The background section covers the history, diagnostic criteria, and epidemiology; other sections cover the neurobiological basis, clinical features, laboratory investigation and differential diagnosis, and treatment and rehabilitation. Especially noteworthy are the details of an approach to managing patients who need respiratory support, which has reduced mortality from 25% to 4% over the past couple decades. Book excerpt: The Guillain-Barre syndrome (GBS) or Landry-Guillain-Barre-Strohl syndrome, also known as post-infectious polyneuropathy or acute idiopathic polyneuritis, is an acute acquired, frequently severe, monophasic autoimmune illness of the peripheral nervous system (PNS). GBS manifests itself with the clinical picture characterised by gait disturbance, pain, weakness, rapidly ascending symmetric flaccid muscle paralysis, areflexia with distal predominance (involving lower motor neuron), sensory disturbance, variable autonomic involvement, and increased cerebrospinal fluid protein without pleocytosis . Although GBS had previously been viewed as a unitary disorder with variations, it is today considered as a group of syndromes with several distinctive variants or subtypes. The aim of this book is to describe and discuss this disease that is not exactly rare but is almost the only inflammatory polyneuropathy and the most frequent cause of acute flaccid paralysis in general medical practice.

Book excerpt: In 1976, a small group of soldiers at Fort Dix were infected with a swine flu virus that was deemed similar to the virus responsible for the great 1918-19 world-wide flu pandemic. The U.S. government initiated an unprecedented effort to immunize every American against the disease. While a qualified success in terms of numbers reached-more than 40 million Americans received the vaccine-the disease never reappeared. The program was marked by controversy, delay, administrative troubles, legal complications, unforeseen side effects and a progressive loss of credibility for public health authorities. In the waning days of the flu season, the incoming Secretary of what was then the Department of Health, Education and Welfare, Joseph Califano, asked Richard Neustadt and Harvey Fineberg to examine what happened and to extract lessons to help cope with similar situations in the future.

Available in PDF, EPUB and Kindle. Book excerpt: Guillain-Barre Syndrome (GBS) is a disorder in which the body's immune system attacks part of the peripheral nervous system. Feelings of tingling and weakness increase in intensity until the muscles cannot be used at all and the patient is almost totally paralyzed. No one knows why Guillain-Barre strikes some people and not others, or what sets the disease in motion. What we do know is that GBS is now the most common cause of acute paralysis in Western countries since the virtual elimination of poliomyelitis with vaccination programs. Guillain-Barre Syndrome, co-authored by a survivor of this illness, addresses all aspects of this condition, including initial symptoms, diagnostic evaluation and criteria, general and illness specific treatments, and typical outcomes. Fortunately, most GBS patients have a good recovery, but the pathway to this goal can be slow and scary. This book covers a wide range of issues including: Features unique to GBS such as pain without injury and other abnormal sensations Workplace adjustment with incomplete recovery Intensive care unit management Practical caregivers guidelines The rehabilitation process Sexual dysfunction, and much more Guillain-Barre Syndrome is a comprehensive book, written in lay terms, covering everything from diagnosis to emotional issues. It is a book that stands alone. This latest volume in the American Academy of Neurology Press Quality of Life Guide series is an essential tool for all individuals, families, and caregivers coping with Guillain-Barre Syndrome.

This book was released on 2009 with total page 8 pages. Available in PDF, EPUB and Kindle. Book excerpt: Background: Guillain-Barre Syndrome (GBS), the leading cause of acute flaccid paralysis worldwide, is an autoimmune disorder involving the loss of the myelin sheaths encasing peripheral nerve axons, leading to a loss of nerve signaling and typically ascending paralysis. A number of infectious triggers have been identified, with Campylobacter being most common. Limited data are available regarding GBS in U.S. service members at a high risk of exposure to numerous GBS-associated infectious agents. Findings: Medical encounter data were obtained from the Armed Forces Health Surveillance Center (Silver Spring, MD). Active duty personnel with an incident GBS diagnosis were matched by age, sex, and time with up to 4 controls. Demographic, antecedent infectious gastroenteritis (IGE), and deployment covariates were used to explore GBS risk in this population. The overall incidence was 2.28/100,000 persons (95% confidence interval: 2.03-2.54) with 19.1% (60/314) receiving GBS-related medical care for more than one year. The majority of cases were male, Caucasian and under 25 years of age. There was an increased risk of GBS three months following a documented episode of IGE (Odds Ratio: 5.33; p = 0.03). We also found an association with service in the Air Force and Navy (compared to Army personnel) with odds ratios of 1.39 (p = 0.05) and 1.44 (p = 0.02), respectively. Conclusion: GBS incidence in the U.S. military is slightly higher than the general population and is associated with an antecedent IGE. Future studies are warranted to assess whether there are GBS-associated infectious or environmental exposures inherent to military populations.

Book excerpt: Childhood immunization is one of the major public health measures of the 20th century and is now receiving special attention from the Clinton administration. At the same time, some parents and health professionals are questioning the safety of vaccines because of the occurrence of rare adverse events after immunization. This volume provides the most thorough literature review available about links between common childhood vaccinesâ€"tetanus, diphtheria, measles, mumps, polio, Haemophilus influenzae b, and hepatitis Bâ€"and specific types of disorders or death. The authors discuss approaches to evidence and causality and examine the consequencesâ€"neurologic and immunologic disorders and deathâ€"linked with immunization. Discussion also includes background information on the development of the vaccines and details about the case reports, clinical trials, and other evidence associating each vaccine with specific disorders. This comprehensive volume will be an important resource to anyone concerned about the immunization controversy: public health officials, pediatricians, attorneys, researchers, and parents.
