement with a non-kin caregiver were positively associated with physical health care utilization. The caregiver effect was modified by race/ethnicity; Hispanics residing with non-kin caregivers had the highest adjusted odds of physical health service use. Male gender independently predicted greater mental health care utilization. Older age and clinically significant scores on the Child Behavior Checklist were associated with more mental health service use, but their effects varied by race/ethnicity. White children with scores above the clinical cut-off and older children of Other background had the highest adjusted odds of mental health care utilization. Clinical and non-clinical factors impact longitudinal physical and mental health care utilization among children in foster care. A patient- and family-centered, medical home approach may reduce service barriers by increasing communication and collaboration between caregivers, caseworkers, and healthcare providers. Further, future research with foster youth would benefit from following a standard measure of health need and service use to disentangle causes of healthcare disparities.
